Dear Mr. Salazar:
On November 3, 2020, staff of the North Carolina Division of Water Resources Winston-Salem
Regional Office (DWR) performed a compliance evaluation inspection of the subject wastewater
irrigation system. This compliance inspection was conducted by DWR staff person Justin
Henderson.
The system was noted as not being constructed, and no residence was present at the time of
inspection. In accordance with Permit Condition I. (2), please contact this office at least 48
hours in advance of operation of the permitted facilities such that an in -place inspection
can be made. As a reminder, the system is designed to serve a four (4) bedroom residence or a
total of 480 gallons per day design flow.

On November 3, 2020, staff of the North Carolina Division of Water Resources Winston-Salem Regional Office (DWR)
