REQUEST FOR ADDITIONAL INFORMATION
Dear Mr. Edwards:
On January 13, 2011, the Division of Water Quality (DWQ) received your 401 application dated
January 11, 2011 regarding your request to construct 4,760 linear feet of 12 foot wide access road
and 12,000 linear feet of 8" force main sewer pipe. The DWQ has determined that your
application is incomplete and/or provided inaccurate information as discussed below. The DWQ
will require additional information in order to process your application to impact protected
wetland, streams and/or buffers on the subject property. Therefore, unless we receive the
additional information requested below, we will have to move toward denial of your application
as required by 15A NCAC 2H .0506 and 2B .0233(8) and will place this project on hold as
incomplete until we receive this additional information. Please provide the following information
so that we may continue to review your project.
Additional Information Requested:
1. It is required that plans show placement of culverts and other structures in waters, streams,
and wetlands placed below the elevation of the streambed by one foot for all culverts with a
diameter greater than 48 inches, and 20 percent of the culvert diameter for culverts having a
diameter less than 48 inches, to allow low flow passage of water and aquatic life.

2. It is necessary to show linear feet of culvert impacts and linear feet of riprap placed in
stream for energy dissipation or scour protection on page 6, item (3.) Stream Impacts, on
your PCN form.
3. It is necessary to illustrate, on plan drawings, installation of instream energy dissipaters or
scour protection below streambed elevation as not to interfere with aquatic passage during
low flow conditions.
Please respond in writing to the DWQ within 30 days of receipt of this letter with the requested
information. If we do not hear from you within 30 days, we will assume that you no longer want
to pursue this project and we will consider the project as withdrawn.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Roberto Scheller of the Washington
