JARRELL, PE — CHIEF OF OPERATIONS CHARLOTTE WATER 4222 WESTMONT DRIVE CHARLOTTE, NORTH CAROLINA 28217 Dear Ms. Jarrell: Subject: Permit No. WQ0021734 Franklin WTP Reclaimed Water Bulk Distribution System Mecklenburg County In accordance with your permit application renewal" request received October 13, .2015, and subsequent additional information received November 10, 2015 and January 7, 2016, we are forwarding herewith Permit No. WQ0021734 dated March 24, 2016, to Charlotte Water for the continued operation of thesubject reclaimed water bulk distribution system. Please note that this renewed permit shall become effective on May 1, 2016 (i.e. the day after the expiration date of the existing permit). Modifications to the subject permit are as follows: ➢ Your permit's format has been updated to :be consistent with the most current reclaimed water permit template, and rule references have been updated pursuant to the new 15A NCAC 02U rules that became effective July 18, 2011. This permit shall be effective from May 1, 2016 until April 30, 2021, shall void Permit No. WQ0021734 issued May 2, 2007, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements listed in Attachments A and D, for they may differ from the previous permit issuance. Failure to establish an adequate system for collecting and maintaining the required operational information shall result in future compliance problems. For your convenience, customized electronic copies of your facility's NDMR reporting forms are available for download at: http://portal.ncdenr.or web/wq/aps/lau/reporting. Please note the following permit condition has been removed/corrected since the last permit issuance dated May 2, 2007. ➢ Old Condition I.I. —This condition has been removed because the facility has been constructed and the Engineering Certification was received September 10, 2008. ➢ Old Condition I.2. — This condition has been removed because the start-up inspection was conducted June 3, 2008. State of North Carolina I Environmental Quality I Water Resources I Water Quality Permitting I Non -Discharge Permitting 1617 Mad Service Center I Raleigh, North Carolina 27699-1617 919 807 6464 2. This permit shall not relieve the Permittee of their responsibility for damages to groundwater or surface water resulting from the operation of this facility. [15A NCAC 02B .0200, 02L .0100] 3. Effluent limitations for generated reclaimed water shall not exceed those specified -in Attachment A. [15A NCAC 02U .0300] 4. Reclaimed water distributed through bulk fill stations may only be utilized -for the purposeslisted in Attachment D. [G.S. 143-215.1] 5. The conjunctive use of reclaimed water permitted herein in no way negates, precludes, or invalidates the most recent reissuance of the Permittee's NPDES permit (Permit No. NC0084549), and the Permittee shall continue to comply with all conditions provided for therein. [G.S. 143-215.1] The following shall be requirements for the reclaimed water bulk distribution facility: a. All reclaimed water piping, valves, outlets and other appurtenances shall be color -coded, taped or otherwise marked to identify the source of the water as being reclaimed water. i. All reclaimed water piping and appurtenances shall be either colored purple- (Le., Pantone 522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER — DO NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and repeated every three feet or less. ii. Identification tape shall be at least three inches wide and have white or black lettering on purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT DRINK." Identification tape shall be installed on reclaimed water pipelines in a visible manner, fastened at least every 10 feet to each pipe length and run continuously the entire length of the pipe. iii. Existing underground distribution systems retrofitted for the purpose of distributing reclaimed water shall be taped or otherwise identified as noted above. This identification need not extend the entire length of the distribution system, but shall be incorporated within 10 feet of crossing any potable water supply line or sanitary sewer line. b. , All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits operation by authorized personnel only. c. Hose bibs shall be located in locked, below grade vaults that shall be labeled as being of non - potable quality. As an alternative to the use of locked vaults with standard hose bib services, other .locking mechanisms such as hose bibs which can only be operated by a tool may be placed above ground and labeled as non -potable water. [15A NCAC 02U .0403] 7. No direct cross -connections shall be allowed between reclaimed water and potable water systems, unless such connection has been approved by the Department pursuant to 15A NCAC 18C .0406. [15A NCAC 02U .0403(f)] 8. The facilities permitted herein shall be constructed according to the following setbacks: a. The setbacks for reclaimed utilization sites shall be as. follows (all distances in feet): i. Surface waters not classified SA: 25 - ii. Surface waters classified SA: 100 iii. Any well with exception to monitoring wells: 100 [15A NCAC 02U .0701] WQ0021734 Version 3.0 Shell Version 151201 Page 2 of 7 M. OPERATION AND MAINTENANCE REQUIREMENTS 1. The reclaimed water bulk distribution facilities shall be properly maintained and operated at all times. The facilities shall be effectively maintained and operated as a reclaimed water system to prevent the discharge of any reclaimed water or partially treated effluent resulting from the operation ofthis facility. [15A NCAC 02T .0108(b)(1)] 2. The Permittee shall maintain an Operation and Maintenance Plan, which at a minimum shall include the following: a. Description of the system in sufficient detail to show what operations are necessary for the system to function and by whom the functions will be conducted; b. A map of all distribution lines and record drawings of all utilization systems under the Permittee's control; c. Description of anticipated maintenance activities; d. Include provisions for safety measures including restriction of access to sites and equipment; and e. Spill control provisions including response to upsets and bypasses including control, containment, remediation, and contact information for plant personnel, emergency responders and regulatory agencies. f. For bulk fill operations, the Permittee shall develop an operation and maintenance plan for bulk distribution activities which includes but is not limited to the following: i. Description of operation; ii. Description of anticipated maintenance; iii. Safety measures, including site restrictions; iv. Spill prevention and spill response plan; v. Contact information for plan personnel, emergency responders, and regulatory agencies; vi. Approval procedures of users; vii. Operational requirements for users; and viii.If