Due to its rural location, PHES utilizes a wastewater treatment plant(1HWTP) surface discharge system for wastewater disposal. The WWTP is located at the rear of the school at approximate coordinates 36.329559, -80.048665. The current NPDES permit NC0087980 authorizes Stokes County Schools to discharge wastewater from PHES WWTP. Existing System Pine Hall Elementary School WWTP current NPDES permit (NC0087980) became effective on March 1, 2017 and will expire at midnight on February 28, 2022. The existing WWTP operates as a recirculating sand filter system, and includes the following components: • Septic tank(+8,000 gallons) • Dosing chamber(+2,800 gallons) • Recirculation tank(+3,500 gallons) • Surface sand filter(f 1,640 sq. ft.) • Dual ultraviolet disinfection unit Stokes County Schools Pine Hall Elementary School treatment facility is currently authorized to discharge treated effluent into Eurins Creek, in the Roanoke River Basin, which is classified as WS-IV waters. K:\Stokes County Schools\Pine Hall Elementary Wine Hall WWTP Cover Letter.docx 1 Proposed Improvements and Repairs The original (f40 years) combination septic/sand filter dosing chamber tank has deteriorated and developed cracks over the years. The tank has been repaired several times over the years. In an effort to be proactive the SCSS wants to replace this tank. This system improvement will include installing a new 8,000-gallon septic tank,4,000-gallon filter bed dosing tank with pumps,control panel, liquid level control floats, and 3-inch supply line to the existing sand filter bed dosing manifold. These are the only modifications/repairs proposed for this project. There will be no modifications or expansions to the overall system treatment system process. Sub-Surface Information There is no sub-surface information available for this project. All excavation on this project is unclassified, and all cost required to perform the necessary work shall be included in the contractors bid. G. Ensuring Normal Operations All work and schedules for performing work shall be coordinated and approved by Stokes County Schools to insure that interruption of the normal operation of the school will be minimal. H. Weather Limitations No work shall be done when the weather is unsuitable. The contractor shall take necessary precautions (in the event of impending storms)to protect all work, materials, or equipment for damage of deterioration due to floods, driving rain, or wind and snowstorms. The owner reserves the right, through the opinion of the Engineer, to order that additional protection measures over and beyond those proposed by the contractor, be taken to safeguard all components or the project. The contractor shall not claim any compensation for such precautionary measures so ordered, nor claim any compensation from the owner for damage to the work from weather elements. The mixing and placing of concrete or pavement courses,the laying of masonry, and installation of sewer and water mains shall be stopped during rainstorms, if ordered by the Engineer; and all freshly placed work shall be protected by canvas or other suitable covering in such manner as to prevent running water from coming in contact with it. Sufficient coverings shall be provided and kept ready at hand for this purpose. The limitations and requirements for mixing and placing concrete, or laying of masonry, in cold weather shall be as described elsewhere in these specifications. The bidder shall prepare his bid with the understanding that unsuitable weather may cause all work to be stopped;yet an extension of the contract period will not be permitted due to unsuitable weather. I. Protection of Property Contractor shall be responsible for the preservation and protection of property adjacent to the work site against damage or injury as a result of his operations under this contract. Any damage or injury occurring on account of any act, omission or neglect on the part of the contractor shall be restored in a proper and satisfactory manner or replaced by and at the expense of the contractor to an equal or superior condition the previously existed. In the event of any claims for damage of alleged damage to the property as a result of work under this contract, the contactor shall be responsible for all costs in connection with the settlement of or defense against such claims. Barricades, Danger, Warning and Detour Signs The contractor shall provide, erect, and maintain all necessary barricades, suitable and sufficient red lights, danger signals and signs, and take all necessary precautions for the protection of the work and safety of the public. L. Existing Utilities The term existing utilities shall be deemed to refer to both publicly owned and privately owned utilities such as electric power and lighting,telephone, water, gas, storm drains, and sanitary sewers, and all appurtenant structures. Where existing utilities and structures are indicated on the drawings, it shall be understood that all of the existing utilities and structures affecting the work may not be shown and that the locations of those shown are approximate only. It shall be the responsibility of the contractor to ascertain the actual extent and exact location of existing utilities and structures. In every instance, the contractor shall notify the proper authority having jurisdiction and obtain all necessary directions and approvals before performing any work in the vicinity of existing utilities. The work shall be carried out in a manner to prevent disruption of existing services and to avoid damage to the existing utilities. Temporary connections shall be provided, as required, to insure no interruption of existing services. Any damage resulting from the work of this contract shall be promptly repaired by the contractor at his own expense in a manner approved by the engineer and further subject to the requirements of any authority having jurisdiction. Where it is required by the authority having jurisdiction that they perform their own repairs or have them done by others,the contractor shall be responsible for all costs thereof. Where excavations by the contractor require any utility lines or appurtenant structures to be temporarily supported and otherwise protected during the construction work, such support and protection shall be provided by the contractor. All such work shall be performed in a manner satisfactory to the engineer and the respective authority having jurisdiction over such work. In the event the contractor fails to provide proper support or protection to any existing utility, the engineer may, at his discretion, have the respective authority provide such support or protection as may be necessary to ensure the safety of such utility; and the costs of such measures shall be paid by the contractor. M. Safety Each contractor alone shall be solely and completely responsible for conditions of the job site in connection with his work, including safety of all persons and property,preparatory to and during performance of the work. This requirement shall apply continuously and not be limited to normal working hours. The construction documents and the joint and in several phases of construction hereby contemplated are to be governed, at all times,by applicable provisions of local and state laws and regulations and federal laws, including but not limited to the latest amendments of the following: Department of Labor, Bureau of Labor Standards, Safety and Health Regulations for Construction, and Williams and Steiger Occupational Safety and Health Act of 1970, including rules and regulations issued pursuant thereto,applicable to the work and performance of the contract(OSHA). All trench excavation must comply with all applicable OSHA regulations, and it is the sole responsibility of the contractor to be sure this compliance occurs. If any part of the plans or specifications direct or might be interpreted to direct the contractor to perform any act,procedure, or method that might be construed to cause an OSHA violation,the contractor must notify the engineer in writing before any such act, procedure or method is undertaken by the contractor and the contractor must outline why this act,procedure or method might be construed to be an OSHA violation and the contractor must