Specifically, injections to treat zones of high contaminant concentrations in soil were performed in accordance with Injection Permit WI0400231 issued by North Carolina Department of the Environment and Natural Resources (NCDENR) on February 29, 2012, and as specified in the Remedial Action Plan (RAP), and Preconstruction Report certified complete on October 9, 2012, and November 5, 2012, respectively. This Construction Completion Report has been prepared to meet the requirements of NCDENR rules and regulations under the Division of Waste Management, Inactive Hazardous Sites Branch, Registered Environmental Consultant (REC) program (Title 15A, North Carolina Administrative Code [NCAC], 13C.0300 Voluntary Remedial Action Oversight by REC [15A NCAC 13C.0300]). The Administrative Agreement for the Kidde facility was executed June 7, 2007, and Division of Waste Management assigned the facility number NONCD 000 1103. Following a brief review of the Site's background, this report summarizes November 2012 remedial injection activities and presents associated final as -built figures. The forthcoming performance monitoring program and schedule are also defined herein. Required REC program work phase completion statements are included in Appendix A. 1.1 Site Description The Site is approximately 60 acres, of which 35 acres are developed (Figure 1-1). Distribution Center (DC)-3, the former main plant building constructed in 1972, is approximately 125,000 square feet. Supporting structures in the vicinity of DC-3 include propane aboveground storage tanks (ASTs), a guard shack, an employee parking lot, and a duck pond. In 2000, DC-4, an open -front environmental shed, was constructed to the north of DC-3. DC-4 historically housed finished cylinders, various production chemicals and wastes, and a 6,000-gallon (oily) wastewater AST. A bermed area, constructed as a waste oil/hazardous waste storage area in the early 1990s, is west of DC-4. Use of the bermed area was discontinued around 2000. East of DC-4 is an open grassy area where underground storage tanks (USTs)/ASTs, with unknown contents, were reportedly formerly located. The approximate 90,000 square foot DC-2, a former manufacturing building, is north of DC-4. This building was constructed in 1978, with renovations/additions in 1980. East-northeast of DC-2 is the Former Fire Extinguisher Testing Area and north of DC-2 is the New Fire Extinguisher Testing Area (NTA). An approximate 2-acre pond, to the north of the NTA, receives runoff from the surrounding area. In 1972, Kidde constructed an industrial fire extinguisher manufacturing plant, which operated until 2008. Kidde currently uses the Site as a logistics center for repacking, storage, and distribution of fire extinguishers. Kidde leases additional office space at 1016 Corporate Drive, an adjoining parcel to the west. 1.3 Nature and Extent of Contamination Based on previous investigations, results of which are detailed in the RAP, various historical releases in the vicinity of the former DC-2 manufacturing building have contaminated soil and groundwater. COCs consist of the following chlorinated volatile organic compounds (CVOCs): tetrachloroethene (common name perchloroethylene or PCE), 1,1-dichloroethene (1,1,-DCE), and 1,1-dichloroethane; and the solvent stabilizer, 1,4-dioxane, which is comingled with the CVOC plume. These COCs are present in unsaturated soil and groundwater beneath the southeastern corner of DC-2, and in shallow, top -of -bedrock, and bedrock groundwater downgradient of DC-2, extending to the eastern property line. Remedial injection activities were designed to address the highest concentrations of COCs at the source area beneath the southeast corner of DC-2 and in groundwater moving immediately downgradient from this area. 1-3 E tISAFE Construction Completion Report Walter Kidde Portable Equipment, Inc. —Mebane, North Carolina Revision: 1 February 5, 2013 2.0 FIELD ACTIVITIES EnSafe and Innovative Environmental Technologies Inc. (IET) injected remedial substrates into the subsurface from November 5 to November 13, 2012, to stimulate in -situ biotic and abiotic reductive dechlorination of CVOCs. Eighty-eight injection points delivered a total of 30,320 gallons of substrate to soil and groundwater source areas with the highest concentrations of COCs, located beneath and adjacent to DC-2. Emulsified Oil/Zero Valent Iron (EZVI) was injected into the unsaturated soil source area from 10 to 25 feet below ground surface (bgs), and a ZVI/EHC° (a commercial carbon substrate and ZVI formulation)/Calcium Propionate/Hydrolyzed Kelp/nutrient solution was injected into the groundwater matrix source area from 25 to 50 feet bgs. Final injection grids are shown on Figure 2-1. Such activities are summarized in text below. Additional details, including injection logs documenting volume deliverable per point, are included in IET's field injection report, included as Appendix B. 2.1 Survey In preparation of injection activities, grids and injection points were located and flagged by Taylor Wiseman & Taylor, a professional land surveyor in North Carolina. Additionally, Taylor Wiseman & Taylor, used ground penetrating radar and industry standard tools to confirm each injection point was clear of any subsurface utilities. 