If you opened two COCs on the same project, then you will need to terminate them both.
Going forward, you do not need to open a second COC on the same project, you can just use our Modification
Form (available on the same web site with the eNOI and eNOT).

Hi Annette,
I have a question and was referred to you by Melissa Joyner. For a site that we are planning to close out soon for a
client, I have found documents showing that there are two NC General Permit Certificate of Coverages, an original and
an amendment (NCC191758 & NCC192121). Will I need to submit two NOT Request forms to close out this project or
will using just the amendment COC # be acceptable? I just want to make sure I tell the client the correct information to
close it out.

Construction for the Burnsville Substation Project in Anson County for Pee Dee Electric Membership Corporation is
complete. We would like to schedule a final close-out inspection and receive approval documentation so that we can
request a Notice of Termination. Certificate of Coverage No. is NCC192121 (attached for reference).
