Sullins Dee Freeman Governor Director Secretary August 23, 2010 CERTIFIED MAIL: RETURN RECEIPT REQUESTED Mr. Gerry Leighton Granville Health System 1010 College Drive Oxford, NC 27565 DWQ Project # 09-1345, Ver. 2 Granville County Subject Property: Granville Medical Center Ut to Jordan Creek [030301, 28-11-5-1, C, NSW] REQUEST FOR MORE INFORMATION Dear Mr. Leighton: On June 17, 2010, the Division of Water Quality (DWQ) received your revised application dated June 16, 2010, to fill or otherwise impact 80 linear feet of perennial stream and 9,480 square feet of Zone 1 Tar- Pamlico River basin protected riparian buffers and 6,459 square feet of Zone 2 Tar-Pamlico River basin protected riparian buffers to conduct the proposed hospital expansion at the site. On August 20, 2010, the DWQ received additional information, however, the DWQ has determined that your application remains incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the additional information requested below, we will place this project on hold as incomplete until we receive this additional information. If we do not receive the requested information, your project will be formally returned as incomplete. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. The depiction of the storm drainage system remains unclear. Some of the issues of concern are: a. On the northern, 0.27 acre parking lot, it appears that a ditch or curbing along the east side of the parking lot will need to be added to direct the stormwater to the BMP. b. On the southern, 0.91 acre parking lot, please provide calculations that show whether the curb will be overtopped during the one inch storm. If the curb is likely to be overtopped during the design storm, then additional inlet structures must be provided. 2. The sediment basin and sand filter do not appear to have enough capacity to store the entire adjusted water quality volume. Please re-design to provide this additional volume and submit calculations to support the design. Please also provide calculations of the draw down rate. 401 Oversight/Express Review Permitting Unit 1650 Mail Service Center, Raleigh, North Carolina 27699-1650 Location: 2321 Crabtree Blvd., Suite 250, Raleigh, North Carolina 27604 Phone: 919-733-17861 FAX: 919-733-6893 Internet: http://portal.ncdenr.org/web/wgtws An Equal Opportunity 1 Affirmative Action Employer Granville Medical Center Page 2 of 2 August 23, 2010 3. The pipes conveying water from the sediment basin to the sand filter is at a very steep slope (over ten percent). In your redesign of the sand filter, please significantly reduce the slope of that pipe to prevent scouring in the sand filter. 4. The embankment that separates the sediment basin from the sand filter looks likely to have stability issues above the PVC pipes. In your redesign of the sand filter, please ensure that there is a continuous, unbroken side slope of no greater than 3:1. Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ. Please call Mr. Ian McMillan or Ms. Amy Chapman at 919-733-1786 if you have any questions regarding or would like to set up a meeting to discuss this matter.