recommend a method to accomplish the same task that will not result in an OSHA violation, with all such work to be performed at no additional cost to the owner. The contractor is fully responsible for all OSHA compliance on this project. The duty of the engineer to conduct periodic construction review of the contractors performance is not intended to include review of the adequacy of the contractors safety measures in, on, or near the construction site. N. Record Drawings The contractor shall keep a complete and up to date record in red pencil of any and all changes made during construction on one set of plans and specifications. At the completion of the work,the contractor shall furnish Record Drawings indicating the final location of all structures,yard piping,property lines,new roads and pavements, curbs, drainage ditches, etc. The Record Drawings shall also show the size and elevation of all piping, finish grades, and elevation of all structures. O. Temporary Toilets The general contractor shall provide temporary toilet facilities for use by all workmen. Temporary toilet facilities shall comply with local and state sanitation laws and regulations Owner's facilities shall not be used by any contractors' personnel. P. Inspection and Tests The contractor shall allow the engineer ample time and opportunity for testing materials and equipment to be used in the work. He shall advise the engineer promptly upon placing orders for materials and equipment so that arrangements may be made, if desired, for inspection before shipment from the place of manufacture. The contractor shall at all times furnish the engineer and his representatives, facilities including labor, and allow proper time for inspecting and testing materials, equipment and workmanship. The contractor must anticipate that possible delays may be caused him in the execution of his work due to the necessity of materials and equipment being inspected and accepted for use. The contractor shall furnish at his own expense, a sample of materials required by the engineer for testing, and shall make his own arrangements for providing water, electric power, or fuel for the various inspections and test of structures and equipment. Where other tests or analyses are specifically required in other sections of these specifications,the cost thereof shall be borne by the party (owner or contractor) so designated in such sections. The owner will bear the cost of all tests, inspections, or investigations undertaken by the order of the engineer for the purpose of determining conformance with the Contract Documents of such tests, inspections, or investigations are not specifically required by the Contract Documents,and if conformance is ascertained thereby. Whenever nonconformance is determined by the engineer as a result of such test, inspections, or investigations,the contractor shall bear the full cost thereof or shall reimburse the Owner for said costs. In this connection, the cost of any additional tests and investigations, which are ordered by the engineer to ascertain subsequent conformance with the Contract Documents, shall be borne by the contractor. Q. Erosion The general contractor should be familiar with the applicable provisions of the Sedimentation Pollution Control Act of 1973, General Statutes, Chapter 113A, Article 4. The general contractor shall be responsible for incorporating conservational procedures necessary to comply with the Act in minimizing erosion and sediment pollution associated with the construction of the project as directed by the engineer. R. Contractors Qualifications The wastewater system installer shall have been in business for not less than ten(10)years, and shall upon request provide documentation of installing previous system of this size and complexity. The installer shall also hold a current and up to date certification with the North Carolina On-Site Wastewater Contractors Certification Board(NCOWCICB) at the required grade level as outlined by NCOWCICB for this size project. S. Final Clean-Up Before finally leaving the site,the general contractor shall wash and clean all exposed surfaces that have become soiled or marked. Each contractor shall remove from the site of the work all accumulated debris and surplus materials of any kind which result from his operation, including construction equipment,tools, sheds, sanitary enclosures,etc. Each contractor shall leave all equipment, fixtures,and work,which he has installed, in a clean condition. The completed project shall be turned over to the owner in a neat and orderly condition. The site of the work shall be rehabilitated or developed in accordance with other sections of the specifications and the drawings. In the absence of any portion of these requirements,the general contractor shall completely rehabilitate the site to a condition and appearance equal or superior to that which exist just prior to construction, except for those items whose permanent removal or relocation was required in the Contact Documents or order by the owner. T. Final Inspections Upon completion of all work,the contractor shall contact the project engineer, and NCDENR/DWQ, and schedule a final inspection. The final inspection will verify that all work has been installed per the Plans and Specifications and is completely operational and ready for start-up. U. Shop Drawings Shop drawings include but are not limited to, layout drawings in plan and elevation, installation drawings, construction drawings, certified wiring diagrams, interconnecting wiring diagrams, manufacturer's data, etc. Contractor shall be responsible for securing all of the information, details, dimensions, drawings, etc. necessary to prepare the shop drawings required under the contract and to fulfill all other requirements of this contract. Shop drawings shall at minimum, accurately and clearly present the following information: 1. All working and installation dimensions. 2. Arrangement and sectional views. 3. Units of equipment in the proposed positions for installation, details of required attachments and connections and dimensioned locations between units and in relation to the structures. 4.Necessary details and information for making connections between the various trades including, but not limited to,power supplies and interconnection wiring between unit, accessories, appurtenance, etc. Contractor shall submit for review by the engineer shop drawings for all fabricated work and for all manufactured items required to be furnished by the Contract Documents. Structural and all other layout drawings prepared specifically for the project shall have a plan scale of not less than 1/4" = 1. Where manufacturer's publications in the form of catalogs, brochures, illustrations, or other data sheets are submitted in lieu of prepared shop drawings, such submission shall specifically indicate the item for which approval is requested. Identification of items shall be made in ink, and submissions showing only general information are not acceptable. All submissions from subcontractors, manufacturers, or suppliers shall be sent direct to the contractor for checking. Contractor shall thoroughly check all drawings for accuracy and conformance to the intent of the Contract Documents. Drawings found to be inaccurate or otherwise in error shall be returned to the subcontractors, manufactures, or suppliers by the contractor for correction before submitting them to the engineer. All submissions shall be bound, dated, properly labeled and consecutively numbered. Information on the label shall indicate specification section, contract drawing number, subcontractor's, manufacturer's, or suppliers name and the name or type of item the submission covers. Each part of a submission shall be marked and tabulated. Shop drawings shall be submitted as a single complete package for any operating system and shall include all items of equipment and any mechanical units involved or necessary for the functioning of such system. Where applicable, the submission shall include elementary wiring diagrams showing circuit functioning and necessary interconnection wiring diagrams for construction. All submissions shall bear the contractor's stamp of approval certifying that they have been checked for conformance and accuracy. Submissions