Duncklee & Dunham, P.C. submits this Underground Injection Summary Report for 2009 through 2013
for the above -referenced site. This report presents a summary of injection activities from our last
summary report in 2009 through the expiration of the permit on March 31, 2013. The potassium
permanganate we injected at the site is remediating the source -area plume to the remediation goals
specified in the Corrective Action Plan. However, we will need to make adjustments to our remedial
approach to enhance delivery of the oxidant throughout the plume.

Introduction
1.1 Site Background
The Asheboro plant of the Kayser -Roth Corporation (Kayser -Roth) is located at 515 West Balfour
Avenue in Asheboro, Randolph County, North Carolina (Figure 1). Kayser -Roth manufactures socks at
the plant. The subject site is developed with the plant building and several, smaller outbuildings south
and southeast of the plant (Figure 2). Assessments in 1999 found several chlorinated volatile organic
compounds (VOCs) in groundwater at concentrations that exceeded the North Carolina groundwater
quality standards. The primary constituent detected was tetrachloroethene, which is also called
perchloroethene or PCE. Duncklee & Dunham injected potassium permanganate at the site as part of our
remedial efforts. We prepared an Underground Injection Summary Report dated June 22, 2010 in which
we summarized injection activities from 2003 through 2009. This report summarizes injection activities
and sampling results since our previous report.
L2 Corrective Action Plan and Injection Permitting
In 2001; Duncklee & Dunham, P.C. (Duncklee & Dunham) continued the site -assessment activities that
Gaia Tech, Inc. had begun in 1999 with a soil and groundwater assessment. Subsequently, we prepared a
Corrective Action Plan (CAP) to address groundwater remediation and submitted it to the North Carolina
Department of Environment and Natural Resources (NCDENR) in 2002. The CAP outlined a plan to
address the dissolved PCE plume through in -situ chemical oxidation (ISCO) by the injection of potassium
permanganate. NCDENR issued injection permit W10400033 to Kayser -Roth in early 2003, _and the
permit was amended in 2005 and 2007. Duncklee & Dunham conducted four injection events between
2003 and 2009, and prepared an Underground Injection Summary Report for 2003 through 2009, dated
June 22, 2010, that summarized the results of those events.
Subsequent to our fourth injection event in 2009, Duncklee & Dunham submitted a request to renew the
injection permit in 2010 and requested an amendment to allow a second injection of potable water. The
purpose of the injection was to improve distribution and contact of residual potassium permanganate with
the aquifer by moving the slurry further from the injection zones. NCDENR renewed the permit and
approved the amendment on April 15, 2011, and extended the permit to expire on March 31-, 2013. We
conducted one injection event during this period and have not renewed the permit. Thus, it expired on
March 31, 2013.
2 Injection Activities
Geologic Exploration of Statesville, North Carolina (Geologic) advanced probes with a Geoprobe® at
seven locations in the source area on July 6-8, 2011. The locations of these seven points — Pt 2, Pt 5, Pt 6,
Pt 7, Pt 5-2, Pt 6-2, and Pt 7-2 — are shown on Figure 3. Geologic used a direct -push rig with two-inch
rods to advance each,point to a depth of 27 feet below land surface (bls). We injected potable water from
the City of Asheboro into the shallow, saprolite portion of the aquifer at depths between 23 and 27 feet

