Rescind an Existing Industrial Permit
Complete the Rescission Request Form and mail one hard copy of the form to Suzanne McCoy, 1612 MSC,
Raleigh, NC 27699-1612. In addition, e-mail the form to Suzanne McCoy. There is no fee for industrial permit
rescissions. Please note that it can take 1-2 months to process the request because we cannot rescind the
permit until DEQ staff conduct a site visit.
Also, may I please have April Hardison's email address, phone number and mailing address.

I am working on getting Domtar, here in Plymouth, NC, ready for the upcoming eDMR reporting for stormwater and
need to revise some of our contact information. For the permit contact person, it should be changed from Joe Kloeker to
J.T. Lilley and the phone number should be changed from 252-793-8693 to 252-793-8825. Also the permit billing contact
should be changed from Dennis Askew to April Hardison. Also on the eDMR Owner Verification Report, on page 2 permit
number NCG210411 is referenced. This permit should not be associated with our facility or the responsible official. I
looked up that facility and it was a Domtar facility that is now closed down, but that facility was never associated with
Domtar in Plymouth, NC. Can you remove that permit from out verification report? If you need any other information
from me, please let me know.
