SECTIONONE Introduction
URS Corporation - North Carolina (URS), on behalf of Atlantic Richfield Company (ARCO), a
BP Products North America Inc. affiliated company, is pleased to submit this Groundwater
Monitoring Report for Former BP Station No. 24163 located at 381 Knollwood Street, Winston-
Salem, Forsyth County, North Carolina (site). This report is intended to satisfy the final project
evaluation reporting requirement specified in Part VII, Item 5 of the Renewal Issuance of
Injection Permit WI0400198 (NCDENR, 2013). This report presents the procedures and results
for the groundwater sampling event performed in April 2016.
1.1 SITE DESCRIPTION
The site is located on the eastern side of Knollwood Street, southeast of the intersection with
Interstate 40 Business in Winston-Salem, North Carolina. A USGS topographic map depicting
the site location and surrounding area is provided as Figure 1. The site property is approximately
0.5 acres, is covered by asphalt and concrete, and was formerly developed as a retail fueling
station/convenience store with gasoline, diesel, waste oil, and kerosene Underground Storage
Tanks (UST). All UST systems and buildings have been removed from the site and the property
is currently utilized as a parking lot for an adjacent shopping center. Site geology, hydrogeology,
UST removal history, petroleum release history, environmental investigation and remediation
activity history, surrounding property uses, and site ownership history were presented in detail in
the site Corrective Action Plan (CAP)(URS 2011).
A site map illustrating current and historical site features, including groundwater monitoring well
locations, is provided as Figure 2. Currently, ten Type II monitoring wells (MW-3, MW-5R,
MW-6, MW-8R, and MW-14 through MW-19) and one Type III monitoring well (MW-6i) are
located at the site. Monitoring well construction details are presented in Table 1.
1.2 PROJECT BACKGROUND
Two activated sodium persulfate (ASP) injections were performed at the site in accordance with
the CAP which was approved by the North Carolina Department of Natural Resources
(NCDENR) Division of Waste Management (DWM) UST Section (NCDENR, 2011 a). An
Underground Injection Control (UIC) permit application, summarizing the proposed injection
activities, was submitted to the NCDENR Division of Water Quality (DWQ) on July 5, 2011.
Injection Permit No. WI0400198 was issued by the DWQ on August 9, 2011 (NCDENR,
2011b).
Pre -injection investigation and pilot test activities performed in October 2011 were previously
documented in a summary report, dated April 23, 2012, submitted to both the DWQ and the
DWM UST Section (URS, 2012a). A final ASP injection design, refined based on the results of
the pre -design investigation and pilot test results, was also presented in the April 2012 report.
The first ASP injection event was performed at the site between June 4, 2012 and June 9, 2012.
Injection activities and groundwater sampling performance monitoring were performed in
accordance with the approved CAP and Injection Permit No. WI0400198. Baseline data, June
2012 ASP injection activities, and post -injection sampling data through September 2012 were
documented in the Activated Sodium Persulfate Injection Completion Report (URS, 2012b).
Groundwater sampling events performed in December 2012 and February 2013 were
documented in a Semi -Annual Data Report (URS, 2013b).
SECTIONONE Introduction
A permit renewal application was submitted to NCDENR DWQ on March 26, 2013, (URS,
2013a) in advance of the July 31, 2013, permit expiration date. Renewal Issuance of Injection
Permit WI0400198 was issued by the DWQ on May 24, 2013.
The second ASP injection event was performed at the site between June 3, 2013 and June 6,
2013. Injection activities and groundwater sampling performance monitoring were performed in
accordance with the approved CAP and Injection Permit No. WI0400198. Baseline groundwater
data (May 2013) and June 2013 ASP injection activities were documented in the Quarterly Status
Report submitted in August 2013 (URS, 2013c). Quarterly groundwater sampling events were
conducted in August 2013, November 2013, February 2014, and June 2014 following the second
injection.