without the contractors stamp of approval will not be reviewed by the engineer and will be returned to the contractor. If the submissions contain any departures from the Contract Drawings, specific mention thereof shall be made in the contractor's letter of transmittal. Otherwise,the review of such submittals shall not constitute approval of the departure. Where errors, deviation, and/or omissions are discovered at a later date in any of the submissions,the engineer's prior review of the submissions does not relieve the contractor of the responsibility for correcting all errors, deviations, and/or omissions. Submissions shall be transmitted in sufficient time to allow the engineer a reasonable time period for review and processing. Contractor shall transmit six copies of each submission to the engineer for review. Submission shall be accompanied by a letter of transmittal containing date, project title,title of submission, notification of deviation, and any other pertinent data. Submission will be marked by the engineer in one of the following ways: "Approved as Noted"- no exceptions are taken, or minor corrections have been noted and shall be made to final drawings. "Revise and Resubmit"- major corrections have been noted and a resubmittal is required. "Rejected"- submittal generally does not comply with specification, and a resubmittal is required. The engineer will retain four submission copies and return two to the contractor. The contractor shall revise and resubmit submissions as required by the engineer until submissions are acceptable to the engineer. Engineer's review of the contractor's submissions shall in no way relieve the contractor of any of his responsibilities under the contract. An acceptance of a submission shall be interpreted to mean that the engineer has no specific objections to the submitted material, subject to conformance with the contract drawings and specifications. Engineer's review will be confined to general arrangement and compliance with the contract drawings and specifications only and will not be for the purpose of checking dimensions, weights, clearances, fittings,tolerances, interferences, coordination of trades, etc. V. Warranty The contractor warrants to the owner that the projects constructed under this contract will be of good material and workmanship, and agrees to replace promptly any part or parts which by any reason of defective material or workmanship shall fail under normal use, free of negligence or accident, for a minimum of 12 months from the date of acceptance. Such replacement shall be free of any charge to the owner or his representative. Section 2- Clearing and Grubbing A. Clear and grub site as specified on the approved drawing. 1. Clear the site of existing vegetation, existing site improvements and obstructions to prepare for new work as shown on approved plans. a. Remove existing trees, stumps, undergrowth, shrubbery and other vegetation. b. Removal of abandoned utilities and debris which will conflict with the new construction. B. Existing Site Conditions 1. The Contractor shall provide all necessary temporary protection to prevent damage existing site property, adjoining public and private properties outside the limits of the construction project. 2. The Contractor shall contact all utilities for possible buried utilities within the construction area, and well be held responsible for protection of the buried utilities or their damage during construction, use precautions to prevent damage to overhead lines. C. Clearing 1. Remove trees, shrubs, vegetation, obstruction, improvements and debris interfacing with installation of new construction and elsewhere in locations as shown. 2. Use equipment and methods for clearing and grubbing in a manner to prevent damage to trees and plants which are to remain within and adjacent to contract limit and to avoid hazard to public and property. 3. Avoid unnecessary skinning and bruising of bark and cutting, breaking of skinning of roots. Do not tie cables or chains around trunks of remaining trees. 4. Remove stumps from fallen trees to full depth. Grub roots over 1 Y2" diameter to a depth of 12" below natural grade. Use only hand methods for grubbing inside drip line of trees to be left standing. 5. Break up and remove old walks, curbs,pavements, foundations, abandoned drainage structures and any other obstructions to a depth of four feet below grade. 6. Do not operate heavy equipment over the root zones of trees to remain. D. Disposal of Waste Material 1. Except for items to be retained by Owner, the tree and plant materials and waste material, rubble and debris removed during site clearing operations shall be removed from the site and disposed of off Owner's property. 2. Legally dispose of waste materials off Owner's property as they are produced. Do not bury, spread or stockpile debris on site, except as directed by the Owner. 3. Do not bum any debris, except as code and with Owner's consent. 4. This work shall consist of the complete removal and satisfactory disposal of all buildings, structure, old pavements, abandoned pipelines and utilities, and any other obstructions within the limits of the earthwork. 5. Building foundations (including any basement walls and floors)within the limits of the earthwork shall be completely removed. Section 3- Earthwork A. Earthwork shall be performed as indicated on the approved drawing. 1. Earthwork can include, but is not limited to, rough grading, excavation, and finish grading as indicated on approved plans. B. Existing Site Conditions 1. The Contractor shall visit the site and familiarize himself with existing site conditions and the scope of work to be preformed. 2. It shall be the Contractor's responsibility to verify all existing underground utility locations, either shown or not shown on the drawings. 3. There is no subsurface information available on this project. The Contractor shall at his expense, if he deems necessary, make his own investigation of subsurface conditions. 4. There is no rock excavation clause for this project. All excavation shall be unclassified, and the contractor shall include in the lump sum bid,the cost to excavate and remove all materials encountered. Remove and dispose of rock excavation, clay, silt, gravel, hardpan, loose shale and loose stone offsite in a legal manner, unless given permission by the engineer to bury the material onsite. C. Inspection and Testing 1. The Contractor shall include his contract bid to provide the necessary inspection and testing services of a qualified engineering testing laboratory. It shall be the testing laboratories decision as to the compliance of any materials. 2. The Contractor shall notify the testing laboratory and Project Engineer, in writing, five (5) days prior to the start of the earthwork. It shall be the Contractor's responsibility to schedule all inspections and testing with the testing laboratory. The Contractor shall notify the Engineer of all inspections and testing. D. Fill Material 1. All material proposed for use as fill shall be checked for suitability with the Project Engineer prior to beginning work of this section. 2.Natural soil used for embankment material shall be soil classified as SC or CL by the Unified Soil Classification. Soils classified as SM, ML, MH and CH soils may be used with the approval of the Project Engineer. Only soils having a maximum dry density of 90 pcf or greater should be used as fill material 3. Borrow material shall be obtained from approved borrow sources. 4. Provide only fill material that is free from roots, organic matter,trash, frozen material, and stones having a maximum dimension greater than 6". Insure that stones larger than 4", maximum dimension, are not place in upper 6"of fill or embankment. Do not place materials in layers greater than 8"of loose thickness. Place layers in horizontally and compact each layer prior to placing additional fill. 5. Add moisture to or dry by aeration each layer as necessary to meet requirements of compaction. Do not place materials in embankments of fills that exceed optimum moisture content by 5% or are 3%below optimum moisture content for the material. E. Equipment 1. It shall be the responsibility of the Contractor to provide the equipment, in number and types of units,to secure continuously the density requirements of this project. 