reclaimed water is to be stored for greater than 72 hours prior to use, included a description of how compliance with reclaimed water quality standards will be ensured the time of use. [15A NCAC 02U .0801] Upon the Water Pollution Control System Operators Certification Commission's (WPCSOCC) classification of the subject non -discharge facilities, in accordance with 15A NCAC 08G .0200 the Permittee shall designate and employ a certified operator in responsible charge (ORC) and one or more certified operator(s) as back-up ORC(s). The ORC or their back-up shall visit the facilities in accordance with 15A NCAC 08G .0200, and shall comply with all other conditions specified in the previously cited rules. At the time of this permit issuance, a certified ORC is not required for conjunctive reclaimed water users. [15A NCAC 02U .0117] 4. Only reclaimed water generated at the Franklin WTP (Permit No. NC0084549) shall be utilized and/or distributed in accordance with this permit. [G.S. 143-215.1] 5. Effluent shall be discharged in accordance with Permit No. NCO084549 if the limit for fecal coliform (e.g., daily maximum concentration of 25 colonies per 100 mL) or turbidity (e.g., instantaneous maximum of 10 NTU) is exceeded, until such time as the reclaimed water standards are met at the generating facility. [15A NCAC 02U .0401(c)] 6. The Permittee shall provide notification to the public and/or employees about the use of reclaimed water, and that reclaimed water is not intended for drinking. Such notification shall be provided to employees in a language they can understand. [15A NCAC 02U .0501] WQ0021734 Version 3.0 Shell Version 151201 Page 3 of 7 7. Reclaimed water used for activities other than land application shall not be used in a manner that causes exposure to aerosols. [15ANCAC 02U .0501(b)(6)] 8. Reclaimed water shall not be used for swimming pools, hot tubs, spas, or similar uses. [15A NCAC 02U .0501(d)] 9. Reclaimed water shall not be used for direct reuse as a potable water supply. [15A NCAC 02U .0501(e)] 10. Bulk distribution operations shall meet thefollowing: a. The Permittee shall approve tank truck drivers for participation in the bulk reclaimed water distribution program prior to distribution of reclaimed water. In addition, the Permittee shall provide bilingual (English and Spanish) training to all tank truck drivers approved to participate in the bulk reclaimed water distribution program at a regular frequency. These sessions shall provide training on, but not limited to, the following topics: i. Reclaimed water description and safety; ii. Allowable and prohibited uses of reclaimed water; iii. Procedures for the application (including buffer maintenance), uses, and storage of reclaimed water as well as the disposal of unused reclaimed water; iv. Procedures for spill prevention, countermeasures, and control as well as notification requirements; v. Personal safety and hygiene issues; and vi. Interaction with the general public. b. Tank trucks used to transport reclaimed water shall meet the following requirements: i. Tank trucks and other equipment used to distribute reclaimed water shall be clearly identified with advisory signs. Signage may include but is not limited to "CAUTION: RECLAIMED WATER — DO NOT DRINK", "NON -POTABLE WATER", or a combination of similar phrases. ii. Tank trucks used to transport reclaimed water shall not be used to transport potable water that is used for drinking or other potable purposes. iii. Tank trucks used to transport reclaimed water shall not be filled through on -board piping or removable hoses that may subsequently be used to fill tanks with water from a potable water supply. Tank trucks owned and operated by the Permittee and dedicated to be filled by reclaimed water only may be filled through on -board piping. iv. Tank trucks shall not have been used to haul septage, biosolids, or other waste or chemical transport. v. Tanks trucks shall not have any type of removable container or portable bladder except if the tank/bladder is permanently labeled for reclaimed water use only and a letter from the truck owner is submitted to the Permittee indicating that in no case shall any removable container or portable bladder be used for both reclaimed and potable water at any time. [15A NCAC 02U .0601] WQ0021734 Version 3.0 Shell Version 151201 Page 4 of 7 I[V. MONITORING AND REPORTING REOUIRMENTS 1. Any Division required monitoring (including groundwater, plant tissue, soil and surface water analyses) necessary to ensure groundwater and surface water protection shall be established, and an acceptable sampling reporting schedule shall be followed. [15A NCAC 02T .0108(c)] 2. A Division certified laboratory shall conduct all laboratory analyses for the required effluent, groundwater or surface water parameters. [15A NCAC 02H .0800] 3. The Permittee shall monitor the reclaimed water from the generating facility at the frequencies and locations for the parameters specified in Attachment A. [15A NCAC 02T .0108(c)] 4. The Permittee shall maintain adequate records for the bulk reclaimed water distribution program. Records shall be maintained for a minimum of five years. These records shall include, but are not - necessarily limited to, the following information: a. Date and time of bulk distribution event, b. Name of receiver or town -issued bulk fill certification number, c. Intended use of the bulk reclaimed water, d. Number of gallons provided at each bulk distribution event, and e. Total volume of bulk reclaimed water distributed each month: Monthly tracking records for bulk distribution shall be summed and reported Form NDMR (see Attachment A) under parameter WQO1 (Flow, Reclaimed Water Distributed). This value shall represent the total volume of reclaimed water distributed via bulk fill for that month. [ 15A NCAC 02T .0108(c)] A maintenance log shall be maintained at this facility. This log shall be maintained at the facility for a period of no less than five years, and shall be made available to the Division upon request. At a minimum, this log shall include: a. Date of calibration of flow measurement device(s); b. Date of calibration of turbidimeter; c. Record of preventative maintenance (e.g., changing/adjusting of equipment, pump and valve maintenance, cross connection control, testing, inspections and cleanings, etc.; and d. Record of all unpermitted releases of reclaimed water to surface water or land surface including date of occurrence, estimated volume of release, cause, and corrective action taken. [15A NCAC 02T .0108(b)(1)] 6. Three copies of all effluent monitoring data and reclaimed water distribution data (as specified in Conditions IV.3. and IVA.) shall be submitted on Form NDMR for each PPI listed in Attachment A. Reporting forms shall be submitted on or before the last day of the following month. If no