Here is another Div. 9 project that they sent me a courtesy copy of but didn't send any to Raleigh. You don't need to send
it back to me, I've already gone out to see it and realized it wasn't in filemaker when I went to enter a staff report, sorry.
Thanks,

Mr. Eric C. Alsmcyer
US Army Corps of Engineers
6509 Falls of the Neuse Road, Suile 120
Raleigh, NC 27615-6814 051089
Dear Mr. Alsmeyer,
In an effort to obtain the necessary permits for the subject bridge repair, we are providing a pre-
construction notification application with supporting documentation for the repair of the bridge over the
Yadkin River in Forsyth County. This is a pennit for a defined river with distinct riverbanks. One of the
piers has shifted and needs to be repaired. Lt order to do this work, a temporary work platform will need to
be constructed to reach the pier and also to slow the swill current during the repair. The platform will be
130 feet long by 40 feet at the base, for a total of 5200 square feet. The pier repair will involve (lie
placement of 260 square feet of large riptap around two picas to prevent. scour. No cndanf crcd species
were found on 5/2/05. We believe this project can be accomplished under Nationwide Permits 3 and 33.
If you have any questions, comments or need additional information alter reviewing this material,
please contact me at (336) 703-6500. Thank you for your cooperation in this matter.

Attach a vicinity imp clearly showing the location of the property with respect to local
landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property
boundaries and development plans in relation to surrounding properties. Both the vicinity map
and site plan must include a scale and north arrow. The specific footprints of all buildings,
impervious surfaces, or other facilities must be included. If possible, the maps and plans should
include the appropriate USGS Topographic Quad Map and NRCS Soil Survey with the property
boundaries outlined. Plan drawings, or other maps may be included at the applicant's discretion,
so long as the property is clearly defined. For administrative and distribution purposes, the
USACE requires information to be submitted on sheets no larger than 1 I by 17-inch format;
however, DWQ may accept paperwork of any sire. UWQ prefers full-size construction
drawings rather than a sequential sheet version of the full-size plans. It' full-size plans are
reduced to a small scale such that the final version is illegible, the applicant will be informed that
the project has been placed on hold until decipherable maps are provided.

USAC1 - In accordance with the Final Notice of Issuance and Modification of Nationwide
Permits, published in the Federal Register on March 9, 2000, mitigation will be required when
necessary to ensure that adverse effects to the aquatic environment are minimal Factors
including size and type of proposed impact and function and relative value of the impacted
aquatic resource will be considered in determining acceptability of appropriate and practicable
mitigation as proposed. Examples of mitigation that may be appropriate and practicable include,
but are not limited to: reducing the size of the project; establishing and maintaining wetland

aquatic resource functions and values by creating, restoring, enhancing, or preserving similar
functions and values, preferable in the same watershed.