bls. Pump pressures ranged from approximately 50 to 170 pounds per. square inch, and the pump rate was
six to eight gallons per minute. We injected 5,920 gallons of potable water into the shallow aquifer.
Construction of Additional Monitoring Wells
Subsequent to the water injection, Duncklee & Dunham oversaw construction of additional monitoring
wells by Geologic in our effort to delineate the lateral and vertical extent of the contaminant plume.
Geologic constructed well numbers MW-23, DW-6 through -13, and CW-1 and -2 in 2011. Duncklee &
Dunham sampled the following wells in 2011: MW-1, -2b, -3, -4, -6b, -7, -8, -1 Ob, -11, -13, -17, -21, -22,
-23, and DW-1 through -9. Upon receipt of laboratory test results, Geologic constructed well nos. MW-
26 through -28, DW-14 through -18, and CW-3 through -6 in 2012. Duncklee & Dunham sampled the
following wells in 2012: MW-4, -12, -13, -17, and -20 through -28; DW-5 through -18; and CW-1
through -6.
During the groundwater -sampling process, we measured geochemical parameters of the groundwater
samples in the field. These data are summarized in Table 1. During well construction in 2012, we used
the Color-Tec® Method in the field to test groundwater samples for PCE to help determine the depths to
set well screens.
4 Direction of Groundwater Flow
Prior to sampling the monitoring wells during each event, we measured the depths to the water table in
the accessible monitoring wells at the site. Table 2 summarizes the depths to the water table in the wells.
We converted the depths measured in November 2012 to elevations and prepared potentiometric maps for
the saprolite and fractured -`rock aquifer (Figures 4 and 5, respectively). These maps show groundwater in
the shallow surficial aquifer flows to the southeast, and groundwater in the fractured -rock aquifer flows
primarily to the southeast and secondarily to the northeast. The flow directions are similar to past events.
5 Laboratory Test Results
The laboratory tested the groundwater samples from sampling events in 2010, 2011, and 2012 for several
chlorinated VOCs, and tested select samples for total chromium, total manganese, and chloride. Table 3
summarizes the test results from monitoring events since our previous injection . summary report, and
includes historical test results prior to these monitoring events. Laboratory test reports are in Appendix
A.
The laboratory tested the sample collected from MW-3 in September 2011 for the HSL metals, the results
of which are shown on Table 4. The sample exhibited manganese and zinc at concentrations that did not
exceed the groundwater quality standards. The laboratory tested selected samples collected in 2010,
2011, and 2012 for VOCs, the results of which are shown on Table 5. Three samples exhibited one
analyte each at concentrations that did not exceed the respective groundwater quality standards.

PCE remains the primary analyte in groundwater at the site. Figures 6 and 7 show PCE concentrations in
the groundwater samples we collected in November 2012 from wells completed in the saprolite and
fractured -bedrock aquifers. PCE concentrations decreased in groundwater samples from the two of the
three saprolite wells in the injection area — MW-8 and -10b — whereas the PCE concentration increased in
the sample from the third saprolite well, MW-6b. PCE concentrations decreased in groundwater samples
collected from three nearby saprolite wells -- MW-3, -13, and -17 -- immediately after injection, but the
concentrations in samples from two of these wells increased during the following sampling event.
Based on the laboratory and field test results, Duncklee & Dunham makes following observations:
• The monitoring wells constructed in saprolite (MW-1 through -28) have delineated the lateral extent
of PCE at concentrations that exceed the North Carolina groundwater quality standard except to the
southeast, where the sample from MW-26 exhibited PCE at a concentration of 4.6 µg/L. We plan to
resample that well this summer to confirm the presence of PCE at this location.
• The monitoring wells constructed in fractured bedrock (DW-1 through -18) and competent bedrock
(CW-1 through -6) have delineated the extent of PCE at concentrations that exceed the groundwater
quality standard to the northwest, nearly so to the northeast -and southeast, and nearly so vertically.
However, the wells have not delineated the extent to the southwest. Duncklee & Dunham plans to
construct additional monitoring wells to the southwest in an attempt to delineate the extent of PCE in
that direction.
• The laboratory detected PCE in samples from 10 of the saprolite wells, 12 of the fractured -bedrock
wells, and one of the competent -bedrock wells. Each of these 23 samples exhibited PCE at a
concentration that exceeded the groundwater quality standard. The samples from MW-17 exhibited
the highest PCE concentrations. The sample collected in April 2010 exhibited PCE at a concentration
of 5,740 µg/L, but the concentrations decreased in each of the successive sampling events to 2,190
µg/L during the event in February 2012.
• The laboratory detected trichloroethene (TCE), a degradation product of PCE, in samples from three
saprolite wells, 14 fractured -bedrock wells, and four competent -bedrock wells. All but one of these
samples exhibited TCE at concentrations that exceeded the groundwater quality standard. The
highest concentration was detected in the sample from DW-8, which exhibited TCE at a concentration
of 49.6 µg/L. The laboratory detected cis-1,2-dichloroethene, another degradation product of PCE, in
the sample from one saprolite well, but the concentration detected did not exceed the groundwater
quality standard.
• The laboratory detected chloroform in samples from 24 of the monitoring wells. However, the
concentrations detected did not exceed the groundwater quality standard. The laboratory also
detected acetone in one sample, 2-butanone in one sample, and trichlorofluoromethane in one sample
collected in 2012. The laboratory did not detect other chlorinated VOCs at concentrations that
exceeded the laboratory detection limits.

