Description'of.treatment process: Shurtape Technologies operates a 10,000 gpd extended aeration type yWVTP to treat domestic: wastewater and: non -contact coolin g water generated by Shurtape's Stony.Pont Tape Plant. The extended aeration plant consists of an inlet bar: screen, aeration: basin, hopper -type: clarifier with a v-notch type weir, tablet= type chlorination, -chlorine contact chamber, tablet de -chlorination, and an aerated sludge holding tank.. Discharge of the treated wastewater is into Third Creek in the Yadkin -Pee Dee River Basin. To optimize the operation of the W V7P, Shurtape conducts process testing'ofMLSS, MLVSS and settleability in the aeration basin. _. II. Performance a.. Description of overall. 12=month performance: The'previous .twelve monthsperformance was reviewed withthe ORC, Mr. Travis Brannon, Certificate No. 990808. FW, Annual System Performance Report Shurtape Stony Point Facility Page 2 of 3 system performance consisted of reviewing the operator's logbook, process control equipment calibration logbooks, sludge removal records, and DMR's from January 2012 through December 2012. During this time period, no effluent violations occurred. During this time period, the process control monitoring equipment was found to have met calibration checks unless further described herein. Buffer solutions used for pH meter calibrations were valid; and sampling results have attached chain of custody forms. The operator logbooks verified that the system had been attended as required with maintenance activities performed. During 2012, there were no events noted in the logbook that would cause the facility to operate in a non -compliant manner. The facility operated in compliance. Two typographical errors not affecting compliance were identified during the records review. First, the March 2012 DMR showed an ORC on -site time for one day as 1 hour, whereas the log book showed 2 hours. Second, the April 2012 DMR showed a downstream sampling time that disagreed with the log book by one minute. These two issues were discussed with Mr. Wes Bell of NCDENR on 6-Feb-2012 and it was agreed that amended DMRs were not required. b. During the time period of January 2012 through December 2012 no effluent violations occurred. C. Description of any known environmental impacts of violations: N/A — No known violations of effluent limitations. d. Description of corrective actions taken to address violations or deficiencies: No known violations existed for the reporting period. e. Shurtape has met the requirements to conduct analysis of field parameters on site through Lab Certification Procedures. f. Lab Certification No. 5087; EPA Lab ID No. NC01311 Proficiency testing was completed during July 2012. The approval report was issued 13-Aug-2012. g. Notification: Shurtape's WWTP serves the manufacturing facility at Stony Point. There are no other users connected to the system. Therefore, Shurtape is the only user of the system. Note that a copy of this report is sent to the Senior Vice President of Manufacturing. III. Certification I certify, under penalty of law, that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. The March DMR has one day that showed 1 hour ORC time on DMR Comments: site, however the log book showed 2 hours. The April DMR showed a downstream sampling time that was off by 1 minute compared to the log book. No other issues were identified during the review. During 2012, sludge was hauled from the facility on several instances. The sludges was hauled by Millers Services and was taken to Sludge Records Reviewed: the City of Taylorsville wastewater facility. The pumping dates were 6-Jan, 13-Feb, 24-Feb, 30-Mar, 15-May, 24-May, 31-May, 14-Jun, 28-Jun, 3-July, 16-Jul, 23-July, 31-July, 3-Aug, and 29-Aug. Logbook appears to contain the operational m required to demonstrate compliance (see exceptions below, if any). March DMR showed one incorrect ORC time on site value (the actual was hour greater than reported ont he DMR), and, the April DMR wed a downstream sampling time that disagreed with the Log Book by one minute. Summary of Review: I Annual Performance Test for Field Parameters: pH: Method 4500 H+B & TRC: Method 4500 CIG. Cornino aH & YSI DO meters both have correction factors noted on meters (current factor is "0").