2.2 Injection Procedure Injectants were delivered to the subsurface by a direct push technology (DPT) drill rig (Geoprobe 6610 and/or AMS VTR-9500) equipped with IET's patented tooling. Specifications for the tooling and injection process are provided in Appendix B. After coring through the concrete or asphalt surface, tooling was driven to the prescribed depth. Next, nitrogen gas was introduced at approximately 20 psi for EZVI or 175 psi for the ZVI/EHC°/Calcium Propionate/Kelp/nutrient substrate. The intent of the nitrogen injection process was to initiate a limited degree of horizontal pneumatic fracturing of the formation around the injection point, to enhance distribution of reagents in the subsurface. Once such a pathway was established, the substrate was injected at a rate of 5 to 8 gallons per minute. Next, a second pulse of nitrogen gas was introduced to clear the lines and further force fluids into the stratum. This process was repeated at each prescribed injection depth and after final injection, a shut-off valve was closed and residual pressure was monitored until a safe level was reached. Finally, injection tooling was removed, the boring was abandoned using hydrated bentonite, and the concrete/asphalt surface was repaired. Table 2-1 provides details of the soil source area injection, including depths and quantity of substrate. No changes were necessary to source area injection specifications, as originally presented in the RAP. At the other grids, a limited volume of substrate re -surfaced on the initial injection at one or more points. At such locations, the upper interval injection was immediately discontinued, and injection volumes into the lower intervals were increased to achieve the prescribed volume at each point. Field adjustments to volumes per depth interval are summarized in Appendix B. Remedial effectiveness is not anticipated to be affected by these minor adjustments. 2-4 ENSAFE Construction Completion Report Walter Kidde Portable Equipment, Inc. Mebane, North Carolina Revision: 1 February 5, 2013 3.0 PERFORMANCE/MONITORING PROGRAMS Remedial effectiveness will be monitored through the collection and laboratory analysis of soil and groundwater samples, as described below. Results will be evaluated versus historical data to gauge remedial performance, with findings documented in certified monitoring reports. 3.1 Soil Source Area Approximately one year following the remedial injection, effectiveness soil sampling will be performed at 15 borings across the soil source injection area (Grid A). Soil borings will be slightly offset from injection points. Samples will be collected from depths of 10 to 15 foot bgs, 15 to 20 foot bgs, and 20 to 25 foot bgs, at each boring, and submitted for laboratory analysis of volatile organic compounds (VOCs). The need for supplemental injection and/or additional effectiveness monitoring for soil will be based on year one results. 3.2 Groundwater Source Area Per REC program guidance, effectiveness groundwater sampling will be performed quarterly during the initial year following the remedial injection. The program will include sampling of the following 12 wells: MW019, MB019, MW033, MW042, MW043, MW044, MW045, MW047, MW048, MW049, MW050, and MW052. Effectiveness groundwater samples will be analyzed for VOCs, sulfate, nitrate, chloride, dissolved gases (methane, ethane, and ethene), hydrogen, total iron, total manganese, total organic carbon, alkalinity, and the following field parameters: pH, temperature, specific conductivity, dissolved oxygen, oxidation reduction potential, turbidity, ferrous iron, and sulfide. Components of the effectiveness monitoring program for year two and beyond, including frequency, wells to be sampled, and analyte list, will be based on year one results. Monitoring wells are presented on Figure 3-1. 3.3 Site -Wide Groundwater Downgradient of the source injection remedy, groundwater will also be sampled to gauge contaminant trends. Such samples will be analyzed for VOCs and 1,4-dioxane during the first year following injection. The site -wide monitoring program, for year two and beyond, will be based on year one results. This report has been prepared to document remediation activities conducted at the site. The injection program was implemented between from November 5th to November 13th, 2012. IET implemented a reductive dechlorination program designed to enhance both abiotic and biological reductive processes (U.S. patent #7531709) in 7 treatment areas encompassing approximately 17,500 sq. ft. from between 25-50 feet below ground surface (bgs). IET also implemented an EZVI injection in