On January 6, 2021, staff persons Justin Henderson and Alex Lowe, of the NC Division of Water
Resources, Winston-Salem Regional Office conducted a reconnnaissance inspection. This inspection
was prompted by the Permittee's request to rescind the subject permit. Findings observed during this
inspection are summarized below:
1) there was an active and ongoing discharge occurring at the time of this inspection.
2) there was no evidence or reported modifications to suggest the outlet pipe had been
permanently plugged or otherwise modified to prevent the potential for future discharges to occurr.
As a result of the findings described above, the Winston Salem Regional Office does not recommend
that the subject permit be rescinded at this time.