2. Compact by sheepsfoot rollers,pneumatic rollers,vibrators or other equipment approved by engineer. F. Grading 1. The Contractor shall grade the entire site to the grades as shown on the approved plans, including cutting,filling, compacting of fills,rough grading and fine grading. 2. All rock encounter shall be considered unclassified,and any cost for it's removal shall be included in the contract price bid 3. All soft surface soils encountered during grading shall be undercut and replaced with fill material approved by the Project Engineer. 4. Benching shall take place at all proposed fill sections. Unsuitable materials shall be removed until firm soil is encountered. Benching shall be equal to half the width of construction equipment horizontally, and a minimum of every two (2) feet in vertical lifts. 5. During construction the excavated surfaces shall be properly graded to provide good surface drainage to prevent ponding of water. 6. All rocks 4"in diameter or larger encountered during filling shall be removed and disposed of off site. 7. Placing and Compacting Fill Material: All fill material shall be compacted as specified below,or by other approved methods so as to produce a minimum degree of compaction of 95% Standard Proctor(ASTM D698)for natural soil fill material (granular or clayey). Fill material shall be spread in uniform lifts not exceeding 6 inches in uncompacted thickness. Before compaction begins,the fill shall be brought to a water content that will permit proper compaction by either: (1) aerating the material if it is too wet; or(2) spraying the material with water if it is too dry. Each lift shall be thoroughly mixed before compaction to insure a uniform distribution of water content. When fill is brought to final grade,the contractor shall dress the fill to drain and prevent ponding. 8. After filling is complete and judged to be substantial compliance with this section by the Project Engineer,no further excavation of filling shall be done, except as approved. The contractor shall take all necessary steps to prevent erosion, saturation, or other deterioration of the filled area. Section 4- Wastewater Treatment System Improvements The contractor shall furnish all equipment, supervision, labor,materials and any other incidentals necessary to construct the Pine Hall Elementary School Wastewater Treatment System Improvements. The installation shall meet all requirements of NCDENR/DWQ/Title 15A Environment and Natural Resources and the approved drawings. A. Piping and Fittings 1. PVC pipe and fittings shall be manufactured from Type 1, Grade 1 PVC, in Conformance with ASTM D 1785. Compound from which pipe is produced shall have a design stress rating of 2,000 pounds per square inch at 73 degrees Fahrenheit, as listed by the Plastics Pipe Institute(PPI). Materials from which pipe and fittings are manufactured shall have been tested and approved for conveying potable water by the National Sanitation Foundation(NSF). 2. Pipe shall conform to the requirements of ASTM D 1785 D, size as shown on Contract Drawings. 3. Fittings for pressure service and/or chemical applications shall conform to the requirements of ASTM D 2466 for Schedule 40 and ASTM D 2467 for Schedule 80. For drainage and waste piping, fittings shall be drainage ASTM D 2165. 4. Solvent Cement for PVC Piping: All socket-type connections shall be joined with PVC solvent cement complying with ASTM D 2564. Cement shall have a minimum viscosity of 2,000 cps and be recommended by the pipe manufacturer. B. Precast Tank Approvals The contractor shall excavate and install all precast tanks in the wastewater treatment system improvements as indicated on the approved drawings. This shall include: 1-min. 10,000- gallon recirculation pump tank. 1. Approvals The tank designs shall have been approved by NCDENR/DWQ and shall have the approval number imprinted on the delivered tank. The tank shall be state approved and equal to that manufactured by a state approved tank manufacturer. Concrete shall have a minimum compressive strength of 4,000 psi @ 28 days. Shop drawings shall be submitted to the engineer for approval. 2. Pre-cast Tank Installation Installer shall excavate for and install all pre-cast concrete tanks as indicated on the approved plans. All tanks shall be installed on a six(6)inch layer of washed#57 stone for leveling and support. Installer shall ensure that the tanks remain level and true to grade during and after placement. 3. Leakage Testing All tanks shall be tested for water tightness by an approved method prior to backfilling around tanks. Leakage Test—Leakage from water filled tank cannot be more than %2-inch in 24 hours. Vacuum Test—Provide a minimum vacuum of 4-inch of mercury and hold the vacuum for a minimum of two (2)minutes without any loss. Warning: It is recommended that the negative air pressure testing of the tanks not exceed seven (7) inches of mercury. Precautions should be taken to minimize the potential risk of excessive vacuum levels. 4. Backfilling Once testing of the tanks is complete,the tanks shall be backfilled around in six(6) inch tamped levels with debris free material to a level that directs surface water away from tanks. 5. Provide access risers at each end of the tank as indicated on the approved plans. The tank access risers must be extended at least six inches (6") above finished ground. 6. Prior to installing all tanks associated with this project, the contractor shall verify the finished grades of the site's sanitary sewer distribution system and adjust the tank elevations and riser heights as necessary. C. Septic Tank Provide septic tank in the capacity as indicated on the approved plans. Use NCDENR/DWQ, DPH, On-Site Water Protection Section, approved pre-cast septic tank. 1. Construction - Construct using sound, durable, watertight materials not subject to corrosion by liquid components of sewage; by gases generated in the digestion of sewage; by decay under normal conditions of septic tank usage, or by cracking, buckling and settlement. Provide two compartment tanks with two thirds to three quarters of the required liquid capacity on the first compartment. Interior length to width ratio of all tanks must not exceed 2:1. Locate the baffle wall opening at between 25% and 50% of the liquid depth. Include a minimum of 2-inch vent opening on the baffle wall top. 2. Inlet - Discharge sewage into the septic tank from the collection sewer. Use straight pipe for tank inlet and seal joints watertight. Set inlet invert elevations at least 2" above the outlet invert elevation. 3. Outlet- The septic tanks shall have one (1) outlet, centered in the tank. Use Schedule 40 PVC sanitary connection with Polylok Septic Tank Effluent Filter (Model No. PL-625 or approved equal) at outlet. Install filters per manufacturers recommendation. Septic tank outlet shall be connected to pump tank with 4" Schedule 40 PVC as indicated on the approved plans. Provide a minimum 2.0% slope for this pipe connection. 4. Access Openings -The septic tank shall be provided with access openings, (inlet and outlet), centered in tank, and extended to six(6) inches above the finished ground elevation with concrete risers to dimensions as indicated on the approved drawings. The risers shall have bolted down watertight tops. 5. Filters -Install one (1)Ploylok Septic Tank Effluent Filter(Model No. PL-625, or approved equal), at septic tank outlet. Filter installation shall be per manufactures recommendation. 6. Identifying Markings - The pre-cast septic tanks shall be provided with a suitable legend,identifying the manufacturer and indicating the liquid capacity of the tank in gallons. Cast or stamp legend into the wall at the inlet end within six(6) inches from the wall top. D. Wastewater Treatment System Filter Bed Dosing Tank Provide pump/dosing tank in the capacity as indicated in the approved construction drawings. Use NCDENR,DWQ approved precast pump/dosing tank. 1. Construction-Construct using sound, durable,watertight materials not subject to corrosion by liquid components of sewage;by gases generated in the digestion of sewage;by decay under normal conditions of septic tank usage,or cracking,bucking and settlement. 