NCO084395 I11 12I 20/10/14 117 18 i c i Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) A routine compliance inspection of the ABC One Hour Cleaners Groundwater Remediation Site was conducted on October 14, 2020. The purpose of the inspection was to verify that the facility is operating in compliance with the conditions and limitation specified within Permit NC0084395. Present during the inspection were Kirsten (Kitty) Hiortdahl (USMC Camp Lejeune, Environmental Managemen Division, Environmental Quality Branch, Installation Restoration Program Manager), Beth Hartzell (Incident Manager for DWM), and Morella Sanchez -King and myself (Bryan Lievre) with DWR. The groundwater treatment building is located on the USMC Base Camp Lejeune, near the southeast corner of NC Hwy 24 and Tarawa Blvd in Jacksonville, NC. The system was designed to recover and treat groundwater contaminated with drycleaning solvents which were documented to have been released from the former ABC One Hour Cleaners which was located at 2127 NC Hwy 24 (Lejeune Blvd). Although other remedial alternatives have been performed at the source area, the purpose of th groundwater recovery and treatment system was to remediate downgradient groundwater contamination. Groundwater was recovered from numerous wells with depths of 28 to 100 feet below grade, located along the south side of NC Hwy 24. Several wells were inspected along the east side of Tarawa Blvd. Each of the wells inspected had a steel manway lid, installed at surface level and had a well cap. The remedial building was secured at arrival. According to the permit, the remedial system was equipped with an equalization tank, dual (2) micron -filter bags, a low -profile air stripper, dual (2) 1-micron bag filters, pH control (acid/caustic feed), a flow meter and a discharge line with an effluent diffuser. Available information indicates that the remedial system has not been in operation circa January 2011. During the recent inspection, the remedial building was found to contain particle filters, a large shallow tray air stripper, a large plastic liquid container as well as assorted hoses and other appurtenances. The vessels were found to have been emptied of their contents and the pump(s) suspected as once being present within the transfer tank was missing. The effluent pipe from the treatment building reportedly travels 1.5 miles underground to the east and discharges along the west side of the streambank of Northeast Creek, south of NC Hwy 24. In the vicinity of the outfall, the Northeast Creek (Stream Segment 19-16-(3.5)) has a classification of SC, HQW, NSW within Subbasin 03-05-02 (HUC:03020301) of the White Oak River Basin. The effluent pipe could not be accessed during the 10/14/2020 inspection, however a follow up inspection was performed by Regional DWR staff on 11/23/2020. During the more recent visit, two 4" diameter pipes were observed in the same general area. The two pipes were separated by the remains of a decayed suspected fallen tree. One 4" line terminated above the water line at the time of inspection was observed to travel inland in a general northwest direction. The second line was a segment 10 to 30 feet long, unconnected at either end, and was fully below the water line during the time of inspection. is suspected that both lines were connected and that the fallen tree disconnected both segments. The facility is in compliance with Permit NC0084395. However, it is understood that annual permit fee: remain unpaid and a Notice of Violation dated 8/6/2018 was issued in regards to this issue. The treatment building (ID # CONT10) is located on USMC Base, Camp Leieune off the southeast corner of NC Hwy 24 and Tarawa Blvd. Access to the facility must be coordinated with the USMC. The treatment plant building was locked upon arrival but access was provided. Since the treatment system has not been in operation circa 2011, persons were not available who are knowledgeable about the exact location of the outfall. An inspection of Regional DWR staff subsequently performed (by boat) on 11/23/2020 verified the presence of two 4" lines in the area of the suspected outfall. It is suspected that a tree fell along the bank, broke the 4" line and now a periodically unsubmerged line is present along with a unattached submerged line. A diffuser is included as a required item within the permit however diffusers were not observed at the end of either line. Although the power meter for the building was still present, additional actvities and supplies would be need to place the systems back into operation.
