As-Built Survey
Dear Sir or Madam:
Enclosed is the as-built survey regarding DWQ Project # 08-1589v4. The action
consisted of placing approximately 10,000 square feet of riprap material on the canal side
of the fish diversion structure to stabilize the interior of the structure. It also included the
maintenance of the existing structure by extending 51 feet of sheet pile from the
previously installed 96 feet in 2008 for a total of 147 feet of sheet pile in the breached
area. The action also extended riprap along 250 feet on the canal side of the fish diversion
structure.

Upon completion of all work approved within the 401 Water Quality Certification and Buffer Rules, and any
subsequent modifications, the applicant is required to return this certificate to the 401 Oversight/Express Permitting Unit
North Carolina Division of Water Quality, 1650 Mail Service Center, Raleigh, NC, 27699-1650. This form may be
returned to DWQ by the applicant, the applicant's authorized agent, or the project engineer. Tt is not necessary to send
certificates from all of these.