NCDENR issued a request for Notice of Residual Petroleum (NORP) on January 23, 2015 and
reduced the risk classification to low. The NORP was recorded with Forsyth County Register of
Deeds and public notice letters were sent to the respective parties. A Notice of No Further
Action was issued by NCDENR on April 28, 2015.
US 1-2
SECTIONTWO Injection Performance Monitoring
2.1 GROUNDWATER SAMPLING PROCEDURES
Groundwater sampling was performed on April 28 and 29, 2016. The sampling was not
completed in one day because the peristaltic pump broke on day one and a backup pump was not
available. The event included sampling of monitoring wells MW-5R, MW-8R, MW-18, and
MW-19 in accordance with the injection well permit and recent correspondence with DWQ.
Prior to sampling, groundwater levels were gauged in each monitoring well utilizing a water
level meter decontaminated with soap and water prior to each measurement. The current water
level measurements are presented along with historical gauging data in Appendix A.
Following gauging, each well was sampled utilizing low -flow techniques with new tubing. After
stabilization criteria were met, groundwater from each well was transferred to pre -labeled,
laboratory -prepared containers and promptly placed on ice in a cooler with a temperature and trip
blank. The samples were shipped under chain -of -custody via overnight courier to Accutest
Laboratories Southeast and analyzed for sulfate utilizing EPA Method 300.
Groundwater pH, dissolved oxygen, temperature, oxidation-reduction potential, and specific
conductance were measured in the field at each well utilizing a calibrated Yellow Springs
Instrument YSI-5569 water quality meter. Purged groundwater was discharged onto a permeable
area of the site in accordance with the UST Section Guidelines for Sampling (NCDENR, 2008).
tS 2-1
SECTIONTHREE
Results and Discussion
3.1 GROUNDWATER SAMPLING RESULTS
Groundwater laboratory analytical results for the April 2016 sampling event are presented in
Table 2. Historical analytical data is presented in Appendix B. The laboratory analytical report
for the groundwater sampling event is included in Appendix C. Groundwater field parameter
data for the sampling event is presented in Table 3. Historical field parameters are presented in
Appendix D. Time -trend graphs of various parameters associated with injection performance
monitoring are included in Appendix E.
3.2 PERFORMANCE MONITORING DISCUSSION
The following list provides a brief summary of notable performance monitoring trends.
• The current groundwater field parameter results were consistent with historical
monitoring events. The pH levels in the monitoring wells ranged from 5.61 (MW-19)
to 8.61 (MW-5R).
• The sulfate concentration in MW-18 was below the groundwater quality standard of
250 mg/L while the sulfate concentrations in MW-5R, MW-8R, and MW-19 were
above the groundwater quality standard; however, the concentrations continue to
decrease.
Based on these conclusions, URS recommends the following course of action:
• It is recommended to continue collecting groundwater samples from MW-5R, MW-
8R, MW-18, and MW-19 and have them analyzed for sulfate on a semi-annual basis
until sulfate concentrations are below the groundwater quality standard for two
consecutive sampling events. Additionally, groundwater field parameters will be
collected during each sampling event.

SECTIONFOUR Statement of Limitations
This report has been prepared based on assumptions made by URS related to data and site
characterization from previous and proposed site investigations. These assumptions, although
thought to be reasonable and appropriate, may not prove to be true in the future. Data and site
characterization utilized to develop this summary report and injection plan are restricted by the
following limitations.
Background information and other data have been furnished to URS by ARCO, NCDENR and/or
third parties, which URS has utilized in preparing this report. URS has relied on this information
as furnished, and is neither responsible for nor has confirmed the accuracy of this information. In
addition, it would be extremely expensive, and perhaps not possible, to perform an investigation
which would completely characterize subsurface geological conditions to ensure the detection of
all materials at the site which now are, or in the future might be, considered "contaminants" by
State or federal regulatory agencies. It is possible that any environmental investigation may fail
to reveal the presence of critical subsurface characteristics or contaminants that may be present.
