5. Directions to Site: Take Highway 150 south of Winston-
Salem. Exit onto Clemmonsville Road and proceed right to
Old Salisbury Road. Turn left and Cone Road is 1.4 miles on
the right. The residence is at the end of the road on the
right.

chlorinator tubes are 25 feet from the creek.

C. Describe receiving stream features and pertinent
downstream uses: The system discharges into the
headwaters of the U.T. behind the house. A large pond
is located 3/4 mile downstream. The stream flows into
a cow pasture just'50 feet below the system.

sq. ft., but is preceded by a ground absorption system.

treatment facilities; The existing facility consists
of the following: a 900 gallon septic tank, an 80 foot
long nitrification line, 90 sq. ft. subsurface
sandfilter, tablet chlorinator w/contact tank and
discharge pipe.
f. Please provide a description of proposed wastewater
treatment facilities. NA
g. Possible toxic impacts to surface waters: NA

IV - EVALUATION AND RECOMMENDATIONS
An inspection of the system on September 1, 1993, did not
reveal any problems. The end of the effluent pipe could not be
located _ec`dWo excessive plant growth. Observations of the U.T.
revealed clear water with no solids present.
WSRO recommends reissuance.

** The daily average dissolved oxygen effluent concentration shall not be less than 6.0 mg/l.
The chlorinator shall be inspected weekly to ensure there is an ample supply of chlorine tablets for continuous disinfection of the effluent.
The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
There shall be no discharge of floating solids or visible foam in other than trace amounts.