reclaimed water distribution activities occurred during the monitoring month, monitoring reports documenting the absence of the activity are still required to be submitted. All effluent monitoring data shall be submitted to the following address: Division of Water Resources, Information Processing Unit, 1617 Mail Service Center, Raleigh, North Carolina, 27699-1617. [15A NCAC 02T .0105(1)] WQ0021734 Version 3.0 Shell Version 151201 Page 5 of 7 7. Noncompliance Notification: The Permittee shall report by telephone to the Mooresville Regional Office, telephone number (704) 663-1699, as soon as possible, but in no case more than 24 hours, or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any failure resulting in a discharge of reclaimed water directly to surface waters or any unpermitted release of reclaimed water to land surface greater than or equal to 5,000 gallons. Unpermitted releases less than 5,000 gallons to land surface shall be documented by the Permittee in accordance with Condition IV.8.e. but do not require Regional Office notification. b. Any time self -monitoring indicates the facilities permitted herein have gone out of compliance with the limitations contained in this permit. Any emergency requiring immediate reporting (e.g., discharges to surface waters, imminent failure of a storage structure, etc.) outside normal business hours shall be reported to the Division's Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-3300. Persons reporting such occurrences by telephone shall also file a written report in letter form within five days following first knowledge of the occurrence. This report shall outline the actions taken or proposed to betaken to ensure the problem does not recur. [15A NCAC 02T .0108(b)(1)] V. INSPECTIONS 1. The Permittee shall provide adequate inspection and maintenance to ensure proper operation of the reclaimed water bulk distribution facilities. [15A NCAC 02T .0108(b)] The Permittee or their designee shall inspect the reclaimed water bulk distribution facilities to prevent malfunction, facility deterioration and operator errors resulting in discharges, which may cause the release of wastes to the environment, a threat to human health or a public nuisance. The Permittee shall maintain an inspection log that includes, at a minimum, the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken. The Permittee shall maintain this inspection log for a period of five years from the date of the inspection, and this log shall be made available to the Division upon request. [15A NCAC 02T .0108(b)] 3. Any duly authorized Division representative may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the reclaimed water bulk distribution facilities permitted herein at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records required to be maintained under the terms and conditions of this permit, and may collect groundwater, surface water or leachate samples. [G.S. 143-215.1] 4. The Permittee shall inspect the utilization sites of all reclaimed water users who obtain more than 10,000 gallons per day of bulk distributed reclaimed water. Permittee inspection of these bulk distribution reclaimed water utilization sites does not need to exceed one inspection per quarter. The Permittee shall maintain records of these inspections for at least five years, and shall make them available to the Division upon request. [15A NCAC 02T .0108(b)] VI. GENERAL CONDITIONS 1: Failure to comply with the conditions and limitations contained herein may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statutes 143-215.6A to 143-215.6C. [G.S. 143-215.6A to 143-215.6C] 2. This permit shall become voidable if the permitted facilities are not constructed in accordance with the conditions of this permit, the Division approved plans and specifications, and other supporting documentation. This permit is effective only with respect to the nature and volume of wastes described in the permit application, Division approved plans and specifications, and other supporting documentation. No variances to applicable rules governing the construction or operation of the permitted facilities are granted, unless specifically requested and approved in this permit pursuant to 15A NCAC 02T .0105(n). [G.S. 142-215.1] 4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other jurisdictional government agencies (e.g., local, state, and federal). Of particular concern to the Division are applicable river buffer rules in 15A NCAC 02B .0200; erosion and sedimentation control requirements in 15A NCAC Chapter 4 and under the Division's General Permit NCGO10000; any requirements pertaining to wetlands under 15A NCAC 02B .0200 and 02H .0500; National Pollutant Discharge EIimination System (NPDES) requirements under 15A NCAC 02H .0100, and documentation of compliance with Article 21 Part 6 of Chapter 143 of the General Statutes. [15A NCAC 02T .0105(c)(6)] 5. In the event the permitted facilities change ownership or the Permittee changes their name, a written permit modification request shall be submitted to the Division. This request shall be made on official Division forms, and shall include appropriate property ownership documentation and other supporting documentation as necessary. The Permittee of record shall remain fully responsible for maintaining and operating the facilities permitted herein until a permit is issued to the new owner. [15A NCAC 02T .0104] 6. The Permittee shall retain a set of Division approved plans and specifications for the life of the facilities permitted herein. [15A NCAC 02T .0108(b)(1)] 7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or permitted under another permit issued by the appropriate permitting authority. [15A NCAC 02T .01050)] 8. This permit is subject to revocation or unilateral modification upon 60 days notice from the Division Director, in whole or part for the requirements listed in 15A NCAC 02T .0110. Unless the Division Director grants a variance, expansion of the permitted facilities contained herein shall not be granted if the Permittee exemplifies any of the criteria in 15A NCAC 02T .0120(b). [15A NCAC 02T .0120] 10. The Permittee shall pay the annual fee within 30 days after being billed by,the Division. Failure to pay the annual fee accordingly shall be cause for the Division to revoke this permit. I have inspected the facilities as constructed and found them to be in compliance with the requirements of the original design and the permit (W00021734) approved on September 17, 2002, by the North Carolina Department of Environment and Natural Resources (NCDENR). Additionally, I have reviewed the operating and maintenance procedures and found them to also be in compliance with the permit requirements. A copy of this certification should be forwarded to NCDENR, Non -Discharge Permitting Unit at 1616 Mail Service Center, Raleigh, NC 27699-1617. Furthermore, the Mooresville Regional Office (704) 663-1699 shall be notified at least 48 hours prior to the first day of operation of the bulk reclaimed water program. I recommend Charlotte -Mecklenburg Utilities copy them on the letter transmitting this certification to NCDENR to provide the notification. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the utilization area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including.those actions that znay be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. . 2. The issuance of this.permit shall not relieve the Permittee of the responsibility fox damages to ground or surface waters resulting from the operation .of this facility:: . .3... Tank trucks used to transport reclaimed water shall not be filled through an -board piping or removable hoses that racy subsequently be used to fill tanks with water from a potable water supply. d. Tank trucks used to transport septage, raw wastewater, or wastewater residuals shall not be used to transport reclaimed water. Where appropriate, such warning shall inform the public or employees to avoid contact with the water.. b. All reclaimed water piping, valves, outlets, and other appurtenances shall be. color -coded, taped, or otherwise marked to identify the source of the water as being reclaimed water. i. All reclaimed water. piping and appurtenances shall be either colored purple (i.e., Pantone 522) and embossed or integrally stamped or marked "CAUTION: RECLAEYIED WATER'— DO NOT DRINK" or be installed with a purple (i.e., P'anton6 522) identification tape or polyethylene, vinyl wrap. The warring shall be stamped on opposite 'sides d the pipe and repeated every three feet or less. ii. Identification tape .shall be at least three inches wide and have white or black lettering on purple (i.e., Pantone 522) field stating '"CAUTION- RECLAIMED WATER —' DO NOT DRINK." Identification tape shall be installed on top of reclaimed water pipelines, fastened at least every, 10 feet to each pipe length and run continuously the entire length of the pipe.. c. All reclaimed water valves and outlets shall be of a type,' or secured, in a mamer, that permits operation by authorized personnel only. d. Above -ground hose I bibs (i.e,, spigots or other hated -operated connections) shall not be present_ Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled as being of non -potable quality. As an alternative to the use of locked below -grade vaults with standard Bose bibs services, hose bibs, which can only be operated bya special tool or connected to a special hose connection, may be placed in non -lockable undergromid services boxes clearly labeled as non -potable water. . ongA,TION AND M4 NANCE R MEREMENTS 1. The facilities shall be properly maintained and operated at all times. NCAC 02T .0913 including operational functions,' maintenance schedules, safety ixieasures, and a spill zesporese plan. 2. Upon classification of the wastewater treatment and high -rate infiltration facilities by the Water Pollution Control System Operators Certification Commission (WPCSOCQ, the Per nittee shall designate and employ a certified operator to be in responsible charge (ORC) and one or more certified operator(s) to be backup ORC(s) of the facilities in accordance with 15,A. NCAC 08G' .0200. The ORC shall visit the facilities in accordance with I5A NCAC 080 .0200 or as speciled in this permit and shall comply with all other conditions specified in thesc..rules. 3., A certified operator of a grade equivalent or greater than the water reclamation facility shall be available on -call 24 hours per day. 4. 'The Permittee shall maintain a Spill Prevention, Countermeasures,. and Control Plan for the water reclamation facility as well as for the bulls reclaimed water distribution program 5. All laboratory analyses. for effluent, ground waters, or surface waters shall be made by a laboratory certified by the Division for the required parameters under '15A NCAC 02H .0800. 4. Prior to con�una.encement of bulky distributio)a of the reclaimed water and thereafter, the effluent from the subject facilities shall be monitored by the permittee at the frequency and location for the Parameters specified in A.ttaclurient A. 5. The Pennittee tracking the amount of reclaimed water applied shall maintain adequate records. These records shall include, but are not necessarily limited to, the following inforrhation: a. Date of reclaimed water application, b. Volume of reclaimed water applied, c, l ateuded uses of reclaimed water, d, Locations of intended uses, - e. Names and approval status of truck operators, f Apiaroval status of trucks, g. Compliance status of the bulk reclamation facility, and h. Weather conditions. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, . compressors; etc. c. Any failure of disposal system, resulting in a by-pass directly to receiving waters. d. , Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations includinng, but not limited to, freeboard measurements, ef0,uent limitations, exceedances of groundwater standards, or overloading of any infiltratibn area. The Permitter- shall }seep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of five years Rona the date of the inspection and shall be made available upon request to the Division or other permitting authority. 3. VT. GENERAL CONDYTIONS 1. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to air enforcement action by the Division in accordance with North Carolina General Statute 143- 215.6A to 143-215.6C. 2. No later than six months prior to the expiration of this permit, the Permittee shall request renewal of this permit on official Division forms. Upon receipt of the request, the Division will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. Please note that Rule 15A NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal application. 4. The Permittee, shall request modification of NPDES Permit Number NCO084549 issued to the Franklin Water Treatment Plant. The modification shall amend the existing NPDES permit to allow the conjunctive use of reclaimed water as provided for in this permit. The modification request shall be made during, or prior to, the next NPDES permit renewal process. II. PERFORMANCE STANDARDS 1. The bulk reclaimed water distribution program and facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters, which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the utilization area to adequately .assimilate the wastewater, the Permittee shall take immediate corrective actions including. those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. 2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to ground or surface waters resulting from the operation of this facility. All wells that are constructed for purposes of groundwater monitoring shall be constructed in accordance with 15A NCAC 02C .0108 (Standards of ,Construction for Wells Other than Water Supply) and any other state and local laws and regulations pertaining to well construction. 