Expedited permits are generally simple 100% domestics (e.g., schools, mobile home parks, etc) that can be administratively renewed with minor changes, but can include facilities with more complex issues (Special Conditions, 303(d) listed water, toxicity testing, instream monitoring, compliance concerns). New expiration date 7/31/2025 Comments on Draft Permit This facility was issued an NOV after an inspection in May 2018. The NOV stated that the lagoon had not been maintained, noting woody vegetation, animal burrows, and erosion on the slopes. MRO recommended that an engineering evaluation to determine the integrity of the system was needed. The NOV also noted that the effluent pipe location was unknown, and advised that the permittee needed to locate it. UCS, Inc. determined that the effluent structure was leaking and was not properly operating. MRO staff developed a compliance schedule for the facility, but the facility did not meet the deadlines. Another NOV-NOI was issued in December 2018 for the ongoing violations. UCS, Inc. did not respond to DWR attempts to make contact. A Civil Penalty was assessed ($9,683.68) in April 2019, which remains unpaid. In this renewal, conditions were added to the permit to require steps to be taken toward compliance. ➢ Added regulatory citations ➢ Updated eDMR requirements ➢ Updated permit map ➢ Added HUC and Stream Index # to the outfall description on the supplement to cover page. ➢ A special condition was added to require an engineering assessment by an NC licensed Engineer ➢ A special condition was added to require monitoring wells to be installed in locations approved by the MRO AFFIDAVIT OF INSERTION OF ADVERTISEMENT The Gaston Gazette Gastonia, N.C. I concur with issuance of the above permit as I have no appreciable expertise in implementation of the Clean Water Act requirements and have minimal understanding of the information provided. As a result. I have no basis for opposing the permit.
