Division of Water Resource records indicate that the above referenced permit, which was issued
January 31, 2008 and expired on December 31, 2016 has not been renewed. It is a condition of your permit
and required by administrative code 15A NCAC 02T .0109 to request renewal of your permit at least 180
days prior to its expiration.
As the Permittee, you are responsible for renewing and maintaining this permit. Failure to do so
will result in assessment of civil penalties in accordance with North Carolina General Statute § 143-215.6A.
Therefore, it is important that you respond by selecting one of the following options within 30' days of
receipt of this notice:
1. APPLY FOR PERMIT RENEWAL — When you apply for and receive a renewal, operation of your
non -discharge system will be permitted to continue. As such, the payment of annual fees, and any
operation and maintenance procedures specified in your permit will be required. You may renew your
residual management system permit by submitting a completed Non -Discharge System Renewal
Application (Form: NDSR 06-16). An electronic version of this application is available for download

2. APPLY FOR PERMIT RESCISSION — If the non -discharge system has not been constructed, or if it
is no longer active, you may request to rescind your permit. By rescinding your permit, all overdue
annual fees will be waived; however, if in the future you wish again to operate the subject non -discharge

Staff of the Mooresville Regional Office (MRO) conducted a technical assistance inspection on
January 25th, 2017, as it was noted this permit expired on December 311t, 2016. The MRO
met with Melinda Beard to discuss options for residuals management. Since the last inspection,
Bessemer City connected the WTP by sanitary sewer to the Two Rivers Utilities Crowder's
Creek facility, for proper handling of the decant water; and, is currently in discussion with
Synagro staff to address removal of Alum residuals from the decant lagoon.
The MRO recommended Ms. Beard contact, nathaniel.thornburg(o)-ncdenr.gov to further discuss
the situation and determine the appropriate steps to either rescind this permit or renew
and return to full compliance. Both the renewal application and the rescission forms may be
located at the NCDEQ-website below.

• The proposed sanitary sewer connection would be to the Crowder's Creek facility not the Cramerton facility.
• The WTP's decant lagoon water is no longer utilized by the neighboring industry. The paragraphdescribing the
former practice under the "Storage" description in your inspection report has been removed, as it was
accidently left in place from the previous inspection.

March 17th, 2016. The facility and records appeared to meet requirements of this
permit, at the time of inspection. In addition, the 2015 Annual Report was received at
the MRO on March 8th, 2016 and indicated there was no activity under this permit in
2015.
The MRO would like to thank Melinda Beard and Jamie Ramsey for their time during the
inspection. Per our conversation, the city is planning to connect the WTP by sanitary
sewer to the Two Rivers Utilities Crowder's Creek facility and is considering the options
to renew or rescind this residuals permit.

This letter is to serve as written notification of the compliance issue I discussed with you via
telephone on the afternoon of December 10.
While preparing information for annual reporting on December 9, a member of our technical
staff discovered that application o esse e City's.W_TE7A-ITiii.residuals occurred on a site that
is not currently listed in the most recently issued version of the City's Land Application Permit
(tWQQ:002264) dated January 31, 2003. This application occurred on October 8 of this year to
the site formally permitted as BC 12-1 (see permit issued June 13, 2001). This site had been
renamed under Synagro's standard site numbering system as NC-GA-112-01, before later being
removed from the City's permit.
Apparently the Synagro Operations Manager was not referencing the newest version of the
project permit and map book and the older permit and map book he used still showed the site as
being permitted. Synagro surface applied 149,500 gallons of WTP residuals to the 11.6 net acres
of the field inaccordance with all buffer and permit requirements that were effective when the
site was permitted. Because the field was in our database due to being a previously permitted
site, the mistake was not realized immediately when the loads were entered in the system.
Since this site was previously permitted and has therefore been determined to be suitable to
receive applications of WTP residuals as well as Class B biosolids, we do not feel that this alum
residuals application poses any environmental threat. In addition, since the only material applied
to the site was alum residuals from the City's WTP, there have been no nutrient loadings to the
site or applications of material requiring site restrictions.
We are very concerned that this incident occurred, and are handling the situation with the same
attention as we would had it involved a Class B biosolids application. We are investigating why
the Operations Manager used the old permit and map book to schedule his application rather than
relying on the new permit and map book package. Synagro has pre -operating procedures in
place that we feel should have prevented this incident from occurring. Synagro will address this
issue with the manager through disciplinary action, as well as work to investigate any potential
new or revised procedures that could serve to further reduce the possibility of this situation

occurring again. We will submit all application data for this field in the annual report just as if it
were a permitted site.

