STORMWATER MANAGEMENT PLAN MODIFICATION APPROVAL
Dear Mr Hughes
On January 12 2007 the Division of Water Quality (DWQ) issued a 401 Water Qualrty Cerhfication
Approval to Umon County Public Schools to impact 529 lmear feet of stream and 0 143 acre of isolated
wetland for the purpose of constructmg Elementary School G in Umon County Condihon 9 of the 401
Water Quality Cerhiication requires that the pro�ect adhere to the stormwater management plan that was
approved as part of the certification
On November 29, 2011 the Division of Water Quality (DWQ) received a revised stormwater
management plan (dated November 21, 2011) from your consultant Christopher Estes of Estes Design
Inc The November 29 2011 submittal requests permission to razse the exishng outlet structure and
reconstruct the forebay berm The DWQ has reviewed the revised stormwater management plan and
approves the proposed modificahon to meet Condition Number (9) in the Water Quality Certificahon
Tlus approval is for the purpose and design descnbed m your applicarion and 401 Certificarion If you
change your pro�ect you must notify us and you may be requ�red to send us a new applicarion If the
property is sold the new owner must be given a copy of tlus approval letter and is thereby responsible for
complying with all condit�ons This approval requires you to follow the condihons hsted m the
401Water Quahty Cerhfic�tion for your pro�ect and the followmg addihonal condihons hsted
below
The SMP approved by this office consists of two bioretenhon cells and one wet detenhon pond as
stormwater best management practices (BMPs) all associated stormwater conveyances inlet and
outlet structures No changes to the structural stormwater prachces shall be made without wntten
authorizat�on from the DWQ The stormwater management structures shall have mamtenance
access and at least a 10 foot dramage easement on each side to allow for the ability to maintain
the structures and to perform correchve actions The stormwater easements shall be recorded to
provide protection of the structures from potential alterations by future properly owners
2 Oneration and Mamtenance of BMPs The approved 5MP must be maintained in perpetuity in
accordance wrth Operation and Maintenance (O & M) agreements signed by you and dated
Wetlands Buffers Stormwater Compliance and Permitting Unit (WeBSCaPe)
