Division personnel have reviewed and approved your application. Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General Statute 143-215 .1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and binding. Please note that this permit is not transferable except after notice to the Division. Facility selected ammonia limit. - 2/4/92: ARO conducted a Compliance Biomonitoring Inspection and the LC50 < 1 %, while Total Residual Chlorine (TRC) was 50 mg/l. - 2/26/98: DWQ sends administrative letter requiring facility to conduct monthly acute 48-hour (Ceriodaphnia) toxicity testing. The majority of LC50 values > 90 %. Reduction in TRC believed to have reduced (or eliminated) toxicity. - 5/93: DWQ discontinues requirement by administrative letter. Compliance Summary - Data reviewed from January 1996 — May 1998 indicated the following: BOD5 range: <1 to 35.4 mg/1 (usually < 10 mg/1) NH3N range: <0.1 to 22 mg/l (usually < 1.0 mg/1) Total residual chlorine (TRC) range: 100-400 ug/l, no TRC limit in permit. - 970923 — Compliance Evaluation Inspection conducted by R. Edwards indicated treatment system is well maintained and operating adequately. There were a few system changes that were recommended (see 9/23/97 Edwards letter). - 5/97 — MBAS monitoring violation. - 12/96 — Fecal Coliform daily maximum violation. The mass balance indicated that NH3N should not be limited during the winter. It appears NH3N winter limit was mistakenly in permit. - MBAS (Methyl Blue Active Substance) concentrations are generally <= 0.3 ug/1. 8/15/97: one high value = 19.1 ug/l. MBAS are associated with detergents and are often required in permitted facilities that have laundries. The water quality standard is 500 ug/1(aquatic life). Discontinue MBAS monitoring. It appears MBAS is no longer a parameter of concern. 7/7/98 - R. Edwards (ARO) concurred with decision. - There is no instream monitoring at this facility. Generally, instream monitoring is only required if both of the following are true: 1. DO minimum occurs at point other than discharge location and 2.