2. Inlet-Discharge effluent into pump tank from the sand filter bed. Use straight pipe for tank inlet and seal joints watertight. Inlet connection shall be watertight using cast-a-seal gaskets cast in during tanks construction. Provide a minimum 2.0% slope for all piping between tanks. 3. Access Openings- The tank shall be provided with access openings, (inlet and outlet)centered in tank, and extend to 6"above ground level with precast concrete risers as indicated on the approved drawings. The concrete access risers shall be sealed watertight where they join the top of the septic tank. E. Filter Bed Dosing Tanks Pumps and Piping The contractor shall install and put into service two(2)new submersible effluent pumps and piping for dosing of effluent to the sand filter bed as indicated on the approved construction drawings. 1. Install two (2) submersible effluent pumps and piping capable of pumping effluent from the dosing tank to the sand filter bed as indicated on the approved plans. 2. Pump Performance- Each recirculation submersible turbine effluent pump shall be capable of pumping±62 GPM @±14.0 feet of total dynamic head. 3. Each pump shall be equipped with a submersible electric motor,rated for 230 volts, single(1)-phase, 60 hertz service. 4. Pumps shall be provided with an adequate length of jacketed-type so power cord suitable for submersible pump applications to reach the above grade control panel without splicing. The power cord shall also be sized according to NEC and ICEA standards and also meet with P-MSHA approval. A starting box shall be provided with each pump. 5. Each pump shall be retrievable and replaceable without entering the filter bed dosing tank and without requiring that the tanks be drained. 6. The retrieval system for each pump shall consist of a Y2"diameter nylon braid lifting rope or stainless-steel chain. The manufacturer shall furnish special mounting accessories, if required,to accommodate the discharges as shown on the approved plans. 7. The pump discharge piping in the pump tank shall be 2" Schedule 40 PVC. Force main piping beyond H-Valve shall be 3" Schedule 40 PVC. All fittings used must be pressure type fittings and solvent welded. 8. Each pump shall be piped with a union and check valve. Both shall be PVC plastic with solvent weld. Discharge piping at union shall be located within 18"of top of riser on the pump tank. 9. Type 316 stainless steel anchor bolts shall be provided for mounting of all items within the pump tank. 10. Float control switches shall be mercury level control weighted switch, suitable for effluent and sewage applications,rated for 115/230 volts, 18/2 type SJOW CPE cord-standard, and approximately V2"liquid level differential in switching action. Install float control switches as indicated on the Approved Drawings. Provide cable of adequate length to terminate in the pump control panel.No splicing of wire allowed between switch and control box. Conduits between the pump tanks and the control panel shall be sealed to prevent sewer gas from entering the control panel. 11. Piping and Valves-All supply piping and fittings shall be Schedule 40 PVC conforming to ASTM D1785 as shown on plans. All couplings and fittings shall be of polyvinyl chloride compounds(ASTM D1784, Type I, Grade I). 12. Ball Valves-All thermoplastic ball valves shall be constructed from PVC Type I Cell Classification 12452 or CPVC Type IV Cell Classification 23447. All o-rings shall be Viton. All valves shall have Safe-T-Shear stem and double stop polypropylene of CPVC handle. All valve union nuts shall have buttress threads for structural integrity. All seal carriers shall be Safe-T-Blocked. All valve components shall be replaceable for valve serviceability using rebuild kits. All valves shall be warranted against defects in material and workmanship for a period of one year. All valves shall be listed for potable water use by the National Sanitation Foundation(NSF). All '/2"through 2"valves shall be pressure tested at 325 PSI and all 2- '/2"through 6"valves shall be pressure rated at 150 PSI for water at 73 degrees Fahrenheit, as manufactured by Spears Manufacturing Company of approved equal. F. Filter Bed Dosing Control Panel The contractor shall furnish all labor,materials,tools, equipment, and perform all work and services necessary for or incidental to the furnishing and installation of the filter bed dosing control panel for all the pumps associated with the wastewater treatment system for the project, as shown on the approved drawings. 1. Control Panel—Install a control panel by SJE Rhombus (or approved equal) as indicated on the approved plans. 2. The control panel shall be equipped with the following: a. Demand/time dosing capability b. NEMA 4X encloser c. Cycle counter d. Elapsed time meter e. Audio/Visual alarm f. Locking hasp g. Manual pump off switch h. Manual pump on switch i. High water level alarm j. Lag on switches 3. Float control switches shall be equal to Connery mercury level control weighted switch, (or approved equal) suitable for effluent and sewage applications,rated for 115/230 volts, 18/2 type SJOW CPE cord-standard, and approximately '/2" liquid level differential in switching action. Install float control switches as indicated on the approved drawings. Provide cable of adequate length to terminate in the pump control.No splicing of wire allowed between switch and control box. 4. Conduits between the pump tanks and the control panel shall be sealed to prevent sewer gas from entering the control panel. 5. The control panel shall be mounted within ten feet of the pump tank and at a height to allow the Operator in Responsible Charge(ORC)to easily adjust and observe the timer control device and other appurtenances of the control panel, (e.g. center of box to be 3 to 5 feet above finished grade). 6. Float control switches shall be equal to Connery mercury level control weighted switch, (or approved equal) suitable for effluent and sewage applications,rated for 115/230 volts, 18/2 type SJOW CPE cord-standard, and approximately 'A"liquid level differential in switching action. Install float control switches as indicated on the approved drawings. Provide cable of adequate length to terminate in the pump control. No splicing of wire allowed between switch and control box. 7. Conduits between the pumptanks and the controlpanel shall be sealed to prevent sewer gas from entering the control panel. G. Recirculation Force Main Force mains shall be installed along the line and grade as indicated on the approved plans. 1. All force main piping and fittings shall be schedule 40 PVC conforming to ASTM D1785. All coupling and fitting shall be of polyvinyl chloride compounds (ASTM D 1784, Type I, Grade I). 2. The installer shall locate all underground utilities, and is responsible for repairing all damages his organization caused to any Stokes County property during the performance of installation. 3. A coated locating wire(#12 THHN of larger)shall be installed with all force mains. The ends of the locating wires shall terminate at the beginning and end of each force main. 4. Force mains shall be tested in the manner set forth in Section 13 of AWWA Standard C- 600, except the contractor will furnish his own gauges and perform the test at no cost to the owner. Before final acceptance,the force main shall be filled with water, care being taken to expel all air. A pressure test of 150 psi shall be applied to the line at the test pump and shall be maintained at that pressure for a minimum period of two consecutive hours. All defective material found will be replaced by the contractor. All leaking joints shall be made tight. The pipe installation will not be accepted unless and until leakage, evaluated on the pressure test of 150 psi for 2 hours,does not exceed 10.0 gallons per day per mile of pipe per inch of normal diameter. 5. Water and Sewer Line Separations 1. Whenever possible sewer should be laid at least 10' horizontally from any existing or proposed water main. If this spacing is not possible, ductile iron pipe is required. 