Chloride is used to evaluate PCE degradation, and manganese and chromium are used as tracers to
evaluate the distribution and impact of the oxidant to the aquifer. Duncklee & Dunham used the results of
the samples collected from the three saprolite wells in the injection area -- MW-6b, -8, and -10b -- to
evaluate the effectiveness of the injection of potable water to disperse the oxidant into the aquifer. We
expect fractures control the efficiency of this flushing mechanism, whereby the samples that showed the
greatest decreases in concentrations may be better connected hydraulically to the injection area. We
compared the results from the sampling event in April 2010, before injection in July 17, 2011, with the
results from the first sampling event after the event, in September 2011. We make the following
observations:
• Manganese concentrations in the samples from MW-6b, -8, and -10b remained elevated but
decreased, possibly due in part to the flushing of the oxidant further into the aquifer. However,
manganese concentrations also decreased in samples from wells near the injection locations -- MW-3,
-13, and -17 -- and from MW-21, which is more distant to the injection locations. The decrease in
manganese concentrations in the samples from these wells coincides with a decrease in PCE
concentrations.
• Chromium concentrations decreased in samples from MW-6b and -17 and increased in, samples from
MW-8 and -10b. Samples from three of the four wells away from the injection zone -- MW-3, -13,
and -21 -- did not exhibit chromium whereas the sample from the fourth, MW-17, exhibited
chromium at similar concentrations in the events both before and after injection. ,
Chloride concentrations decreased in samples from two of the wells in the injection area and
increased in the sample from the third well. The samples from wells outside the source area -- MW-3
and -17 -- increased, and continue to be elevated in source -area well MW-8.
Field parameters measured during the sampling events in 2010, 2011, and 2012 are shown on Table 1.
The ORP readings for samples from only one well in the injection area, MW-6b, showed an increase,
whereas the ORP reading decreased in the samples from MW-8 and -10b. This contrasts with the
consistent increases in ORP readings after the previous water -injection event. The sample from MW-6b
exhibited the highest ORP reading.
6 Conclusions and Recommendations
6.1 Conclusions
Based on the 2011 injection event and the groundwater -monitoring results, Duncklee & Dunham
concludes the following:
• The permanganate solution and slurry have been effective in treating the PCE and TCE, and has
reduced a large mass of PCE.
• Test results show two degradation compounds, TCE and cis-1,2-dichloroethene, in samples from one
saprolite well, and TCE in samples from three other saprolite wells.

The sample from MW-17 exhibited the highest PCE concentration in the last sampling event.
However, PCE concentrations in the samples from that well have decreased in each of the three
events since water injection in July 2011. The injection of water appears to have had the desired
effect of flushing the oxidant from the injection area to this well, but additional activation of the
permanganate slurry is needed:
The PCE concentrations, in the samples from MW-8 have continued to decrease since our sampling
event in October 2007. The concentration in September 2011 was less than 46.0 µg/L, which is the
lowest since sampling began in 1999. This well is adjacent to and hydraulically downgradient of the
slurry -injection and water -injection area. Although we did not sample this well in 2012, the water -
injection events appear to have had an impact in this well, but additional data are needed to evaluate
the long-term effects.
• PCE concentrations in the samples from DW-7, the only fractured -bedrock well in the injection area,
have increased slightly since the water injection in July 2011. PCE concentrations in the two of the
three nearest fractured -bedrock wells, DW-2 and -6, also increased .slightly after water injection.
These increases may be the result of the formation of PCE-concentration gradients in the aquifer that
formed by the injection of permanganate.
6.2 Recommendations
Based on the 2011 injection event and the groundwater test results, Duncklee & Dunham recommends the
following:
• Continue to use potassium permanganate as the injectate for the remediation of PCE and TCE in the
saprolite and bedrock plumes.
• Collect soil samples from the injection area and test the samples to determine if injectate/injection
slurry remains in the soil.
• Test water samples from select saprolite wells in the water -injection area to ascertain remaining
oxidative strength of the permanganate injectate slurry. If sufficient oxidant is available, conduct a
pilot test by extracting water from DW-1 and/or -2 and injecting it into on -site saprolite wells where
water with permanganate remains and can be used to oxidize the contaminants. If successful, this
closed loop process may be used as an ongoing remedial method and expanded to offsite wells.
• Conduct a sampling event of all wells to evaluate the effectiveness of remediation since previous
sampling events.
