We impose this same requirement on original eNOls. Mr. Maimone does not necessarily have to sign the NOT request just because he signed the original application. You just need someone who fulfills the following requirements for the permitted entity (NCSU), per NPDES regulations. This can be someone who is delegated authority by the University to sign such reports. *This form must be signed by a responsible corporate officer that owns or operates the construction activity, such as a president, secretary, treasurer, or vice president, or a manager that is authorized in accordance with Part IV, Section B, Item (6) of the NCGO10000 General Permit. For more information on signatory requirements, see Part IV, Section B, Item (6) of that permit. I hope this gives more guidance. Charles Maimone signed the original Certification form that I submitted... why does he need to submit the online form also? It seems like the hard copy form should suffice since that is considered the original record of certification.
