On August 2, 2011 the Division of Water Quality (DWQ) received the application sets for the above
referenced project. The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below.
Additional information Requested:
?l DWQ is unable to set up and review this project as neither Andale, Inc nor Tom Furlong is listed
on the NC Department of the Secretary of State's Corporation website. Here is the referencing
site: littp://www.secretaa.state.nc.us/Corporations/CSearch.aspx . This matter will need to be
rectified before DWQ can handle this project.
® DWQ requires five (5) complete PCN application sets and maps to begin the review and approval
process.
® One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or
color). If the plans are too large to store in TIFF format, they can be stored in PDF.
® The PCN indicates that 401 written approval is being requested. Written approval for a 401
Water Quality General Certification must include the appropriate fee. The fee to remit to: DENR
- DWQ is $240.