Per our telephone conversation last week, I advised you to submit a permit
renewal application package into us as soon as possible for review. In this
way, the renewal of the permit for land application of water treatment plant
residuals only may proceed. Also, it is DWQ policy to allow land application
under the conditions of the existing permit when an application is being
reviewed by us. So, because the renewal process may take some time, there will
probably be sufficient time to complete the clean out. It will be important,
however, for Synagro SE and the Permittee to keep on top of this to make sure
that the residuals from the WWTP are all cleaned-out/land applied be the time
that the permit renewal is issued.

Qualifier Meaning
J2 Surrogate recovery limits have been exceeded; values are outside lower
control limits
J3 The associated batch QC was outside the established quality control range
for precision.
JS The sample matrix interfered with the ability to make any accurate
determination; spike value is high
J6 The sample matrix interfered with the ability to make any accurate
determination; spike value is low
Qualifier Report Information
ESC utilizes sample and result qualifiers as set forth by the EPA Contract Laboratory Program and
as required by most certifying bodies including NELAC. In addition to the EPA qualifiers adopted
by ESC, we have implemented ESC qualifiers to provide more information pertaining to our analytical
results. Each qualifier is designated in the qualifier explanation as either EPA or ESC.
Data qualifiers are intended to provide the ESC client with more detailed information concerning
the potential bias of reported data. Because of the wide range of constituents and variety of
matrices incorporated by most EPA methods,it is common for some compounds to fall outside of
established ranges. These exceptions are evaluated and all reported data is valid and useable
unless qualified as 'R' (Rejected).
Definitions
Accuracy - The relationship of the observed value of a known sample to the
true value of a known sample. Represented by percent recovery and
relevant to samples such as: control samples, matrix spike recoveries,
surrogate recoveries, etc.
Precision - The agreement between a set of samples or between duplicate samples.
Relates to how close together the results are and is represented by
Relative Percent Differrence.
Surrogate - Organic compounds that are similar in chemical composition, extraction,
and chromotography to analytes of interest. The surrogates are used to
determine the probable response of the group of analytes that are chem-
ically related to the surrogate compound. Surrogates are added to the
sample and carried through all stages of preparation and analyses.

ESC Level 2 Data Package
The data package includes a summary of the analytic results of the quality
control samples required by the SW-846 or CWA methods. The quality control
samples include a method blank, a laboratory control sample, and the matrix
spike/matrix spike duplicate analysis. If a target parameter is outside
the method limits, every sample that is effected is flagged with the
appropriate qualifier in Appendix B of the analytic report.
Method Blank - an aliquot of reagent water carried through the
entire analytic process. The method blank results indicate if
any possible contamination exposure during the sample handling,
digestion or extraction process, and analysis. Concentrations of
target analytes above the reporting limit in the method blank are
qualified with the "B" qualifier.
Laboratory Control Sample - is a sample of known concentration
that is carried through the digestion/extraction and analysis
process. The percent recovery, expressed as a percentage of the
theoretical concentration, has statistical control limits
indicating that the analytic process is "in control". If a
target analyte is outside the control limits for the laboratory
control sample or any other control sample, the parameter is
flagged with a "J4" qualifier for all effected samples.
Matrix Spike and Matrix Spike Duplicate - is two aliquots of an
environmental sample that is spiked with known concentrations of
target analytes. The percent recovery of the target analytes
also has statistical control limits. If any recoveries that are
outside the method control limits, the sample that was selected
for matrix spike/matrix spike duplicate analysis is flagged with
either a "J5" or a "J6". The relative percent difference (%RPD)
between the matrix spike and the matrix spike duplicate
recoveries is all calculated. If the RPD is above the method
limit, the effected samples are flagged with a "J3" qualifier.
