Maria Schutte of the Mooresville Regional Office (MRO).viewed an active land application event on
---April-29 ,-201-5; and, --followed the "transportvehicle to the Mt. Holly WWTP-toview the residuals
storage and loading areas. Analytical data at the plant was located in a locked office and
unavailable for review at that time, but was received by email from the land application contractor
on May. 4th, 2015. The analytical data, facility storage/loading areas and general field site
conditions met permit requirements; however, there are a couple of items noted in the comments
section that should be addressed to maintain overall program compliance.

Comments: * Mt. Holly contracts with Synagro for land application. Buffer flags were in place
and agreed with the application map. Per conversation with Mike Cox (Synagro) he swung the
application tractor a little wide on one turn and applied some material within a parcel buffer. The
land surface at that location was relatively flat and there was no residuals runoff.

The rain gauge was left at a previous field, so a make -shift rain gauge from a cup was being used;
while, this is better than nothing, the MRO recommends a graduated rain gauge be located at field
sites in accordance with permit conditions.
The transportation vehicle did not have a copy of the spill plan. Mr. Cox was able to provide his
copy from the tractor, at my request, as the driver was the one traveling on public roads and Mr.
Cox is very familiar with incident protocol. Mr. Cox further explained that he had several copies
and had given them to drivers the day before, but was assigned a different transport vehicle that
morning and had distributed all of his extra copies. The MRO recommends that Synagro address
this issue to maintain overall compliance for the permittee.

Gaston County
Staff of the Mooresville Regional Office (MRO) conducted a compliance inspection on
February 13th, 2018, which included on -site records management and residuals storage.
This inspection was conducted in conjunction with your NPDES (NC0021156) inspection
for which a separate report was sent.
This permit is currently in modification for additional land with our central office (CO)
permitting staff. MRO staff is scheduled to meet with Doug Shoutd and your Synagro
contractor on February 28th for the land review. You may contact Troy. Dobv(a)ncdenr.gov
with specific questions related to the permit modification.

Other noted conditions:
The third aeration basin, which suffered a wall collapse in 2017. is no lon2,er in use and will not be repaired
since the entire plant is now planned for decommission, upon completion of the pump station connection to
Charlotte Water. Time frame of the connection is uncertain as it is dependent on construction of the new
Charlotte WWTP and additional infrastructure. Residuals from the inoperable aeration basin are to be
many end by pump and haul to an appropriate landfill.

contractor from the aerobic digester ready to be land applied. The permittee performs pH

of inspection. Southern Soil Builders, the current contract hauler for the City of Mount Holly, mailed
these records to the Mooresville Regional Office for this inspection review. After review of the records,
technically, the City is compliant, however; it is recommended that contractor supply the City with
compliance testing records prior to land applying sludge and application records after the application
event as required by the City's permit monitoring and reporting requirements.

The City of Mt. Holly is permitted to land apply annually 500 dry tons of sludge from its WWTP,
56 dry tons of sludge from the flow equalization basin, and 130 dry tons of alum sludge from the
water treatment facility. The storage and truck loading area was clean and well maintained.
Records were not available for review. The land application contractor had not copied the
permittee on application activities as required by the permit. The contractor (SSB) was called
and the information for this inspection was mailed to the MRO. The field applied to was MH2-2
(Charles Killian). Data received from the contractor reflects compliance.
The Permittee faxed a copy of the inspection performed at the time of application on August 21,
2007.

We are in receipt of your letter dated 12/2/02 regarding failure to submit Annual Report for Land Application
of Residual Solids for calendar year of 2001. Please find attached Documents that reveal the city mailed Reports and
that they were received and signed for by a Karen Yates on February 5, 2002 at 10:04 am. The Annual Report was then
forwarded to the Mail Service Center and was signed for by a S. Yarborough. From this point it appears the
Annual Report is completely in the Division of Water Quality's Hands.
From review of the attached information, the City of Mt. Holly is requesting the NOV issued on 12/2/02 to the

Enclosed is the sludge land application inspection report for the inspection conducted on July
3, 2002 by Ms. Ellen Huffman with this Office.
The report indicates compliance with the subject permit. Analytical records were readily
available for review and reflected compliance. However, it was noted that that an inspection log is
required to be kept (by the permittee) as set forth in section IV, part 1 of the permit issued December
28, 19999. It was also noted that the permittee can petition to the Division to reduce the TCLP
monitoring frequency to once per permit cycle, as noted in section III, part 5 of the permit.

The purpose of this letter is a reminder that the monitoring reports required by the subject permit must be
submitted as an Annual Report for calendar year 2001 by no later than March 1, 2002. The report must be
submitted (in triplicate) to the following address:

Due to limitations in storage space, please submit the reports in bound form rather than in notebook form.
Permittees are subject to civil penalty for failing to submit the Annual Report as required by their permit.
Enclosed are the summary and certification sheets that must be used for your annual report depending on
whether your residual is Class A or Class B. Please complete the nessesarry sheets and if necessary make
copies of the blank forms if extras are needed. The forms have not changed since last year (although minor .
aesthetic changes have been made) so you may use those you already have on hand.
You will however note that the forms include information required by EPA as outlined in 40 CFR Part
503. Since the requirements of 40 CFR Part 503 are self implementing, they are legally binding and
enforceable Federal requirements for all persons subject to the requirements of 40 CFR Part 503, even if
these requirements are not currently contained in the state permit. The annual reports for the EPA must be
received by EPA at the following address on or before February 19, 2002.

Comments: The contractor faxed the MRO test results for metals and nutrients as well as the

The Non Discharge Compliance / Enforcement Unit has completed its review of the submitted 1997 Annual Report
of land application activities. State issued permit number WQ0001240 requires the Pathogen and Vector attraction
be re ormed in accordance with 40 CFR 503. This requirement is contained in section III. 6. of the state issued
perlSit. The submitted 1997 Annual Report contains residual data Reference Number: 97 2915. This sample was
taken on July 2, 1997 and received at PAR Laboratories on 7/29/97. The holding time stated in EPA/625/R-92/013;
Control of Pathogens and Vector Attraction in Sewage Sludge is 1 (one) hour.
You are hereby notified that these biosolids did not meet the proof of pathogen and vector attraction required by
permit condition I. 10. and were land applied in violation of state permit number WQ0001240. You must respond
within 15 days as -to this non-compliance situation.
Also please note that residual sampling must be summarized on the appropriate state supplied forms.
Please reference the state issued Permit Number when providing the requested information. All information should
be signed and submitted in triplicate to the attention of Kevin Barnett at the letterhead address.
Nothing in this letter should be taken as removing from you the responsibility or liability for failure to comply with
all applicable state laws and regulations.
