Holly WWTP-toview the residuals storage and loading areas. Analytical data at the plant was located in a locked office and unavailable for review at that time, but was received by email from the land application contractor on May. 4th, 2015. The analytical data, facility storage/loading areas and general field site conditions met permit requirements; however, there are a couple of items noted in the comments section that should be addressed to maintain overall program compliance. Holly contracts with Synagro for land application. Buffer flags were in place and agreed with the application map. Per conversation with Mike Cox (Synagro) he swung the application tractor a little wide on one turn and applied some material within a parcel buffer. The land surface at that location was relatively flat and there was no residuals runoff. The transportation vehicle did not have a copy of the spill plan. Mr. Cox was able to provide his copy from the tractor, at my request, as the driver was the one traveling on public roads and Mr. Cox is very familiar with incident protocol. Mr. Cox further explained that he had several copies and had given them to drivers the day before, but was assigned a different transport vehicle that morning and had distributed all of his extra copies. The MRO recommends that Synagro address this issue to maintain overall compliance for the permittee. This inspection was conducted in conjunction with your NPDES (NC0021156) inspection for which a separate report was sent. This permit is currently in modification for additional land with our central office (CO) permitting staff. MRO staff is scheduled to meet with Doug Shoutd and your Synagro contractor on February 28th for the land review. You may contact Troy. Time frame of the connection is uncertain as it is dependent on construction of the new Charlotte WWTP and additional infrastructure. State issued permit number WQ0001240 requires the Pathogen and Vector attraction be re ormed in accordance with 40 CFR 503. This requirement is contained in section III. 6. of the state issued perlSit. The submitted 1997 Annual Report contains residual data Reference Number: 97 2915. This sample was taken on July 2, 1997 and received at PAR Laboratories on 7/29/97. The holding time stated in EPA/625/R-92/013; Control of Pathogens and Vector Attraction in Sewage Sludge is 1 (one) hour. You are hereby notified that these biosolids did not meet the proof of pathogen and vector attraction required by permit condition I. 10. and were land applied in violation of state permit number WQ0001240. You must respond within 15 days as -to this non-compliance situation. Also please note that residual sampling must be summarized on the appropriate state supplied forms. Please reference the state issued Permit Number when providing the requested information.