The failure of URS to discover hazardous materials or contaminants through reasonable and
mutually agreed -upon scopes of work does not guarantee that hazardous materials or
contaminants do not exist at the site. Finally, opinions presented herein apply to the existing and
reasonably foreseeable site conditions at the time of our assessment. Changes in the condition of
this property may occur with time due to natural processes or works of man at the site or on
adjacent properties. Changes in applicable standards may also occur as a result of legislation or
the broadening of knowledge. Accordingly, the findings of this report may be invalidated, wholly
or in part, by changes beyond our control.
No expressed or implied representation or warranty is included or intended in our reports except
that our services were performed, within the limits prescribed by our clients, with the customary
thoroughness and competence of our profession.

Laboratory Report Glossary
Client Sample ID: Normally refers to a point of collection — a monitoring well, discharge outfall, treatment facility
intake, soil core grid location and depth, or any other identification client assigns to a sample.
Lab Sample ID: Letter prefix identifies one of Accutest laboratories and the rest is a consecutive number of the job (or
SDG) received. Number after dash is a sample number and it is unequivocally linked in the LIMS to the Client Sample
ID (see above).
Matrix (Matrix Code):
• AQ- Water Samples
• SO- Soil/Solid Samples
• LIQ- Non -Water Liquid Samples
• OIL- Oil Samples
Matrix Type:
• SW for Surface Water
• SO for Soil/Sediment
• GW for Ground Water
• DW for Drinking Water
All available definitions are found on Chain of Custody form.
Deg. C: Degrees Celsius, measurement of temperature.
Method: Analytical and preparation methods used for the analysis, with the version or revision identified.
Date Sampled: This information is entered from Chain of Custody at the time of login for every sample.
Date Received: When the job was received by Accutest Laboratories.
Percent Solids: Applicable only to SO matrix. For other matrices this field defaults to "n/a".
Run #: Provides information how many attempts were made in the analysis of the sample. LIMS can merge information
from several attempts and lists all of them, including dilution, confirmation, etc. #1 designation is assigned to the
analytical run with majority of analytes reported from it, not necessarily in chronological order.
File ID: Actual instrument data acquisition file that produced the final result. Letter prefix identifies the instrument; the
rest is a consecutive injection number for that instrument.
DF (Dilution Factor): Most common reasons are either to fit into the range of the calibration, or alleviate matrix
interference. DF other than 1 are accompanied with a comment at the end of the sample report.
Analyzed: Date of analysis.
By: Field Technician or Analyst uniquely identified by initials.
Prep Date: Date of sample preparation. If hold time is 72 hours or less, time of preparation is also indicated.
Prep Batch: Letter prefix OP followed by a consecutive number. For VOC analysis preparation happens at the time of
analysis, therefore analytical batch and preparation batch are the same. Size of prep batch is limited to 20 field samples
of similar matrix and the entire batch should be completed within 12 hour time.
Analytical Batch: Letter prefix identifies the instrument and is followed by a consecutive number. Not limited by a
number of samples.
Initial Weight or Initial Volume: Raw sample size used for preparation.
Final Volume: Final volume of extract. If different from method -prescribed volume, reasons are reflected in the
comments at the end of the report form.
CAS Number: Chemical Abstracts Service (CAS), a division of the American Chemical Society.
Compound: Most commonly used names of chemical compounds.
Result: Depending on project requirements, this field could be set up as text, such as ND (for Non Detected) or a
number. The number may be reported with a qualifier.
MDL (Method Detection Limit): This value is defined as 99% probability that analyte above this concentration is
positively (qualitatively) identified.
RL (Reporting Limit): This value is supported by the low calibration standard and defines lowest point of quantitative
identification of analyte.
DL (Detection Limit): The smallest analyte concentration that can be demonstrated to be different from zero or a blank
concentration with 99% confidence. At the DL, the false positive rate (Type I error) is 1 %.
LOD (Limit of Detection): The smallest concentration of a substance that must be present in a sample in order to be
detected at the DL with 99% confidence. At the LOD, the false negative rate (Type II error) is 1%.
