STORMWATER MANAGEMENT PLAN (SMP)
On January 17, 1997, the Division of Water Quality (DWQ) issued a 401 Water Quality Certification to
the Crosland Land Co. to place fill within or otherwise impact 3.0 acres of wetlands for the purpose of
constructing a commercial development at Landen Town Center, of which the proposed Kiddie Academy
is part. A stormwater management plan (SMP) was approved by the DWQ on October 29, 2001 to meet
the SMP condition of the 401 Certification.
This office has received a request from Mr. Kevin Caldwell of Geosciences Group, Inc. for acceptance
into the Express Review Program regarding a SMP proposed to treat additional impervious surface
associated with the Kiddie Academy development.
This letter advises you that your project will be accepted into the Express Review Program once the
following items are received:
a. The application fee of $1000.00 made payable to the North Carolina Division of Water Quality
[SMP with one (1) drainage area];
b. Two (2) complete and collated copies of the Express Review Stormwater Management Plan
Application [http•//portal ncdenr org_lweb/wg/sw?/ws/401/express/forms];
c. Two (2) collated copies of all supporting information pertaining to this project including
i. Design calculations for the proposed BMP(s) sized for both on-site and any off-site
drainage;
ii. Most current BMP Supplement completed for each BMP including Design Summary
and Required Items Check List along with all required items (available at
http://portal.ncdenr.org/web/wq/ws/su/bmp-manual ;
iii. Completed, signed and notarized Operation & Maintenance Agreement(s) for the
BMP(s), available at the same website in c.ii above.
d. Two (2) collated copies of full size plans (with detailed construction information) including, but
not limited to, the following:
i. Existing conditions map of project area showing all stream and wetland features;
ii. Proposed site grading and drainage plans showing existing and proposed contours,
stream, wetland and buffer impacts (if any), storm drainage system - Collection points

(inlets) to discharge points (the Outfalls), all impervious areas (buildings, roads, streets,
amenities, parking, etc) and delineated drainage area to each proposed BMP;
iii. Detailed plan and cross sectional views of each BMP showing forebay, pool levels,
bottom and top elevations of berm, outlet structure detail, topographic contours to
receiving stream, etc (at appropriate scales as listed in the Required Items Checklist);
iv. Construction sequence and planting schedule (if applicable) for the proposed BMPs;
e. One (1) data CD of full size plans in TIFF Group 4 format (black and white, not grayscale or
color). If the plans are too large to store in TIFF format, they can be stored in PDF.
Please note that a pre-application meeting has been scheduled for December 28, 2010 at 11:00am at the
address located at the bottom of the first page This meeting is required prior to the submittal of your
co meted application and the owner(s) can waive their right to attend this meeting. The clock for this
project will not start until receipt of the completed application package and required fee. If the drainage
areas exceed what is indicated on the Initial Interest Form and/or the services are different you may be
required to remit an additional fee.