2. Whenever it is necessary for a water main to cross above the sewer line and a vertical separation of 18 inches cannot be maintained,then the sewer line shall be ductile iron pipe 10' each side of the water line. Whenever a sewer line crosses above a water main,the sewer must be ductile iron pipe 10' each side of the water line. 3. When sanitary sewer crosses storm sewer, allow 1' of vertical separation between pipes. If this cannot be met, ductile iron sanitary sewer, 10' each side of storm sewer will be required. For storm sewers 24"and larger, where clearance above sanitary sewer is less than 1' concrete piers on each side of sanitary sewer to support storm sewer will be required, in addition to ductile iron pipe. Section 5- Force Main Piping A. General The contractor shall furnish all labor,materials,tools, equipment, and perform all work and services necessary for or incidental to the furnishing and installation complete of all pressure pipe construction as shown on drawings and as specified in accordance with provisions of the contract documents and completely coordinated with that of all other trades. Although such work is not specifically shown or specified, all supplementary or miscellaneous items, appurtenances, and devices incidental to or necessary for a sound, secure, complete and compatible installation shall be furnished and installed as part of this work. 1. Quality Standards Procedures for handling, laying,protection, and use of pipe shall be in accordance with the pipe manufacturers recommendations, these specifications, and the requirements of NCDENR/DWQ. 2. Submittals The contractor is to submit to the engineer the manufacturer's name and type of material for all materials used on this project. Such materials shall meet with the approval of the engineer. If the materials submitted do not meet with the approval of the engineer,the contractor is to submit other types and makes that may be approved. 3. Product Delivery, Storage and Handling Units shall be delivered,handled, and maintained in a manner to avoid damage to the pipe. 4. Materials In general,the all pressure pipe and fittings used on this project shall be Schedule 40 PVC pipe, or Schedule 80 where noted. 5. Installation The work included under this article consists of but is not limited to furnishing and installing piping systems used for the purposes of carrying fluids under pressure and shall include pressure testing. Disinfection shall be included for potable water lines. Materials at all times shall be handled in such a manner as to protect them from damage. Pipe and fittings should be handled with mechanical equipment at all times that the work site will permit. At no time shall pipe and fittings be dropped or pushed into ditches. Pipe and fitting interiors shall be protected from foreign matter and shall be inspected for damage and defects prior to installation. In the event foreign matter is present in pipe and fittings, itui shall be removed before installation. All force main pipe shall be in nominal lengths of 18' and shall have at least 48"of cover,unless shown differently on the plans. Pipe shall be laid on true lines as directed by the engineer. Ditches are to be dug of sufficient width to adjust the alignment. Bell holes shall be dug at each joint to permit proper making of the joints, The pipe shall be laid and adjusted so that the alignment with the next succeeding joint will be centered in the joint and the entire pipeline will be in continuous alignment both horizontally and vertically. Pipe joints shall be fitted so that a thoroughly watertight joint will result. All joints will be made in conformance with the manufacturers recommendations for the type of joint selected. All transition joints between different types of pipe shall be made with transition couplings approved on shop drawings showing the complete assembly to scale. Fittings and bends shall be blocked with 3,000 PSI concrete, as detailed on the drawings. Prior to beginning construction,the contractor will contact local utility companies and verify the location of utilities. When existing utilities are in conflict with construction they shall be exposed prior to beginning construction to prevent injury to the utilities. Valves and specials are to be placed as specified by the engineer and as shown on the plans. An iron valve extension box and cover shall be furnished with each valve and shall be placed by the contractor, unless a different style of box is noted on the plans. All valves with an operating nut greater than 3' below grade shall have an extended operator to bring the nut to 6"of finished grade,unless the valve is furnished with a hand wheel or hand operated lever actuator. One tee handle wrench shall be furnished by the contractor to operate all buried valves with an operating nut, and one tee handle wrench for all valves with an operating lever. 6. Pressure Testing Before applying the specified test pressure, all piping shall be thoroughly flushed and Air shall be expelled from the pipe. If outlets are not available at high places,the contractor shall make the necessary taps at points of highest elevations before the test is made. After installation and backfilling of the pressure mains, each section(as required by the engineers)of the pipeline system shall be subject to pressure test a scheduled below. Pipe Description Test Pressure and Type PVC Pressure Testing 150 psi-Hydrostatic Do not test against closed valves at pressures higher than the allowable seating pressure for individual valves. Contractor may test open valves at pressures up to that specified for the valve bodies. In sections of the line where the test pressure are greater than the allowable seating pressures for the valves,the contractor shall provide temporary plugs and restraints as required to test against. Tests may be made of isolated portions of the piping(valve to valve)unless directed by the engineer. Any repairs made in the piping system will subsequently necessitate retesting of the affected portions of the system. The hydrostatic test pressure shall be maintained in the section tested for a period of 3 hours. Any defective material causing excessive leakage shall be repaired or replaced and the test repeated. 7. Protecting Open Pipe Lines All pipes installed under this contract shall be thoroughly blocked against access to the pipe of any water from extraneous sources,any vermin, animals,mud, silt or other deleterious materials by installation of plugs designed for that purpose at every pipe end at all times when the pipe ending is not attended by contractor personnel. Section 6-Trenching, Backfilling, and Compaction General The contractor shall furnish all equipment, supervision, labor,materials,and any other incidentals in connection with the excavation,trenching, and backfilling of underground utilities as shown on the approved construction drawings. Utilities included in this project consist of but is not limited to the installation of the following: • Wastewater and Effluent Force mains • Gravity Wastewater Collection System • Electrical Power and Control Wiring Conduits • Related utilities appurtenances 1. Protection of Existing Utilities a. Verify location and existence of all underground utilities. Omission from or inclusion of located utility items does not constitute non-existence or defmite location. Secure and examine local utility surveyor records for available location data. b. Take necessary precautions to protect existing utilities from damage due to any construction activity. Repair all damages to utility items at sole expense. Assess no cost to owner,engineer,or auxiliary party for damages. Any existing water services damaged by the contractor must be repaired or replaced on the same day the service was damaged. c. Avoid surcharging ditch banks by placing excavated material a sufficient distance back from edge of excavation to prevent slides or caving. Maintain and trim excavated materials in such a manner to be as little inconvenience as possible to public and adjoining property owners. d. Exercise care to avoid excavations below established grade where form earth conditions exist. If over-excavation occurs,backfill in 6" lifts and thoroughly compact with pneumatic. e. Trench stabilization stone(No. 57) shall be used to backfill trench bottom areas that must be undercut due to soft,wet, or yielding conditions. Trench stabilization stone may only be used after the engineers or owners representative has approved the amount of undercutting. Payment basis shall be by the tone of trench stabilization stone placed. f. In case of rock excavation,carry excavation 12"below established grade and backfill with Trench Stabilization stone. Form bell holes in trench such that only the barrel of pipe is firmly supported by bedding material. 