4. Diversion or bypassing of the untreated wastewater from the reclaimed water treatment facilities is prohibited. 5. The Permittee shall approve all tank trucks for participation in the bulk reclaimed water distribution program prior to distribution of reclaimed water. Tank trucks shall meet the following minimum criteria: a. Tank trucks and other equipment used to distribute reclaimed water shall be clearly identified with advisory signs. b. Tank trucks used to transport reclaimed water shall not be used to transport potable water that is used for drinking or other potable purposes. . c. Tank trucks used to transport reclaimed water shall not be filled through on -board piping or removable hoses that may subsequently be used to fill tanks with water from a potable water supply. d. Tank trucks used to transport septage, raw wastewater, or wastewater, residuals shall not be used to transport reclaimed water. . WQ0021734 Version 2.0 Shell Version 070320 Page 2 of 10 6. The Permittee shall approve tank truck drivers for participation in the bulk reclaimed water distribution program prior to distribution of reclaimed water. In addition, the Permittee shall provide training (bilingual (English and Spanish) as necessary) to all tank truck drivers approved to participate in the bulk reclaimed water distribution program at a regular frequency. These sessions shall provide training on, but not limited to, the following topics: a. Reclaimed water description and safety, b. Approved and disapproved uses of reclaimed water, c. Procedures for the application (including buffer maintenance), uses, and storage of reclaimed water as well as the disposal of unused reclaimed water, d. Procedures for spill prevention, countermeasures, and control as well as notification requirements, e. Personal safety and hygiene issues, and f. Interaction with the general public. 7. The following uses of reclaimed water shall be strictly forbidden by the Permittee: a. Irrigation of direct food crops, b. Filling of swimming pools, hot tubs, or similar uses, c. Use as a raw potable water supply, and d. Use after 72 hours of storage. The following uses of reclaimed water distributed through the bulk reclaimed water distribution program shall be approved: a. Sewer line flushing, b. Dust control for street sweeping, c. Vehicle washing at construction sites, with no steam cleaning and no engine'or parts cleaning, d. Landscape irrigation, and e. Construction uses such as soil compaction, dust control, and asphalt reclamation. All other uses for reclaimed water shall not be allowed by the Permittee. In the event there is a desire for the permitting of additional uses, a formal permit modification request must be submitted to the Division accompanied by an application fee and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 8. Effluent limitations shall not exceed those specified in Attachment A. 9. The facilities permitted herein must be constructed according to the following setbacks: a. The setbacks for reclaimed utilization sites shall be as follows (all distances in feet): i. Wetted areas and surface waters not classified as SA including wetlands as delineated or designated by the appropriate state or federal agency or any 25 swimming pool: ii. The following shall be requirements for the reclaimed water distribution, storage, and utilization facilities: a. All reclaimed water valves, storage facilities, and outlets shall be tagged or labeled to warn the public or employees that the water is not intended for drinking. Where appropriate, such warning shall inform the public or employees to avoid contact with the water. b. All reclaimed water piping, valves, outlets, and other appurtenances shall be color -coded, taped, or otherwise marked to identify the source of the water as being reclaimed water. i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone 522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER — DO NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and repeated every three feet or less. ii. Identification tape shall be at least three inches wide and have white or black lettering on purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT DRINK." Identification tape shall be installed on top of reclaimed water pipelines, fastened at least every 10 feet to each pipe length and run continuously the entire length of the pipe. c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits operation by authorized personnel only. d. Above -ground hose bibs (i.e., spigots or other hand -operated connections) shall not be present. Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled as being of non -potable quality. As an alternative to the use of locked below -grade vaults with standard hose bibs services, hose bibs, which can only be operated by a special tool or connected to a special hose connection, may be placed in non -lockable underground services boxes clearly labeled as non -potable water. III. OPERATION Alen) MAINTENANCE REQUIP.EMEl`NTS The facilities shall be properly maintained and operated at all times. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. The Permittee shall maintain an Operation and Maintenance Plan pursuant to 15A NCAC 02T .0913 including operational functions, maintenance schedules, safety measures, and a spill response plan. 2. Upon classification of the wastewater treatment and high -rate infiltration facilities by the Water Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall designate and employ a certified operator to be in responsible charge (ORC) and one or more certified operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 08G .0200. The ORC shall visit the facilities in accordance with 15A NCAC 08G .0200 or as specified in this permit and shall comply with all other conditions specified in these.rules. A certified operator of a grade equivalent or greater than the water reclamation facility shall be available on -call 24 hours per day. 4. The Permittee shall maintain a Spill Prevention, Countermeasures, and Control Plan for the water reclamation facility as well as for the bulk reclaimed water distribution program 5. Adequate measures shall be taken to prevent ponding or runoff from the reclaimed water utilization areas. 6. Reclaimed water application shall not be performed during inclement weather or when the ground is in a condition that will cause ponding or runoff. 7. All reclaimed water application equipment must be tested and calibrated at least once per year. Records of the calibration must be maintained for five years. No type of reclaimed water other than that from the Franklin Water Treatment Plant shall be distributed through the bulk reclaimed water distribution program and facilities. 