Per previous inspection reports, this facility also maintains NPDES permit # NCO077763 although it is not
being utilized. Currently the decant water from the Alum residuals storage tank is diverted via sewer to the
WWTP. The flow control valve was viewed while on -site. The original permit notes two "plug" devices;
however, per discussion with Mr. Joseph Roy (WTP-ORC), they realized they only needed one diversion "plug"
prior to the river, because diverting flow at the clarifier would have allowed Alum residuals into the system.

Per conversation with the plant ORC (Joseph Roy), this facility also maintains NPDES permit # NCO077763
although it is not being utilized. Currently the decant water from the Alum residuals storage tank is diverted
via sewer to the WWTP. The flow control "plug" is housed in a manhole and was viewed while on -site. The
original permit. notes two "plug" devices; however, per discussion with Mr. Roy, diverting flow at the
clarifier would have allowed Alum residuals into the system. They realized they only needed one diversion
"plug" prior to the river.

Dear Mr. Webb:
The Mooresville Regional Office, Aquifer Protection Section received an odor complaint from a
neighbor on October 1, 2008, regarding a land application event for the subject permit. Based on a site
visit, alum sludge had been land applied that morning on fields owned by James Henkle along Sifford
Road, Lincoln County. Our office inspected the site the same day as the complaint and concluded that
there was only minimal odor associated with this application.
However, it was noted that the alum sludge was land applied at distances less than the required
minimum setbacks noted in Condition 11.10. of the subject permit. Notably, that condition requires
setbacks of 100 feet to a water supply well and to a habitable residence. It also requires a setback of 50
feet to a property line. The residence and well are located at the property of Joseph Henkle, a relative of
James Henkle, at 7083 Sifford Road, on a parcel that appears to have recently been carved out of the
larger parcel owned by James Henkle. This letter is being issued as a notice of deficiency advising you
to be aware of these permit conditions and, in particular, the necessity to revise land application
practices in the field in response to any recent changes in land use at application sites. This notice may
be taken into account should future violations of this nature occur.

Directions:
Sifford Road is accessible off HVVY 16 in 'Lincoln Co south of the
intersection with HWY 73.
Comments:
The site visit confirmed that this was associated with land application of
alum sludge from the Belmont VVTP. Joseph Henkle, part-owner of the
property was present, as was the land applicaiton equipment, but the
appliers were not there. Fresh material was readily apparent on one field
and part of another. There were minimal odors associated with the
applied material. However, it appeared that the application violated
buffers adjacent to the property owner's house and water supply well.
The well was located 88.2 feet from the edge of the applicaiton area and
the house is slightly closer. Property lines were not clear in the field, but
the application area was likely too close as well.

Enclosed, please find a copy of the inspection report for the Part 503 inspection conducted
on September 24, 2003 by the U.S. Environmental Protection Agency and the North Carolina
Department of Environment and Natural Resources. Based on the inspection findings, this office
offers the following comments and recommendations and requests a response within thirty days of
receipt of this letter:
1. Class B pathogen reduction is demonstrated by use of alternative #1: monitoring of fecal
coliform as per 40 CFR 503.32(b)(2). However, the seven samples are collected on one
day. It is recommended that the seven be collected over the monitoring period, or at least
over a two week period. —
2. Vector attraction reduction (VAR) is demonstrated via option #1: reduction of volatile
solids (VS) content as per 40 CFR 503.33(b)(1). Samples are taken from the digesters
and holding tank based on a batch mode operation. The City may monitor VS from the
waste activated sludge line, to take into account the VS reduction across the digesters.
3. Please inform Statesville Analytical that total solids (TS) should be conducted via
Standard Method 2540G.
4-. TS and VS are conducted in=house to demonstrate VAR. The appropriate test method is - _

Introduction:
On September 2.4, 2003, personnel from the United States Environmental Protection
:Agency, (EPA) Region 4, and the North Carolina Department of Environment and Natural
Resources .(NCDENR); conducted a Compliance`Evaluation-Inspection (CEI) at"the-City of
Belmont Wastewater Treatment Plant (WWTP)..The,purpose of the CEI was to evaluate the
City's Part 503 program.

