Division of Water Resources' Central and Regional staff has reviewed the application package
received May 20, 2020. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on August 27, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through C, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper

A. Application:
1. Form Residual Source Certification (RSC 06-16): Please resubmit the Sodium Absorption Ratio
calculations. As per the data provided, the S.A.R values obtained are 0.353
2. Hazardous waste determination: Please submit the TCLP sampling data to ensure compliance with
this condition.
3. Metals determination: Please submit the sampling laboratory data for metals on their dry weight
basis to meet the ceiling concentration limits for Class B residuals.
4. Nutrient/Micronutrient determination: Please submit the sampling data for nutrients and
micronutrients.
5. Pathogen reduction and Vector Attraction Reduction: Please submit the required alkaline
stabilization data as per 15A NCAC 02T. 1106 and 15A NCAC 02T .1107 respectively, to meet
these requirements.
B. Soil Evaluation:
1. Pursuant to 15A NCAC 02T. 1104(c).2. Please submit the soils report for the addition of the new
fields AG-09-1 and AG-09-2. Also include the standard soil tests and the heavy metal sampling
results.
C. Site Map:
1. Please submit the site maps for the new fields AG-09-01 and AG-09-02 as per 15A NCAC 02T.
