The Mooresville Regional Office (MRO) conducted a compliance inspection on May 27U', 2015,
which included a land application event of Third Creek residuals under the SYNAGRO Permit; and,
a records review and the residuals processing and storage at the Fourth Creek WWTP.. The MRO
would like to thank Eric Thompson (Synagro) and Andy Smith (Statesville) for their time during the
inspections.
Permit expiration dates are listed below for your convenience.
W00004040 Third Creek Monofill, expires 12/30/2018
The monofill units were not inspected at this time.
W00006245 Statesville Class A — Lime Stabilized, expires 09/30/2020
Log entries for the lime stabilization analyses and cross referenced records for the pH
meter calibration appeared well organized and improved since the previous inspection.
WQ0014543 Statesville Class B - Emergency Land Application, expires 02/28/2019
There is currently no activity under this permit.
WQ0016247 SYNAGRO Class B - Land Application, expires 02/28/2018
This permit is held by SYNAGRO, but they rely on Statesville to collect and analyze for all
required parameters including Pathogen and Vector Attraction Reduction (PVAR), which

��nversation with Andy Smith, the City of Statesville has increased the. residuals
"SOUR" testing to once/month to help address odor and vector concerns noted in a
previous inspection and to ensure VAR is met, particularly with the Third Creek residuals,
which typically have more odors than the Fourth Creek residuals. The once/month
sampling.exceeds the permit requirement; therefore, the City of Statesville may reduce
sampling frequency back to once/quarter at their discretion.

This inspection included an active land application (LA), of Third Creek residuals at field NC-IR-20-03,
under the Synagro — WQ0016247 permit; and, a review of records and residuals storage at Statesville's
Fourth Creek WWTP, for both of their permits referenced above. The 4th creek plant was undergoing
construction for repairs at the lime processing and storage areas, at the time of this visit. Statesville
maintains separate bay areas for the "class A" residuals storage and distribution, and the 3rd creek and 4t`
creek "class B" residuals handled by SYNAGRO.
® Strong odors were noticed in the Class B residuals at the field LA site; however, vectors
(flies) were minimal. Statesville has increased sampling frequency for SOUR testing to
ensure VAR is met. The once/month testing is at the discretion of the City of Statesville and
may be reduced to the permit requirement of once/quarter at anytime.

Adjacent fields had been flagged for application. Per conversation with Eric Thompson the existing maps
were obsolete due to setback changes. The buffers he flagged appeared adequate at the time of this
inspection.

Statesville provides continuous, 24-hour per day, water and sewer service. There is
always a certified operator on duty at each plant as required by the state. This
ensures that the city's water supply and the water returned to nearby streams are
of the best quality possible. Operators are certified through the N.C. Division of
Environmental Management.
Statesville actively promotes beneficial reuse of water and wastewater treatment
byproducts. Water and wastewater processes produce sludge as a byproduct of
treatment. Statesville dewaters the sludge and blends it with lime and a drying
agent to produce an alkaline stabilized product, called Sta-Lime. Sta-Lime is
distributed locally to farmers as a liming agent.

Enclosed are two non -discharge facility inspection reports for the inspections of the class A
lime facility and the residuals land application (emergency) facility conducted on June 28, 2001 by
Ms. Ellen Huffman with this Office.
Both reports indicate compliance, including proper lab data and record keeping. There has
been no land application activity reported since the permit was issued in 1998. The inspection for
permit #WQ0006245 revealed that the City plans to replace the overhead conveyor from the de -
watering facility to the lime mixer/reactor with an auger type conveyor to correct the clogging and
spillage problem.
The City of Statesville is also listed as a source of sludge for a land application permit issued
to SYNAGRO (permit #WQ0016247). This arrangement could cause the City problems with its land
application program should the City's current land application contract be discontinued with
SYNAGRO. Permit WQ 0016247 reflects only 29.4 acres of land available for land application.
There is also the issue with compliance, as SYNAGRO has no treatment control over the sludge that
the City produces. The Mooresville Regional Office plans to recommend that this permit be issued to
the generator (the City) or combined with the City's existing (emergency) land application permit
(WQ0014543) when it is up for renewal.