The City owns and operates a 5 million gallons per day (MGD) activated sludge facility.
The City generates a Class B liquid sludge- and land applies on approximately 340 permitted acres.
Southern Soil Builders (SSB), a land applier, is contracted by the City to monitor Part 503
parameters and to land apply City sludge.
Permit:
. The 503 rules are self -implementing. Publicly owned treatment works (POTWs) greater
than one MGD, POTWs that serve a population of 10,000 or greater, and Class I management
facilities that are POTWs required to have an approved pretreatment program, are required to
submit annual reports to the permitting authority.
The facility is also permitted under the NCDENR, NPDES program and, is authorized,
under'NPDES permit no.- NC0021181. In addition, the facility is permitted under the NCDENR
nondischarge permit WQ0003281, for land application.
Rating: satisfactory
Records/Reports:
In-house process data and records concerning Part 503 were kept. Part 503 sampling is
collected by SSB for fecal coliform, inorganic pollutants, and nutrients, and submitted to two
commercial laboratories. Results and chain of custody records ate maintained on -site. The City
monitors and records total solids (TS) and volatile solids (VS) for vector attraction reduction
(VAR).

Annual Report - The City submitted year 2002 annual report to EPA and was received on
February 10, 2003.
The report provided facility name, address, data of inorganic pollutants, nutrients, and
pathogen reduction and VAR requirement data, total amount of sludge disposed, and certification
statements.
Rating: satisfactory
Self Monitoring Program:
Samples for fecal coliform, inorganic pollutants, and nutrients are collected at least once
per quarter from the holding tank. "
Class B pathogenteduction�is demonstrated; by use of alternative"#I: monitoring of fecal `
coli�form as per 40 CFR 503.32(b)(2). The seven samples are collected on one day. It is
recommended that the seven be collected over the'monitoring period; or: at least over a: two week '
period. _
VAR is demonstrated via option #1: reduction of VS content as per 40 CFR 503.33(b)(1).'
Samples are taken from the digesters and holding tank based on a batch mode operation. The
City should monitor VS from the waste activated sludge (WAS) line, as the volatile solids "in"
(VSin) when computing the fractional volatile solids reduction (FVSR). The City uses the Van
Kleeck formula to compute the FVSR.

Site Review:
The facility is a 5 MGD activated sludge WWTP. The sludge treatment process includes
two aerobic digesters, and a sludge holding tank.
The two digesters are 277,000 gallons each. The sludge holding tank has a capacity of 1.5
million gallons. Wasting is based on maintaining a mixed liquor of 1,200 to 1,500 mg/l. Sludge is
wasted into the digesters at 112,OOO gpd. Sludge from the digesters is sent to the holding tank in
batch mode, sampled for VSin, and held for 20 days. VSout is then sampled after the 20 days.
This cycle is repeated until the holding tank is full.

Class A. pathogen reduction is demonstrated by use of alternative #5: use of a Process to -
Further Reduce Pathogens (PFRP) as per 40 CFR 503.32(a)(7). The City operates in a
combination windrow and in -vessel compost facility. NCDENR considers this operation as in -
vessel, in which the minimum standard is: compost temperature is maintained at 55 °C .(131 °F) or
higher for 3 consecutive days. Each pile is turned once per "run" day, and temperatures are
manually checked throughout the piles...
VAR is demonstrated via option #5: aerobic processed at greater than 40°C as per 40
CFR 503.33(b)(5).
All temperatures are logged and summarized in an electronic spreadsheet. Compliance
with the pathogen reduction and VAR are checked manually. Temperature probes are checked
for accuracy.

f Site Review:
The facility is a 6 MGD activated sludge WWTP. The sludge treatment process includes a
sludge holding tank, 2 belt filter presses and the compost operation.
The sludge holding tank has a capacity of 210,000 gallons and receives primary clarifier
sludge and waste activated sludge. Wasting is based on maintaining a mixed liquor. of 3,000 to
3,500 mg/l. The sludge is then dewatered via, two 2-meter Roediger belt presses, operating at 150
-200 gpm. The presses operate up to 3 days per week, 6 hour shifts. The dewatered sludge is
mixed with bulking agents in a ratio of 1:2 sludge to bulking agent on a volume basis.
The covered compost facility includes 10 bays, with each bay having dimensions of 6' x 6'
x 80'; biofilters, storage area, and distribution area. Compost piles are approximately 6' x 4' and
are aerated with an underdrain blower system. Minimum detention time for the piles is 21 days.

Monitoring Frequency - Disposal records were spot checked. From January through
July 2003, the City processed through the belt presses and composted, approximately 718 dry
metric tons. Because the City disposes`its sludge in "other containers", the monitoring frequency
is based on the amount of sludge received, as per 40 CFR 503.16. Extrapolating, the annual
disposal amount will require the City to monitor at the minimum frequency of once per quarter.
The City monitors at least once per quarter. Fecal coliform sampling is once per batch and
temperatures are monitored daily.