2. Trench Backfilling Use only backfill material for trenches that are fee from boulders, large roots, sod, other vegetative or organic matter, and frozen material. Hand or pneumatic tamp backfill under and around pipe up to 24"above top of pipe in lifts not exceeding 8" loose thickness. Backfill and compact remainder of trench in 24"lifts to density specified. Perform pneumatic tamping evenly on both sides of pipe to top of excavation or to a depth such that pipe will not be injured by subsequent method of compaction used to achieve required density. Exercise extreme care in backfilling operations to avoid displacing pipe joints either horizontally or vertically and avoid breaking the pipe. Water ponding for backfill consolidation is not permitted. 3. Compaction Compact all trench backfill in areas under structures,buildings,roads,parking areas and sidewalks as directed by engineer to a density of 95%of maximum dry density (STANDARD PROCTOR) as determined by AASHTO Method T99 (ASTM D-698), expect for the uppermost foot,which shall be compacted to 98%maximum dry density(STANDARD PROCTOR). In all locations not covered above, compact trench backfill to a minimum 95%of maximum dry density(STANDARD PROCTOR), as determined by AASHTO Method T99 (ASTM-698). Perform in-place moisture-density test as ordered by engineer to insure trench backfill complies with requirements. Tests shall be performed by the geotechnical engineering firm(designated by the engineer)and all costs are to be paid by the owner,through the contractors testing allowance. The contractor shall not be allowed any profit mark-up on these testing costs. Copies of test results will be furnished to contractor and engineer. Section 7-Sanitary Sewer Collection System 1. PVC Gravity Sanitary Sewer Pipe(SDR35) a. The PVC sewer pipe shall meet the requirements of ASTM D3034, Type PSM, with a maximum STR of 35. Pipe joints shall be elastomeric gasket,push-on type, in accordance with ASTM D321R. b. PVC Sewer Pipe shall be tested by the Contractor for deflection. The pipe shall be mandrelled with a rigid device sized to be cylindrical in shape and constructed with 9 to 10 evenly spaced arms, The mandrel shall be hand pulled by the Contractor through all sewer lines. Any sections of sewer not passing the mandrel shall be uncovered and the Contractor shall reround or replace the sewer and retest that section of line. The Contractor shall submit mandrel drawings and testing schedule to the engineer for approval. c. All PVC sewer pipe shall be installed in accordance with ASTM D2321. Class I, II or III"Embedment Material"as outlined in ASTM D2321 shall be used for the installation of PVC sewer pipe. 2. Alignment and Grade a. Sanitary sewer lines shall be laid to the line and grade shown on project drawings. If grades are not provided on the project drawings,then the Contractor shall install sewer lines as required by local and state codes. b. Protection shall be afforded to all underground and surface structure using methods acceptable to the Contractor at his expense. c. Deviations from line and grade may be made only with written permission of the Owner when such deviations arise from grade or line conflicts with existing utilities, structures,or other sources of conflict. There will be no extra compensation allowed for measures necessary to protect existing utilities or structures during excavation or construction,to negotiate crossing or existing utilities or structures from the ditch. 3. Water, Storm Sewer and Sanitary Sewer Line Separations and Crossings a. Whenever possible sewer should be laid at least 10'horizontally from any existing or proposed water main. If this spacing is not possible, ductile iron pipe is required. b. Whenever it is necessary for a water main to cross above the sewer line and a vertical separation of 18 inches cannot be maintained,then the sewer line shall be ductile iron pipe 10' each side of the water line. Whenever a sewer line crosses above a water main,the sewer must be ductile iron pipe 10' each side of the water line. c. When sanitary sewer crosses storm sewer, allow 1' of vertical separation between pipes. If this cannot be met,ductile iron sanitary sewer, 10' each side of storm sewer will be required. For storm sewers 24"and larger, where clearance above sanitary sewer is less than 1',concrete piers on each side of sanitary sewer to support storm sewer will be required, in addition to ductile iron pipe. 4. Installation a. Responsibility for materials shall rest solely with the Contractor,the Contractor shall replace at his own expense all material found to be damaged or defective. b. Depth of pipe cover shall be to all applicable codes or as directed otherwise by the Owner. 5. Excavations and Preparation of Trench a. Trench excavation shall be carried out in such a manner as to conform to the line and depth of cover shown on the plans. The trench shall be so braced and drained that workmen may work therein safely and efficiently. It is essential that discharge from pumps be led to natural drainage channels. All excavations is unclassified. b. Trench width may vary with the depth of trench and nature of the excavation materials,but in any case shall be of ample width to permit the pipe to be laid and jointed properly and the back sheeted trench shall be 20",but in no case shall the width be less than 12" greater than the nominal diameter of the pipe except by consent of the Owner. c. Pipe foundation shall at times be such that the pipe rests uniformly along its entire barrel length on the trench bottom. Any part of the trench excavated below grade shall be corrected with approved compacted materials before the pipe is laid. Where the subgrade is unstable or water is present in quantities sufficient to make uniform bedding of the pipe impossible,the Contractor shall stabilize the trench bottom with crushed stone. d. Pipe clearance in rock shall be a minimum of 6"below the pipe and 6" on each side of the pipe. e. Subgrade in rock trench shall be of approved materials thoroughly compacted. Contractor shall furnish suitable material for subgrade at his own expense. f. Rock excavation is unclassified. There will be no extra compensation allowed for rock excavation. g. Sheeting or bracing shall be used wherever necessary to prevent caving of trench banks. Where sheeting or racing are used,the allowable trench width shall been laid and backfilled to a point remove. 2' above the pipe. 6. Property Protection a. Trees,fences, poles and all other property shall be protected unless their removal is authorized, and any property not authorized for removal damaged by the Subcontractor shall be satisfactorily restored by the Subcontractor. 7. Pipe Laying a. Handling of pipe and accessories shall at all times be done in a manner as to prevent damage to lining or body. All pipe, fittings,valves and hydrants shall be carefully lowered into the trench by approved methods. Under no circumstances shall pipe or accessories be dropped or dumped into the trench. b. Pipe shall be clean before it is laid,and any pipe which cannot be cleaned with a swab shall be removed and cleaned with suitable apparatus. Any pipe showing evidence of oil,tar or grease shall be permanently marked and removed from the job until it has been cleaned to the satisfaction of the Owner. c. Laying of pipe and jointing of pipe shall be done according to manufacturers recommendations with care being taken to provide uniform bearing for the pipe. Bell and spigot of pipe shall be cleaned properly lubricated where a mechanical joint or a"crush-on"type joint is employed. d. Open ends of pipe shall be plugged with a standard plug or cap at all lines when pipe laying is not in progress. Trench water shall not be permitted to enter the pipe. 8. Backfilling Pipe a. Backfill around pipe and to a point(2)two feet above barrel of pipe shall be selected materials free of rock and boulders. Backfill shall be in minimum 6" layers and compacted to a minimum of 95%of a maximum soil density. b. Rocks and boulders shall be excluded from backfill for at least(2)two feet above the top of the pipe,and no stone larger than(4)inches in its largest dimension, in any case, shall be used in the backfilling. 