In accordance with the application submitted to the Underground Injection Control (UIC) Program that was received on
8/6/2009, the Aquifer Protection Section (APS) acknowledges your intent to construct a closed-loop geothetmal water-
only injection well system for the operation of a ground-source heat pump located at 1240 Arbor Lane~ Winston Salem ,
Forsyth County, NC 27104. This system is deemed pennitted by rule (North Carolina Administrative Code Title 15A,
Subchapter 2C, Section .021 l(u)(2)).
However, it is recommended that you contact the Forsyth County Health Department, as they may have additional
construction or permitting requirements for this type of system. If you modify your system at any time, including the
addition of antifreeze, corrosion inhibitors, or any other substances to the circulating fluid, you must contact the APS to
verify compliance with applicable niles.

noticed this is going to be a 5QW test well. Will this well or future wells continue to be water -only? If future wells are
going to be converted to SQM, it would be better to permit this as a SQM. That way you do not need to get another
permit and we won't have different permit numbers assigned to the samefacility and owner.

The system is still designed for water only. Some or all of the bores will have steel casing installed to
bedrock, approximate·ly 45 ft. Casing will be grouted from the top of casing down 20 ft. on the outside.
Thermally enhanced grout will be pumped from the bottom of the bores to the top of casing, as stated in
the notification on file.

I noticed this is going to be a SQW test well. Will this well or future wells continue to be water-only? If future wells are
going to be converted to SQM, it would be better to permit this as a SQM. That way you do not need to get another
permit and we won't have different permit numbers assigned to the same facility and owner.