.Pathogen Reduction - The most current results were reviewed. The City selected Class
A alternative #5 - PFRP. Fecal coliform data indicated samples were below the 1,000 MPN/gm
regulatory limit. Time and temperature requirements were met. Several fecal coliform results
indicated >1,600 MPN/gm. The City surmised that those results were due to sampling
contamination. However, as a conservative measure, all batches indicating >1,000 MPN/gm were
re -treated.
Vector Attraction Reduction - Disposal records were spot checked. The City selected 40
CFR 503.33(b)(5) option #5. The time and temperature requirements were met.

This is an inspection conducted for a new alum sludge land application permit. Transport
records indicated that Southern Soil Builders land applied sludge in January 2002 on Tony Jones
BT6, field 7. Analytical records were readily available for review and reflected compliance.
The permittee was reminded that an inspection log is required to be kept (by the permittee) as
required by section IV, part 1 of the permit. Belmont also has a land application permit for it's

Introduction:
On September 24, 2003, personnel from the United States Environmental Protection
Agency, (EPA) Region 4, and the North Carolina Department of Environment and Natural
Resources (NCDENR), conducted a Compliance Evaluation Inspection (CEI) at the City of
Belmont Wastewater Treatment Plant (WWTP). The purpose of the CEI was to evaluate the
City's Part 503 program.

The City owns and operates a-5 million gallons per day (MGD) activated sludge facility.
The City generates a Class B liquid sludge and land applies on approximately 340 permitted acres.
Southern Soil Builders (SSB), a land applier, is contracted by the City to monitor Part 503
parameters and to land apply City sludge.
Permit:
The 503 rules are self -implementing. Publicly owned treatment works (POTWs) greater
than one MGD, POTWs that serve a population of 10,000 or greater, and Class I management
facilities that are POTWs required to have an approved pretreatment program, are required to
submit annual reports to the permitting authority.
The facility is also permitted under the NCDENR NPDES program and is authorized
under NPDES permit no. NCO021181. In addition, the facility is permitted under the NCDENR
nondiscbarge permit WQ0003281, for land application.
Rating: satisfactory
Records/Reports:
In-house process data and records concerning Part 503 were kept. Part 503 sampling is
collected by SSB for fecal coliform, inorganic pollutants, and nutrients, and submitted to two
commercial laboratories. Results and chain of custody records are maintained on -site. The City
monitors and records total solids (TS) and volatile solids (VS) for vector attraction reduction
(VAR)-

Annual Report - The City submitted year 2002 annual report to EPA and was received on
February 10, 2003.
The report provided facility name, address, data of inorganic pollutants, nutrients, and
pathogen reduction and VAR requirement data, total amount of sludge disposed, and certification
statements.
Rating: satisfactory
Self Monitoring Program:
Samples for fecal coliform, inorganic pollutants, and nutrients are collected at least once
per quarter from the holding tank.
Class B pathogen reduction is demonstrated by use of alternative 41: monitoring of fecal
coliform as per 40 CFR 503.32(b)(2). The seven samples are collected on one day. It is
recommended that the seven be collected over the monitoring period, or at least over a two week
period.
VAR is demonstrated via option 41: reduction of VS content as per 40 CFR 503.33(b)(1).
Samples are taken from the digesters and holding tank based on a batch mode operation. The
City should monitor VS from the waste activated sludge (WAS) line, as the volatile solids "ir f
(VSin) when computing the fractional volatile solids reduction (FVSR). The City uses the Van
Kleeck formula to compute the FVSR.
Rating: satisfactory
Site Review:
The facility is a 5 MGD activated sludge WWTP. The sludge treatment process includes
two aerobic digesters, and a sludge holding tank.
The two digesters are 277,000 gallons each. The sludge holding tank has a capacity of 1.5
million gallons. Wasting is based on maintaining a mixed liquor of 1,200 to 1,500 mg/l. Sludge is
wasted into the digesters at 112,000 gpd. Sludge from the digesters is sent to the holding tank in
batch mode, sampled for VSin, and held for 20 days. VSout is then sampled after the 20 days.
This cycle is repeated until the holding tank is --iftll.

Laboratory:
For inorganic pollutants, nutrients, and fecal coliform, the City uses two commercial

The results from Statesville Analytical indicated that TS was conducted via Standard
Methods (SM) 2540B. Fecal coliform was conducted via SM 9221 C. TS should be conducted
using SM 2540G.

The commercial laboratories were not evaluated. Chain of custody records were spot
checked. Sample type and preservation of samples appeared to be within acceptable protocol.
The City conducts TS, and VS in-house. Chain of custody, bench sheets, and calculation
records are maintained. However, the City must conduct TS and VS in accordance with SM
2540G.

Sludge Handling/Disposal:
Monitoring Frequency -The annual disposal amount is borderline between the minimum
frequency of once per year and once per quarter. The City monitors at least once per quarter.
VS sampling is per batch.

