NOT Request Not Approved; NCC190548_Certificate of Cove rage_20190610.pdf
High
This NOT request was rejected earlier this morning because documentation was insufficient. I am reviewing another
one from you for a different store (NCC190548), and it has the same deficiency (A certificate of occupancy is not
adequate close-out documentation — must be final close-out inspection documentation from the E&SC approval
authority).
However, I also realized with this one that your organization is not the original permittee on either of these. The COC
was issued to Circle K Stores, Inc., and that is the organization that must submit the rescission/NOT request. Not
WIMCO Corp. I will have to reject this other one on those grounds, too.
If you have questions, please let us know.

Based on the current guidance to minimize the spread of COVID-19, the Department of Environmental Quality has
adjusted operations to protect the health and safety of the staff and public. Many employees are working remotely or
are on staggered shifts. To accommodate these staffing changes, all DEQ office locations are limiting public access to
appointments only. Please check with the appropriate staff before visiting our offices, as we may be able to handle your
requests by phone or email. We appreciate your patience as we continue to serve the public during this challenging
time.

Great. Thank you all so much. I wanted to make sure that I was taking care of all that was necessary. I'll be sending the

Good Evening Suzanne,
I've completed the NOT Request as attached above. Could you please let me know whether or not the fee of $100 still
needs to be paid as stated in the attached invoice or is this something that would only become due had we not
terminated? Please advise.
Thanks,
