Exit onto Clemmonsville Road and proceed right to Old Salisbury Road. Turn left and Cone Road is 1.4 miles on the right. The residence is at the end of the road on the right. 6. Site size and expansion area consistent with application ? X Yes No If No, explain: 8. Topography (relationship to flood plain included): The house and system are located adjacent to the stream. The chlorinator tubes are 25 feet from the creek. 9. Location of nearest dwelling: Other dwellings are located to the east of Cone Road ice, 10. Receiving stream or affected surface waters: U.T. to South Fork Muddy Creek a. Classification C b. River Basin and Subbasin No.: YAD-04 C. Describe receiving stream features and pertinent downstream uses: The system discharges into the headwaters of the U.T. behind the house. A large pond is located 3/4 mile downstream. The stream flows into a cow pasture just'50 feet below the system. Part II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. a. Volume of Wastewater to be permitted: 450 MGD (Ultimate Design Capacity) b. Current permitted capacity of the WWTP? 450 gpd. 2. M W C. Actual treatment capacity of the current facility (current design capacity)? The sandfilter is only 90 sq. ft., but is preceded by a ground absorption system. d. Date(s) and construction activities allowed by previous A/C's issued in the previous two years. NA e. Please provide a description of existing wastewater treatment facilities; The existing facility consists of the following: a 900 gallon septic tank, an 80 foot long nitrification line, 90 sq. ft. subsurface sandfilter, tablet chlorinator w/contact tank and discharge pipe. f. Please provide a description of proposed wastewater treatment facilities. NA g. Possible toxic impacts to surface waters: NA h. Pretreatment Program (POTWs only): NA Residuals handling and utilization/disposal scheme: a. Other disposal/utilization scheme (Specify): Septage pumper for disposal in the nearest POTW. Treatment plant classification (See attached rating sheet). NA for SFR. Other Special Items: IV - EVALUATION AND RECOMMENDATIONS An inspection of the system on September 1, 1993, did not reveal any problems. The end of the effluent pipe could not be located _ec`dWo excessive plant growth. Observations of the U.T. revealed clear water with no solids present. WSRO recommends reissuance. The chlorinator shall be inspected weekly to ensure there is an ample supply of chlorine tablets for continuous disinfection of the effluent. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
