Division of Water Resources' Central and Regional staff has reviewed the application package
received September 30, 2020. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on December
10, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A, or failure to provide the additional information on or before the above requested date may
result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

A. Application:
1. Land Application Site Certification form (FORM LASC 06-16): The total acreage of land
application sites currently certified was listed as 52.51 acres, and the proposed acreage to be
certified after the modification is listed as 34.61 acres. The currently certified acreage in the April
22, 2019 permit was 26.56 acres, and the proposed acreage per the application should be 27.98
acres. Please resubmit the application form with corrected acreages.
2. Setback Waiver: Setback waivers shall be executed in accordance with 15A NCAC 02T .1108(e),
which requires that they be written, notarized, signed by all parties involved, and recorded with the
county Register of Deeds. While the waiver submitted was notarized, no record of this waiver was
found in the online Ashe County Register of Deeds. Please submit a copy of the recorded waiver
that lists the Deed Book and Page in order for this waiver to be incorporated into the permit.