9. When sanitary sewer crosses storm sewer, allow 1' of vertical separation between pipes. If this cannot be met, ductile iron sanitary sewer, 10' each side of storm sewer will be required. For storm sewers 24"and larger, where clearance above sanitary sewer is less than 1', concrete piers on each side of sanitary sewer to support storm sewer will be required, in addition to ductile iron pipe. 10. Prescast Manholes a. Precast concrete manholes will be built where shown on the plans or as directed by the Engineer. The inside diameter of the manholes shall be at least four feet(4'). Inverts will be built up to a depth of three quarters (3/4) of the diameter of the pipeline as directed to properly take care of the flow through the manholes and to ease the drop from one pipe to the other. Benches will be slopped for drainage, as shown on the plans. b. Joints between sections of the precast manhole shall be sealed with a Neophrene "0" Ring Gasket. Manholes shall be bedded on a layer of crushed stone. The stone shall be the same as that specified for stabilizing ditches (Size No. 67), and shall be spread in a layer at least six inches (6")tick. c. Manhole steps shall be constructed of reinforced molded polypropylene plastic shell. Steps shall meet or exceed NCDOT STD.840. 66. d. Manhole inverts shall be constructed with a width and height equal to that of the effluent pipe. Alignment and smoothness of the invert shall be so that a minimum of energy loss occurs in the manhole invert. Manholes with precast inverts may be used. e. A flexible connector shall be provided for each inlet and the outlet of all manholes. f. All sanitary sewer manholes shall be vacuum tested for leakage in the presence of the Project Engineer. All testing equipment shall be approved for vacuum testing. All lifting holes shall be plugged with an approved non-shrink grout. All pipe entering the manhole shall be plugged. A vacuum of 10-inches of mercury shall be drawn and the vacuum pump shut off. With the valves closed, the time for the vacuum to drop to 9-inches of mercury shall not be less than that shown in the following table. If the manhole fails the initial test, necessary repairs shall be made with an approved non-shrink grout while the vacuum is still being drawn. Retesting shall proceed and continue until a satisfactory test is accomplished. 11. Low Pressure Air Test for Gravity Sanitary Sewers All gravity sanitary sewer lines shall be low pressure tested prior to being placed in surface. Described below is the procedure for air testing gravity sewer lines. *Summary of Method- The section of the sewer line to be tested is plugged. Low-pressure air is introduced into the plugged line. The amount and rate of air loss is used to determine the acceptability of the section being tested. *Preparation of the Sewer Line- Flush and clean he seer line prior to testing, thus serving to wet the pipe surface as well as clean out any debris. A wetted interior pipe surface will produce more consistent results. Plug all pipe outlets to resist the test pressure. Give special attention to stoppers and laterals. *Procedures- Determine the test duration for the section under test by computation from the applicable equations shown in ASTM C 828, or from prepared air test tables. The pressure-holding time is based on an average holding pressure of 3 psi (21 kPa) gauge or a drop from 3.5 psi (24 kPa)to 2.5 psi (17 kPa) gauge. Surface Restoration a. Upon completion of the installation, all disturbed surfaces and property shall be restored to a condition equal to that existing before construction began, and the Contractor shall furnish all labor and materials incidental thereto. 13. Clean-up The Contractor shall upon completion of his work and prior to final payment, clean-up and remove all debris and waste material from the project site. Section 8- Start-Up 9. Start-up 1. The Contractor shall be responsible for complete system start-up. After start- up is complete, the system shall be fully operational and need no additional work. 2. After start-up, the Contractor shall monitor the system daily for a period of 5 days to ensure the system is operating correctly. If any repairs or adjustments are necessary the Contractor will make corrections immediately without any cost to the Owner. 3. The Contractor shall contact the Owner 48 hours prior to start-up, so the Owner can have necessary representatives present. 4. The Contractor shall provide the Owner's representative instructions in proper operation and maintenance of the system and its equipment. Section 9- Erosion Control General The Contractor shall furnish, install and maintain the erosion control measures as indicated on the approved drawings or as required by governing agencies with modifications. The erosion control measures shall be installed when appropriate and shall be maintained to the approval of governing agencies until all disturbed areas are stabilized. A. Installation and Maintenance 1. The Contractor shall provide erosion control and sedimentation control for all disturbed area included within the project area. 2. The Contractor shall be responsible for maintaining and repairing all erosion control measures until all disturbed areas are stabilized. 3. The Contractor shall provide temporary ground cover as soon as possible. Within 30 days after completion of construction phase of any specific area. 4. Erosion control measures such as silt fence, barriers, sediment traps, rock dams, gravel construction entrance, diversions, or other structures shall begin prior to any land disturbing activity. Additional erosion control measures shall be installed as required during construction. 5. The Contractor shall inspect and maintain all erosion control devices after each storm. (2) ELECTRONIC REPORTING OF MONITORING REPORTS [NCGS 143-215.1 (b)] Federal regulations require electronic submittal of all discharge monitoring reports (DMRs) and program reports and specify that, if a state does not establish a system to receive such submittals, then permittees must submit monitoring data and reports electronically to the Environmental Protection Agency (EPA). The final NPDES Electronic Reporting Rule was adopted and became effective on December 21, 2015. Initial recipient of electronic NPDES information from NPDES-regulated facilities means the entity (EPA or the state authorized by EPA to implement the NPDES program) that is the designated entity for receiving electronic NPDES data [see 40 CFR 127.2(b)]. (3) TEMPORARY MEANS OF DISINFECTION [NCGS 143-215.3 (a)(2)] In the event that the wastewater treatment plant's ultraviolet (UV) disinfection system should fail, underperform, or otherwise be removed from effective service, the permittee or his agent should immediately inform the Water Quality Regional Operations Section staff of the Wilmington Regional Office of this development and discuss temporary, alternate means for disinfection of the effluent. If chlorine compounds are used as temporary means of disinfection, the total residual chlorine (TRC) concentration of the effluent must be monitored on a daily basis while this method of disinfection is in use and values must be reported on the discharge monitoring report. TRC in the effluent may not exceed 17 µg/L; therefore, dechlorination methods may also be necessary to ensure protection of water quality in the receiving stream. In the event that TRC monitoring should occur, the Division shall consider all effluent TRC values reported below 50 pg/L to be in compliance with the permit. However, the Permittee shall continue to record and submit all values reported by a North Carolina certified laboratory (including field certified), even if these values fall below 50 pg/L. Use of a disinfection system other than the permitted UV system during circumstances not deemed as emergency and/or temporary (replacement of lamps, lamp cleaning or maintenance, etc.) will be considered a violation of the terms of this permit.
