RE: Revised PCN -Carrollton Development Sewer Outfalls 1 & 2
Owner: PM Development, LLC
Dear Cyndi:
Thank you for processing the above referenced application after documentation was provided by
the owner to address mitigation of cumulative impacts. Since then, it has come to our attention
that the PCN contained an error in Section VI.4, namely the stream name in the table
inadvertently included "UT to". Note that the written description in Section VI.1 was correct.
After bringing this to his attention, Daryl Lamb of the WSRO asked us to resubmit page 9 of the
PCN with the correct stream name in the table. Four (4) copies of revised page 9 are enclosed
for your distribution with the previously submitted PCN's. At his direction, we are sending a
copy directly to Mr. Lamb for his file. We assume the 401 certification does not need revision.
If you have questions or need further information, please call our office at (336) 335-1108.
Sincerely,
Ecologic Associates, P.C.

4. Individually list all intermittent and perennial stream impacts. Be sure to identify temporary
impacts. Stream impacts include, but are not limited to placement of fill or culverts, dam
construction, flooding, relocation, stabilization activities (e.g., cement walls, rip-rap, crib
walls, gabions, etc.), excavation, ditching/straightening, etc. If stream relocation is proposed,
plans and profiles showing the linear footprint for both the original and relocated streams
must be included. To calculate acreage, multiply length X width, then divide by 43,560.