9. The bulk reclaimed water distribution facilities shall be accessible and operated by authorized personnel only. 10. Public access to the application sites and contact with reclaimed water shall be controlled during reclaimed water distribution and use. Such controls may include public meetings, publication of notices in local newspapers, notification through television and radio news programs, as well as distribution of the bilingual educational flyers. 11. The residuals generated from these treatment facilities must be disposed / utilized in accordance with 15A NCAC 02T .1100. The Permittee shall maintain a residual management plan pursuant to 15A NCAC 02T .0914. 12. Diversion or bypassing.of the untreated wastewater from the treatment facilities is prohibited. 13. All wastewater shall either be routed to the existing backwash clarifiers of the water treatment facilities or discharged in strict accordance with the conditions of the facility's existing NPDES permit should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the levels have stabilized below these maximum levels. 14. For any emergency that requires immediate reporting (e.g., discharges to surface waters, imminent failure of a storage structure, etc.) outside normal business hours must be reported to the Division's Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733- 3300. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. V. INSPECTIONS 1. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of five years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions' of this permit, and may obtain samples of groundwater, surface water, or leachate.' VI. GENERAL CONDITIONS 1. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143- 215.6A to 143-215.6C. 2. This permit shall become voidable unless the bulk reclaimed water distribution program and facilities are constructed and operated in accordance with the conditions of this permit, the approved plans and . specifications, and other supporting data. 3. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. No variances to applicable rules governing the construction and / or operation of the permitted facilities are granted unless specifically requested and granted in this permit. 4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances, which may be imposed by other government agencies (local, state, and federal) that have jurisdiction. Of particular concern to the Division are applicable river buffer rules in 15A NCAC 02B .0200, erosion and sedimentation control requirements in 15A NCAC Chapter 4 and under the Division's General Permit NCGO10000, and any requirements pertaining to wetlands under 15A NCAC 02B .0200 and 02H .0500. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division on official Division forms, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. The Permittee of record shall remain fully responsible for compliance until a permit is issued to the new owner. 6. The Permittee shall retain a set of approved plans and specifications for :the life of the facilities permitted herein. 7. This water will be used as supplemental, supply for existing potable water uses, such as irrigation of public, commercial, and industrial grounds, street washing, dust control, vehicle washing, aesthetic purposes, and general construction purposes (non -potable). The wash water will be pumped from an. existing above ground collection box prior to discharge and pumped to a truck loading station which will allow the filling of tanker trucks. This facility will be anz intermittent use facility. System Layoot (See Appendix 11, for Design Drawings) The design of the facility will consist of installing a submersible pump (See Appendix C for specification) in the existing Effluent Splitter Box which collects overflow from two existing backwash water clarifiers. Currently the splitter'box directs the overflow to the point of discharge for the existing XPDES permit.- A. portion of the clarifier overflow will be pw-nped to a new truck loading station for loading into tm�lcer trucks. The truck loading station will consist of a concrete pad with 3-inch track loading piping installed above the pad. A. 3-inch water meter will be provided in the truck loading piping for measurement of flow to trucks. A quick connect and dedicated flexible hose will be provided for use in connecting to trucks prior to filling. The truck loading pad will contain a sump in the center to collect any drippage that may occur during connecting. and disconnecting of the flexible hose to the truck. Instrumentation Continuous turbidity mox-litoring .will be provided for the reclaimed wash water by installation of a turbidimeter in the EfEnent Splitter Box adjacent to the new truck loading. pump. The analyzer will measure the turbidity of the clarified wash water continuously even when truck loading is not being performed. Flow measurement will be performed via the water meter installed at the truck loading station. Since reclaimed wash water flow will not be continuous, ail instatataneous flow measurement device: will not be utilized. Flow will be measured on a volume basis by the water meter. Daily recordings will be wade to measure the total reclaimed water utilized. Inside the control panel will be the ON/OFF switch ;for the truck loading pump, as well as the local readout for the turbidimeter. This will allow the operator to verify turbidity compliance while loading trucks. � if the turbidity limit is exceeded at any time, the pump will immediately be turned off. The key to the control panel will be in the possession of the plant personnel. This will eliminate the possibility of loading of reclaimed wash water without operator knowledge. Truck Loading Procedure The procedure far loading reclaimed WTP wash water into approved tankers is as follows: 1, When tm6k arrives to be filled, truck driver will notify operator to me6t at the track loading facility. 2. Operator will verify truck driver has been Mined regarding use of reclaimed water by checking their name against the List of Approved Reclaimed Water Truck Drivers. 3. Truck driver will connect hose provided at the truck loading station to the tanker truck as required for loading. 4. Operator will unlock truck loading panel and verify current turbidity and latest water quality data meet perruit limits. 5 _ Operator will switch the truck loading pump to ON and till the truck. 6. Once filled, the pump will be switched off, and the amount of reclaimed water loaded into truck along with turbidity and intended use will be written on the Reclaimed Water.Load Form. 7. The operator will lock the truck loading control panel prior to leaving. Should the turbidity analyzer read a value above 10 NTU at any time, the pump will be tttrrred off ix=ediately. Compliance'with State Regulation 15A NCAC 2H .0219(k)(1.)