In accordance with the conditions of the NCDWQ Mitigation Bank Instrument number 07-0747, issued to the GreenVest/Flat
Swamp, LLC on June 15, 2009, we are seeking approval from the NCDWQ for our Ledger Transaction Number: FSMB 2011-0009
for the withdraw of 258 nutrient pounds for the project known as the Atlantic Medical Group, located on 2541 North Queen
Street at the intersection of North Herritage Street and North Queen Street, in the City of Kinston, North Carolina.
Attached is a copy of the Bank's Ledger indicating the withdraw and the letter from Mr. Steven W. Miller, Water Resource
Manager, for the City of Kinston dated September 16, 2011 approving the number of nutrient offset pounds required for this
project which is located in the Middle Neuse River Basin (CU03020202).

RE: Atlantic Medical Nitrogen Offset Payment Confirmation Letter
Dear Mr. Brydge;
The City of Kinston has reviewed the site plans for the construction of the above project at the intersection of North
Herritage Street and North Queen Street in Kinston. The project information is as follows:
Project Name: Atlantic Medical
Transaction #: SP 11-003
Nutrient Involved: Nitrogen
Equation: 4.301b/ac/yr x 2.00 acres x 30 years = 258.0 lbs
Location CU: 03020202
Based on the review of the plans, this development has been instructed to pay an offset payment to mitigate this
impact. Attached is the current list of mitigation banks in the Neuse Basin. You must use a mitigation bank in
the hydrologic unit listed above. I will need to receive a receipt or confirmation from the mitigation bank
regarding the offset payment before a construction permit can be issued for the project.
Finally, the City of Kinston does charge a stormwater utility fee to all developed property inside the city limits.
The monthly fee for non-residential properties is based on the impervious area of the property and is currently $4
per egwvalcut residential unit (ERU). The number of ERUs is determined by dividing the total impervious area
by 3,059 ware feet, rounded up to the nearest whole number. The monthly stormwater utility bill for this
property will be $40-00. However, we do offer credits to reduce the utility fee for properties that are managing or