LOQ (Limit of Quantitation): The smallest concentration that produces a quantitative result with known and recorded
precision and bias.
SGSACCUTEST
FA33584
Units: ug/1 (micrograms per liter) for aqueous samples and ug/kg (micrograms per kilogram) for solids (or ppb — parts
per billion). The units could be set according to project or state -specific requirements, such as mg/1(milligrams per liter),
or mg/kg (milligrams per kilogram).
Qualifiers (Q): Defmitions of most often used qualifiers are found at the bottom of each result page. Applied depending
on the program — state -specific (Florida A.C. 62-160), CLP-like, AFCEE, DOD QSM, etc.
Tentatively Identified Compound (TIC): Used when client requests a search for analytes that are not part of
instrument calibration. Unknown peaks are compared with published spectral libraries and best match is reported as TIC.
Surrogate (Sl, S2, S3 etc.): are positive controls that are used in most organics methods to ascertain preparation
efficiency and matrix effect in individual samples. These chemicals mimic common method constituents but are unlikely
to be found in real samples. Recoveries can be reported for every analytical run used in the analysis.
IS (Internal Standard ISl, IS2, IS3, etc): quantitative reference used to adjust for instrument performance
fluctuations.
Area (of chromatographic peak): signal intensity directly related to compound concentration.
RT (Retention Time): time required for analyte to traverse the length of analytical column. Used for compound
identification.
ICAL (Initial Calibration): Must pass calibration criteria established by method.
ICV (Independent Calibration Verification): Used to verify ICAL preparation and concentration of calibration points.
CCV (Continuing Calibration Verification): Used to assess calibration status of the instrument and must recover
within established acceptance criteria.
MB (Method Blank): is a negative batch control. MB is an aliquot of matrix free of analyte of interest (either ASTM
Type II water or appropriate solid substance) that is put through all the preparation and possible clean-up steps alongside
investigative (field) samples. MB should be free of interferences above a set level.
BS (Blank Spike, Laboratory Fortified Blank - LFB, Laboratory Control Sample - LCS): is a positive control used
to determine method accuracy - in clean matrix, i.e. matrix free of analytes of interest.
BSD (Blank Spike Duplicate): Used to assess recovery reproducibility - method precision — per analytical method
requirement. %Recovery and Relative Percent Difference (%RPD) are compared with the established acceptance criteria.
MS and/or MSD (Matrix Spike and Matrix Spike Duplicate): positive batch controls which indicate matrix effect on
the precision and accuracy of the method in given sample matrix. Results are expressed in %Recovery and Relative
Percent Difference (%RPD), and compared with the established acceptance criteria.
DUP (Matrix Duplicate): Positive batch control, a way of assessing laboratory's precision; however, the composition
of the samples is unknown and may not yield meaningful results.
REC (Recovery in Percent): expresses method accuracy.
RPD (Relative Percent Difference): expresses method precision.
Limits: Recovery limits for surrogates and spikes

Notes:
1.) Sodium persulfate test kits results of 0.07 g/L are treated as "non -detections", as they are within the
margin -of -error for detection.
2.) Values are not consistant with historic values and may be the result of instrument malfunction.
3.) Dissolved oxygen results above saturation (approximately 9.1 mg/L at 2(°C) are considered to represent
measurement error and are not considered representative of actual dissolved oxygen concentration in
groundwater. Error in measurements for the sensor used, a steady-state polarographic sensor, can be caused
by membrane fouling and interference gases, among other causes (YSI, 2009). The likelihood of
measurement error from these sources may have been heightened by the injection.
4.) Elevated pH is the result of the October 2011 sodium hydroxide pilot test injection adjacent to MW-6.
Subsequent elevated pH measurements in MW-5R, MW-8R, MW-18, and NM-19 are associated with June
2012 and June 2013 sodium persulfate and sodium hydroxide injections.
- activated sodium persulfate injections were performed at the site from June 4, 2012, to June 9, 2012, and
from June 3, 2013, to June 6, 2013.