(D) The following design components are intended to satisfy the compliance criteria present in the State Regulation 15A NCAC 2H .0219(k)(1)(D). ■ All tanker trucks carrying reclaimed WTP wash water will be required to have signage, as such "CAUTION: RECLAIMED WATER —DO NOT DRINK." Compliance will be verified prior to loading trucks with reclaimed water. Tanker trucks which haul reclaimed water will not be used to also haul potable water. * All piping from the truck loading, pump to the truck loading station will be painted the color purple (Pantone 522) and identified "CAUTION: RECLAIMED WATER —DO NOT DRINK" at 10-foot increments. * Truck loading station concrete pad will have a sign pe=anently affixed to the concrete stating "CAUTION. RECLAIMED WATER FILL STATION ONLY." Reclaimed Water Source The reclaimed water source being utilized is water treatment plant wash water. Water treatment plant wash water is a byproduct of the normal water treatment process. One of the final steps in the water treatment process is to allow the "raw untreated water" to :flow through a conventional mixed media filter. This process removes small particles which may be remaining in the raw water after the initial coagulation, flocculation, and sedimentation steps of the water treatment process. Periodically, the filters -get dirty an need to be cleaned. To clean the filters, potable water is sent through the filter !a the reverse direction to remove any'solids buildup that may have occurred during the filtration process. The resulting flow of 'water is called spent wash water. Currently, at the Franklin Water Treatment Plant, the spent wash water receives some treatment and is discharged to an adjacent tributary for disposal. The intent of this reclaimed WTP wash water system is to utilize the spent wash water currently discharged to the adjacent tributary for beneficial purposes. Reclaimed Water Usage Reclaimed WTP wash water has been accepted as a supplement for potable .water usage regarding only the following applications • Irrigation of residential lawns, golf courses, parrs, landscape areas, and other public, industrial, or commercial grounds. • Stireet cleaning (dust control only'). • Vehicle washing (for vehicle washing on construction sites only: Vehicle underbody not to be washed with reclaimed water). • Dust control. • Aesthetic purposes (i.e., decorative ponds and fountains). • General construction purposes (non -potable, non -contact). All other applications will be considered to not be in compliance with the permit by which reclaimed water usage was accepted at this facility —Franklin Water Treatment Plant. Public Noti ication/5ignage All vehicles utilized to haul reclaimed water shall be clearly identified on all sides with advisory signs "CAUTION: RECLAIMED WATER —DO NOT. DRINK." Tanker trucks utilized to haul potable water for drinking shall not be used for reclaimed water. The following is a summary of the applicable requirements of this program. ENGINEERING REQUIREMENTS • Reclaimed water may be manually distributed from a 3-inch hose connected to a quick -connect coupling and controlled by a lockable control panel at a newly constructed truck filling station. • The dedicated truck loading hose with multiple connections will be provided at the truck loading station to prevent the possibility of a truck -mounted hose -used for reclaimed water contaminating potable water, trucks.. • All reclaimed water distributed must be measured and recorded. • Bulk reclaimed water may only be distributed to approved trucks and drivers. • . The program shall be operated such that no discharge of wastes to surface water occurs. PERFORMANCE STANDARDS • A professional engineer must send a letter to DENR to certify that facility has been constructed in accordance with all permit details. • The Mooresville Regional Office must be notified at least 48 hours in advance of the first day of operation of the reclaimed water program and facilities so that an inspection can be made and operation observed. • The reclaimed water program and facilities shall be effectively maintained and operated at all times so that there is no discharge to surface waters nor any contamination of ground waters. • The Permittee shall maintain a Spill Prevention, Countermeasures, and Control Plan for both the facility and the distribution program. • Diversion or bypassing of untreated wastewater is prohibited. • The reclaimed water facilities shall be accessible and operated by authorized personnel only. Franklin Water Treatment Plant Reclaimed Water Program Requirements TANK TRUCK REOUIREMENTS • After Permittee approval, tank trucks will meet the following criteria: a. Tank trucks and other equipment used to distribute reclaimed water shall be clearly identified with advisory signs. b. Tank trucks used to transport reclaimed water shall not be used to transport potable water that is used for drinking or other potable purposes. C. Tank trucks used to transport reclaimed water shall not be filled through on -board piping or removable hoses that may subsequently be used to fill tanks with water from a potable water supply. d. Tank trucks used to transport septage, raw wastewater, or wastewater residuals shall not be used to transport reclaimed water. e. Reclaimed water Information Data Sheets will be issued to each driver and must be kept inside the vehicle while reclaimed water is being hauled. DRIVER TRAINING REQUIREMENTS • After Permittee approval, tank truck drivers will be trained on, but not limited to, the following topics: a. Reclaimed water description and safety, b. Approved and disapproved uses of reclaimed water, c. Procedures for the application (including buffer maintenance), uses, and storage of reclaimed water, as well as the disposal of unused reclaimed water, d. Procedures for spill prevention, countermeasures, and control, as well as notification requirements, e. Personal safety and hygiene issues, and f. Interaction with the general public. Bilingual training will be provided as necessary. APPLICATIONS AND RECORDKEEPING • The Permittee shall maintain applications and of all approved tank trucks and drivers on file for the duration, not less than one year, that each tank truck and driver participates in the bulk reclaimed water distribution program. Franklin Water Treatment Plant 4 Reclaimed Water Program Requirements TRUCK LOADING PROCEDURES 1) When truck arrives, truck driver will notify operator to meet at loading site. 2) Operator will verify truck driver has been trained regarding use of reclaimed water by checking his name against the list of approved reclaimed water truck drivers. 3) Truck driver will connect provided hoses. 4) Operator will unlock loading panel and verify current turbidity reading and latest water quality data meets requirements. 