the shallow soils within an area that encompassed approximately 2,500 square foot targeting the interval from 10-25 feet bgs. IET completed all direct push points in the locations proposed and injected 25,200 gallons of solution including 24,640 pounds of ZVI, 22,401pounds of EHC, 1,680 pounds of nutrient, 11,199 pounds of calcium propionate, 4,929 pounds of Hydrolyzed Kelp, 1,680 pounds of yeast extract and 1,680 pounds of sodium sulfite. Any minor adjustments to volumes and depths in each of the individual points are reflected in the attached logs. In the EZVI treatment area 5,120 gallons of EZVI were injected in the targeted intervals with no surfacing issues. Any adjustments to volumes and depths in individual injection points are reflected in the attached EZVI field logs. INTRODUCTION The site located in Mebane, NC, was identified by ENSAFE, their consulting engineer, as having soils and groundwater impacted by the historical release of chlorinated solvents. The primary compounds of concern for this phase of remediation at the site were CVOCs. An injection program was proposed by IET for the remediation of residual levels of VOC impacts at the site utilizing reductive technologies. REMEDIATION IMPLEMENTATION 1.1 INJECTION PROCEDURES IET completed a total of 464 injections, via 88 injection locations at the site in Mebane, NC. Direct -Push -Driven Perforated Rod Placement A Geoprobe 6610 unit comparable to the one pictured below and an AMS VTR-9500 were utilized to drive the injection screens (as seen in Figure 2) to depths ranging from 10 to 50 feet bgs across the site. Figure 2: IET Injection Tooling Injection Technique Subsurface Pathway Development Compressed nitrogen gas was used to propel all injectants into the subsurface. Nitrogen gas was first injected into the subsurface at approximately 150 pounds per square inch (psi) until a significant pressure drop was observed at the injection pressure vessel. This process is referred to as pre -injection pathway development. The intent of this step is to open pathways in the subsurface for the injectants to follow. These pathways are believed to be the more permeable pathways along which CVOCs are likely to have migrated. Liquid and liquid -entrained injectants are then delivered with pressurized gas to the pathways that were produced during this step. Synergistic Reductive Dechlorination Technology Injection A sodium sulfite solution was injected to remove oxygen from groundwater and soil moisture. This will facilitate the anaerobic conditions that are preferred for the reductive dehalogenation of chlorinated hydrocarbons by indigenous bacteria. Groundwater mounding was also promoted by completing series of injections in localized areas over a relatively short period of time. Yeast extract was included in the injection slurry in order to provide essential nutrients to subsurface microorganisms for reproduction and to facilitate the reductive chlorination process. Hydrolyzed kelp, vitamins B2 and B 12, nutrient was also included in the slurry for similar purposes. ZVI, in the absence of oxygen, was injected to rapidly reduce higher concentrations of dissolved phase CVOCs via direct contact chemical reactions. ZVI can continue to react with dissolved CVOCs in groundwater until it is completely oxidized by CVOCs, oxygen, or other oxidants that contact residual concentrations of ZVI. Rinse Water was injected to clear the injection lines of any residual liquids. Again, this also promotes mounding of groundwater into the vadose zone. Post Injection Line Purge Nitrogen gas is injected to clear the lines of remedial liquids before moving to the next interval and/or injection location. 1.2 Direct -Push Injection Areas Treatment Areas The 88 injection locations in treatment areas A through G were completed as designed using the Geoprobe 6610 and/or an AMS VTR-9500. Due to surfacing issues during the injection of shallow injections in the outside areas E, F and G, the upper intervals of the injection point were commonly bypassed and the lower intervals were doubled in order to limit the amount of material lost to surfacing. Shallow intervals were repeatedly attempted, but due to large cracks in the concrete pad in the areas located in the loading dock, they were bypassed due to nitrogen influence at the surface. Both top down and bottom up retractable tooling was attempted in these areas. A detailed description of the surfacing issues encountered for these areas can be viewed in the attached field logs. Field adjustments were required in order to limit daylighting of material in Areas F and G. IET encountered tight formations from the 45-50' injection interval but, was able to pulse nitrogen and the slurry in order to deliver the necessary volumes into the formation. The injection program implemented in November 2012 was a success and looks forward to evaluating the remedial process along with ENSAFE throughout the duration of the sampling schedule.
