Dear Mr. Riser:
In accordance with your application received December 6, 1985, we are forwarding
herewith Permit No.13241 dated June 9, 1986, to Thousand Trails Resorts, Inc. for the
construction and operation of the subject wastewater collection, treatment and non -
discharge disposal facilities.
This permit shall be effective from the date of issuance until May 31,1991, and
shall be subject to the conditions and limitations as -specified therein.
If any parts, requirements, or limitations contained in this permit are
unacceptable to you, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of this permit. This request must be
in the form of a written petition, conforming to Chapter.150B of North Carolina
General Statutes, and filed with the Office of Administrative Hearings, Post Office
Drawer 11666, Raleigh North Carolina 27604.
If you wish to have the hearing before the Hearing Officer with this
Department, you must indicate in the petition that you waive the right to have the
contested case conducted by a Hearing Officer in the office of Administrative
Hearings, and wish to have the matter conducted in the Department of Natural Resources
• and Community Development. Unless such demands are made this permit shall be final
and binding.

In accordance with the provisions of Article 21 of Chapter 143, General Statutes
of North Carolina as amended, and other applicable Laws, Rules and Regulations

replacement of four (4) of seven existing pump stations in camping Areas A,B,C and D
with new grinder pumps stations and operation of the existing sewer collection system
serving 200 camp sites and 18 cabins; construction and operation of additional sewer
. facilities to serve 83 new -camp sites in Area-D, said additions consisting of
approximately 4,044 lineal feet of 6-inch gravity sewer with 4-inch laterals to each
camp site, a 6 GPM grinder pump station with dual pumps and high-water alarm, and
a 32 GPM grinder pump station with dual pumps and high-water alarm; modification of
existing septic tanks and/or addition of new auxilliary tanks at each pump
station in Areas A,B,C and D to provide emergency wastewater storage capacity;
construction and operation of approximately 430 lineal feet of 3-inch force main, 455
lineal feet of 4-inch force main, and 935 lineal feet of 6-inch force main; and
construction and operation of a 45,000 GPD extended aeration wastewater treatment
facility with effluent storage lagoon and spray irrigation disposal system, said
facilities consisting.of a 14,800 gallon capacity aerated surge tank with influent
bar screen, dual 74 GPM submersible pumps, and high-water alarm, a 45,000 gallon
capacity aeration tank, dual 285 CFM blowers, a 4500 gallon sludge holding tank, a
7500 gallon clarifier, a 940 gallon baffled chlorine contact tank with hypochlori-
nator, a 0.408 14G capacity lagoon with PVC liner for storage of treated effluent,
and dual 142 CPM effluent dosing pumps discharging to a 9-acre spray irrigation field
with thirty (30) sprinkler heads with no discharge of treated wastewater to the
surface waters of the State.
pursuant to the application received December 6, 1985, and in conformity with the
project plans, specifications, and other supporting data subsequently filed and
approved by the Department of Natural Resources and Community Development and con-
sidered a part of this permit.
This permit shall be effective from the date of issuance until May 31; 1991;
- • and shall be subject to the following specified conditions and limitations:
j_ 1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans,. specifications and other supporting data.