5) Operator will switch pump to `ON' and fill truck. 6) After filling tank truck, operator will turn off pump, record amount of water loaded, finishing turbidity reading, and intended use on Reclaimed Water. Load Form. 7) The operator will lock the truck loading panel before leaving the site. 8) Should the turbidity value read above 10 NTUs at any time,.the pump will be turned off immediately. Franklin Water Treatment Plant Reclaimed Water Program Requirements RECLAIMED WATER USE REQUIREMENTS • The following uses of reclaimed water are strictly forbidden: a. Irrigation of direct food crops, b. Filling of swimming pools, hot tubs, or similar uses, c. Use as a potable water supply, and d. Use after 72 hours of staorage. • Only the following uses of reclaimed water are approved: a. Sewer line flushing, b. Dust control for street sweeping, c. Vehicle washing at construction sites, with no steam cleaning and no engine or parts cleaning, d. Landscape irrigation, and e. Construction uses such as soil compaction, dust. control, and asphalt. reclamation • Al other uses for reclaimed water shall not be allowed by the Permittee. • In the event there is a desire for the permitting of additional uses, a formal permit modification request must be submitted to the Division accompanied by an application fee and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or not be approved. • The following buffers shall be maintained during application of the reclaimed water: a. 100 feet between wetted areas and water supply wells, b. 10 feet between wetted areas and non -potable supply wells, and c. 25 feet between wetted areas and surface waters not classified as SA including wetlands as delineated or designated by the appropriate state or federal agency or any swimming pool. Public access to the application sites and contact with reclaimed water shall be controlled during water distribution use. Such controls may include public meeting, publication of notices in local newspapers, notification through television and radio programs, as well as distribution of bilingual educational. fliers. Franklin Water Treatment Plant Reclaimed Water Program Requirements OPERATION AND MAINTENANCE REOUIREMENTS • The bulk reclaimed water distribution program and facilities shall be properly maintained and operated at all times. • ; The Permittee shall designate and employ a certified operator to be in responsible charge (ORC) and one or more operators to be back-up ORC's of the facilities. The Permittee shall submit ORC and back-up ORC designations to WPCSOCC at least 60 days prior to introduction of waste into the system. The designated ORC shall visit the system in accordance with 15A NCAC 8G .0204 and comply with all other conditions in these rules. • Reclaimed water shall not be distributed for land -based uses nor, shall reclaimed water be used for land -based uses during inclement weather or when the ground is in a condition that will cause runoff. • The Permittee shall implement adequate measures to prevent reclaimed water runoff from the application area. • The bulk reclaimed water distribution program and facilities will be effectively maintained and operated as a non -discharge system. • No type of.reclaimed water other than that from the Franklin Water Treatment Plant shall be distributed through this program or facilities. • All wastewater shall either be routed to the existing backwash clarifiers should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity. (instantaneous maximum of 10 NTUs) be exceeded, until such time that the levels have stabilized below these maximum levels. • All reclaimed water distributed but not used within a period not to exceed 72 hours from the time of distribution shall be reintroduced at the existing backwash clarifiers for disposal. Franklin Water Treatment Plant Reclaimed Water Program Requirements NONCOMPLIANCE NOTIFICATION INSTRUCTIONS The Permittee shall report to the Raleigh Office by telephone, (919) 571-4700, as soon as.possible, but in no case more than 24 hours or the next working day, following the occurrence or first knowledge of any of the following: a) Any occurrence at the truck filling station which results in the compromise of the quality of the treated wastewater, or any other unusual circumstances. b) Any process unit failure, due to known or unknown reasons, that renders the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c) Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly into the receiving waters without treatment of all or any portion of the influent to such a station or facility. d) Any spill of reclaimed water from the bulk reclaimed water distribution facility during filling, from tank trucks during transportation, or from tank trucks during application. e) Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not re -occur. INSPECTIONS • Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to ensure proper operation and management of the bulk reclaimed water distribution and facilities. • The Permittee shall inspect the bulk reclaimed water distribution program and facilities to prevent malfunction and deterioration, operator errors, and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. • The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. The log of inspections shall be maintained by the Permittee for a period of three years from the date of inspection. Franklin Water Treatment Plant Reclaimed Water Program Requirements APPLICATIONSAND RECORDKEEPING • The Permittee shall maintain applications and of all approved tank trucks and drivers on file for the duration, not less than one year, that each tank truck and driver participates in the bulk reclaimed water distribution program. • Adequate. records -shall be maintained by the Permittee that tracks the amount of bulk reclaimed water distributed. These records shall include, but are not necessarily limited to, the following information: a) Date and time of distribution, b) Volume of reclaimed water distribution, c)' . Intended uses of reclaimed water, r d) Locations of intended uses, e) Names and approval status of truck operators, f) Approval status of trucks, g) Compliance status of the bulls reclamation facility, and h) Weather conditions. • Three copies of all operation and distribution records shall be submitted on or before the last day of the month following the month that was monitored. Three copies of all effluent data on Form NDMR-1 shall be submitted on or before the last day of the month following the month that was monitored: • Periodic inspections are to be made to prevent malfunctions, deterioration, operator errors, etc. A log of these inspections is to be kept, noting observations made, maintenance, repairs, or corrective actions taken. This log must be kept for a minimum of 3 years and be readily available, upon request, to the Division or other permitting authority.
