Dear Mr. McClinton:
The Division has received and reviewed your request for modification or waiver of
monitoring requirements. In January of 1984, the Division enacted new minimum monitoring
requirements that apply statewide to all facilities. These new monitoring requirements have
been incorporated into permits as they became eligible for renewal or upon 60 days notice for
those facilities which have a questionable compliance status. The old monitoring frequency was
insufficient to allow the Division to accurately determine the compliance status of most
facilities. In addition, the new requirements have abolished many of the unnecessary influent
and stream monitoring requirements. In accordance with Title 15, North Carolina
Administrative Code (NCAC) Section 213 .0508(b) (1), the Director of the Division of
Environmental Management may modify or waive any of the tests and measurements, sampling
points, or frequency of sampling requirements. Accordingly I have decided to grant your request
to monitor once per month for pH and once per month for Oil and Grease. Attached is a revised
effluent sheet which reflects the requested changes. This sheet should be inserted into your
permit and the old effluent sheet should be discarded.
If any parts, measurement frequencies or sampling requirements contained in this
permit are unacceptable to you, you have the right to an adjudicatory hearing upon written
request within thirty (30) days following receipt of this letter. This request must be in the
form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes,
and filed with the Office of Administrative Hearings, Post Office Drawer 11666, Raleigh, North
Carolina 27604. Unless such demand is made, this decision shall be final and binding.

Enclosed please find a check for $25.00 to, cover the minor modifications requested
below, and an authorization letter from Mr. Humphries of Little Huff, Inc. Requested
modifications for Permit No. NC0075426 are as follows:
H Measurement Frequena from Twice onth to Once/Month
1) pH limits established in the NPDES permit are greater than 6.0 but less than 9.0
for effluent samples. Table 1 shows that pH in the discharge water has been
consistently within the specified range. Delta is requesting that the sampling
frequency be changed from twice/month to once/month.
Oil and Grease Measurement Frequency from TwicelMonth to Once/Month
2) Also shown in Table 1, oil and grease found. in the discharge water has been
consistently well below set daily levels specified in the NPDES permit, with all
measurements below 5 mg/L except for three. Due to the high cost of sampling,
analysis fees and transportation costs, Delta requests the sampling frequency be
changed from twice/month to once/month.

Dear Ms. Harris;
This letter is to inform your office that the April 1990 sample collected from the
treatment system effluent at the above referenced site has failed the mini -chronic
bioassay test. Previously submitted DMR-1 forms for the months of October 1989 and
January 1990 document that the effluent was not toxic to the Ceriodaphnia dubia as
indicated through passing results. These samples were collected during times of proper
system operation where a constant flow was passing through the treatment system.
On the date the first sample was to be collected during April 1990, A Delta
Environmental Consultants, Inc technician found that the system was not operating.
Monitoring data indicates that the system was down for approximately one and one half
weeks. System breakdown was attributed to a clogged upper float control in the
recovery well, which reactivates the pump during cycling events. Clogging was the
result of biological buildup on the float. After the float was cleaned, the
recovery/treatment system was reactivated. However, during the down time, stagnated
water was retained in the treatment system separator tank and time was not allowed for
purging the system with fresh water before sampling. The stagnated water is believed
to be the reason for test failure. Due to previous passing results, the recent failure is
not anticipated to represent future results. ' As specified in the NPDES permit,
arrangements are currently being made to reschedule another sampling event for the
month of March 1990. The results of the testing will be forwarded to your attention
when they are received. Enclosed is a copy of the testing results from April 1990.