Pathogen Reduction - The most current results were reviewed. The City selected Class
B alternative #1. Fecal coliform data indicated samples were below the 2 million MPN/gm
regulatory limit.
Vector Attraction Reduction - The most recent records were spot checked. The City
selected 40 CFR 503.33(b)(1) option #1. Results reported indicated FVSR was at least 38%.

Enclosed, please find a copy of the inspection report for the Part 503 inspection conducted
on September 24, 2003 by the U.S. Environmental Protection Agency and the North Carolina
Department of Environment and Natural Resources. Based on the inspection findings, this office
offers the following comments and recommendations and requests a response within thirty days of
receipt of this letter:
Class B pathogen reduction is demonstrated by use of alternative #l: monitoring of fecal

1) Only residuals approved for this permit were applied to the permitted sites.
2) Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or
the limit specified in the permit.
3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three (3) copies
of laboratory results are attached.
4) Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached.
5) All other monitoring was performed in accordance with the permit and reported during the year as required and three (3)
copies of certified laboratory results are attached.
6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T .1105(a) or the Pollutant
Loading Rates in 15A NCAC 02T .1105(b) (applicable to 40 CFR Part 503 regulated facilities).
7) All general requirements in as specified in the Land Application Permit were complied with (applicable to 40 CFR Part
503 regulated facilities).
8) All monitoring and reporting requirements in 15A NCAC 02T .1111 were complied with (applicable to 40 CFR Part
503 regulated facilities).
9) All operations and maintenance requirements in the permit were complied with or, in the case of a deviation, prior
authorization was received from the Division of Water Resources.
10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are
attached to include appropriate actions and remediations.
11) Vegetative cover was maintained and proper crop management was performed on each site receing residuals, as
specified in the permit.
12) No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
13) All buffer requirements as specified on the permit were maintained during each application of residuals.

All of the residuals produced by the City of Belmont's Water Treatment Plant in 2015 were
stored. There is a land application event planned as soon as weather and fields conditions will

1) Only residuals approved for this permit were applied to the permitted sites.
2) Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or
the limit specified in the permit.
3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three (3) copies
of laboratory results are attached.
4) Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached.
5) All other monitoring was performed in accordance with the permit and reported during the year as required and three (3)
copies of certified laboratory results are attached.
6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T .1105(a) or the Pollutant
Loading Rates in 15A NCAC 02T .1105(b) (applicable to 40 CFR Part 503 regulated facilities).
7) All general requirements in as specified in the Land Application Permit were complied with (applicable to 40 CFR Part
503 regulated facilities).
8) All monitoring and reporting requirements in 15A NCAC 02T .1111 were complied with (applicable to 40 CFR Part
503 regulated facilities).
9) All operations and maintenance requirements in the permit were complied with or, in the case of a deviation, prior
authorization was received from the Division of Water Resources.
10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are
attached to include appropriate actions and remediations.
11) Vegetative cover was maintained and proper crop management was performed on each site receing residuals, as
specified in the permit.
12) No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
13) All buffer requirements as specified on the permit were maintained during each application of residuals.

l ) Only residuals approved for this permit were applied to the permitted sites.
2) Soil pH was adjusted as specified in the permit and lime was applied (if needed) to achieve a soil pH of at least 6.0 or
the limit specified in the permit.
3) Annual soils analysis were performed on each site receiving residuals during the past calendar year and three (3) copies
of laboratory results are attached.
4) Annual TCLP analysis (if required) was performed and three (3) copies of certified laboratory results are attached.
5) All other monitoring was performed in accordance with the permit and reported during the year as required and three (3)
copies of certified laboratory results are attached.
6) The facility did not exceed any of the Pollutant Concentration Limits in 15A NCAC 02T .1105(a) or the Pollutant
Loading Rates in 15A NCAC 02T .1105(b) (applicable to 40 CFR Part 503 regulated facilities).
7) All general requirements in as specified in the Land Application Permit were complied with (applicable to 40 CFR Part
503 regulated facilities).
8) All monitoring and reporting requirements in 15A NCAC 02T .1111 were complied with (applicable to 40 CFR Part
503 regulated facilities).
9) All operations and maintenance requirements in the permit were complied with or, in the case of a deviation, prior
authorization was received from the Division of Water Quality.
10) No contravention of Ground Water Quality Standards occurred at a monitoring well or explanations of violations are
attached to include appropriate actions and remediations.
1 1) Vegetative cover was maintained and proper crop management was performed on each site receing residuals, as
specified in the permit.
12) No runoff of residuals from the application sites onto adjacent property or nearby surface waters has occurred.
13) All buffer requirements as specified on the permit were maintained during each application of residuals.