• 2, Construction of the sewers shag: be scheduled so as not to interrupt service by
the existing utilities nor result .n an overflow or bypass discharge of
wastewater to the surface waters of the State.
3. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
4. The facilities shall be properly maintained and operated at all times.
5. The sewage and wastewater collected by this system shall be adequately treated
in the Thousand Trails' Forest Lake Preserve Wastewater Treatment Plant prior to
being sprayed into the irrigation field.
6. This permit is not transferable.
7. The Permittee shall provide for the pump stations and force mains the following
items:
a. Pump-on/pump-off elevations located so that 2-8 pumping cycles may be
achieved per hour in the pump station.
h. An air relief valve located at all high points along the force main.
c. A screened vent for the wet well.
• d. Fillets located in the wet well at the- intersection of. the flooring and y
sidewalls.
e. Three (3) feet of cover (minimum) over the force main or the use of ferrous
material where three (3) feet cannot be maintained.
f. Sufficient devices which will protect the pump station from vandals.
g. Flood protection if the pump station is located below the 100-year flood
elevation.
8. The spray irrigation system shall be effectively maintained and operated at all
times so that there is no discharge to the surface waters, nor any contamination
of ground waters which will render them unsatisfactory for normal use. In event
the facilities fail to perform satisfactory, including the creation of nuisance
conditions, or failure of the irrigation area to adequately absorb the
wastewater, the permittee, shall take such immediate corrective action as may
be required by the Division of Environmental Management.
9. The Winston-Salem Regional Office, telephone number 919/761-2351, shall be
notified at least twenty-four (24) hours in advance of start-up of the installed
wastewater treatment and disposal facilities so that an inspection can be made
of said facilities prior to operation. Such notification to the Regional Supervisor
shall be made during normal office hours from 8:00 a.m.' until 5:00 p.m. on
Monday through Friday, excluding State Holidays.
10. The sludge generated from these treatment facilities must be disposed of in

accordance with General Statute 143.215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
11. The Permittee shall employ a certified wastewater treatment plant operator in
responsible charge of the wastewater treatment facilities. Such operator must
hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
12. Irrigation shall not be performed during inclement weather or when the ground is
in a condition that will cause runoff.
13. Dikes shall be constructed so as to prevent surface water runoff from the spray
field.
14. Diversion or bypassing of the untreated wastewater from the treatment facilities
is prohibited.
15. A suitable year round vegetative cover shall be maintained on the spray
irrigation field:
15. The wastewater application rates shall not exceed:
(a) 0.9 inch/acre/week
(b) 0.11 inch/hour during dosing
(c) the treated wastewater shall be uniformly applied to avoid runoff,
ponding or erosion.
17. Freeboard in the treated wastewater storage lagoon shall not be less than one
(1) foot at any time.
18. The issuance of this permit shall not relieve Thousand Trails, Inc. of the
responsibility for damages to surface or ground waters resulting from the
operation of this facility.
19. The facilities shall be effectively maintained and operated as a non --discharge
system to prevent the discharge of any wastewater into the surface waters of the
State.
20. No type of wastewater other than that from the Thousand Trails, Inc. Forest Lake
Preserve shall be sprayed onto the irrigation area.
21. No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal maintenance is being performed.
22. The wastewater treatment and disposal facilities -.shall be above the 100—year
flood elevation.
23. Any monitoring deemed necessary by the Division of Environmental Management -to
insure surface and ground water protection will be established and an acceptable
sampling reporting schedule shall be followed.
24. Monitoring wells shall be constructed and sampling schedules arranged as
required by the Division of Environmental Management and constructed in
accordance with 10 NCAC 2C 0.100.

. (a) Within 90 days of permit issuance, four (4) monitoring wells (one upgradient
and three downgradient) shall be installed to monitor groundwater quality.
The location and construction details for these wells shall be by approval
of the Division's Wiziston-Salem Regional Office, from which a well construction
permit must be obtained. Note: The four wells may include existing wells GW3
and GW5, previously installed by Law Engineering Testing Company, provided that
the spray pattern can be adjusted so that an adequate buffer zone (as determined
by the Winston-Salem Regional Office) is provided between the spray field and
that Well GW5 is deepened to reach the water table.
(b) The groundwater monitoring wells shall be sampled initially after construction
(and prior to waste disposal operations) and thereafter every March, July,
and November for the following parameters:

The measurement of water level shall be made prior to sampling for the
remaining parameters.
(c) Any additional groundwater monitoring deemed necessary by the Division
shall be provided.
• 25. The Permittee, at least six (6) months prior to the expiration.of this Permit,
shall request its extension. Upon receipt of the request, the Commission will
review the adequacy of the facilities described therein and if indicated, will
extend the Permit for such period of time and under such conditions and
limitations as it may deem appropriate.