Andy Smith
Both WWTP's generate biosolids that are currently stabilized using an aerobic digester, and dewatered
using a belt press. After dewatering, the residuals are transferred to the N-Viro lime stabilization process
area where the biosolids are set aside for contract land application off of the plant site. The contractor is
Synagro and they hold permit # WQ0016247 for this. The primary method of biosolids disposal is the N-
Viro process and that permit # is WQ0006245.

The Mooresville Regional Office (MRO) conducted a compliance inspection on June 23rd, 2014,
which included a land application event, records review and residuals processing and storage at
the Third Creek and Fourth Creek WWTPs. The MRO would like to thank Andy Smith for his time
during the inspection.
Please note the expiration dates and potential compliance issues are listed under each permit
below. Time sensitive matters and issues of most concern are highlighted.
W00004040 Third Creek Monofill, expires 12/30/2018
The monofill units were not inspected at this time. This permit is mentioned to acknowledge
the receipt of the 2013 Annual Report, which reflected there was no disposal of residuals in
the monofill.
WQ0006254 Class A — Lime Stabilized, expires 09/30/2015
• The permit renewal application is due 180 days prior to expiration, which is
03131 /2015.
• Records for the pH meter calibration are incomplete. Calibration dates should
match dates for the lime stabilization analyses. This was discussed with Andy
Smith for correction on future log entries.
WQ0014543 Class B - Emergency Land Application, expires 02/28/2019
There is currently no activity under this permit.
WQ0016247 Class B - SYNAGRO Land Application, expires 02/28/2018
This permit is held by SYNAGRO, but they rely on Statesville to collect and analyze for all
required parameters including Pathogen and Vector Attraction Reduction (PVAR). In
addition, the comments below would also apply to Statesville's Class B permit
#W00014543, were it being utilized for land application.
• Odors and flies were detected at both the residuals storage area and the land
application sites. This is an indication that VAR is not being properly met.

• The city of Statesville meets both their PVAR requirements once per quarter. To
maintain compliance with permit conditions, it is recommended that SYNAGRO,
schedule land application events to coincide as close as possible to the PVAR
results time frame, and that ONLY currently, stored and analyzed residuals be
land applied.

This inspection included the Third Creek WWTP, Fourth Creek WWTP, and covers all three City of
Statesville residuals permits: #WQ0006245 (Class A), WQ0014543 (Emergency LA - not used) and
WQ0016247 (Class B - Synagro LA). The third Creek Monofill, permit #WQ0004040, was not inspected
for this report. It is only mentioned here to note the receipt and review of the 2013 Annual Report.
The 4th creek plant was the only plant actively processing sludge (dewatering), at the time of inspection,
and 4th creek is where all residuals are stored. There are separate bay areas for the "class A" residuals
storage and distribution, and the 3rd creek and 4th creek "class B" residuals handled by SYNAGRO. Issues
to address to maintain compliance are noted here and in the cover letter.
• Strong odors and vectors were noticed in the Class B residuals storage areas and at the field
Land Application sites. More timely land application is recommended upon passing PVAR
parameters.
• The pH calibration log for the Alkaline Stabilization meter should have matching dates to
the pH time and temperature records.

4th CREEK & 3rd CREEK WWTP SLUDGE NARRATIVE
Both WWTP's generate biosolids that are currently stabilized using an aerobic digester, and dewatered
using a belt press. After dewatering, the residuals are transferred to the N-Viro lime stabilization process
area where the biosolids are set aside for contract land application off of the plant site. The contractor is
Synagro and they hold permit # WQ0016247 for this. The primary method of biosolids disposal is the N-
Viro process and that permit # is WQ0006245.
