Dear Mr. Beacham:
The Division of Water Quality (DWQ) has found typographical errors in the supplement
page of the NPDES permit issued to you on June 7, 2002. Please find enclosed the
revised permit page. The revised page should be inserted into your permit. The old
supplement page may then be discarded. All other terms and conditions contained in the
original permit remain unchanged and in full effect. This permit modification is issued
under the requirements of North Carolina General Statutes 143-215.1 and the
Memorandum of Agreement between North Carolina and the U. S. Environmental
Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit
modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must
be a written petition conforming to Chapter 150B of the North Carolina General Statutes,
filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and
binding.

SUPPLEMENT TO PERMIT COVER SHEET
Beacham Associates, Ltd. is hereby authorized to:
1. Continue to operate an existing 0.100 MGD wastewater treatment system with
the following components:
♦ Dosing Chamber
♦ Dual extended aeration
♦ Dual clarification
♦ Aerobic sludge digestion
♦ Dual high rate tertiary sand filters
♦ Chlorination
♦ Post aeration
♦ Flow recording
The facility is located near the intersection of NCSR 1400 and US Highway 17 at
Beacham's Apartments #2 in Jacksonville in Onslow County.
2. Discharge from said treatment works at the location specified on the attached
map into an unnamed tributary to Brinson Creek, classified SC -NSW waters in
the White Oak River Basin.

In reviewing the following permits, I had some comments or questions
about them I thought you may want to consider.
NC0062359-Horse Creek Farms --Why the TRC limit?
NC0051853-Southgate MHP--why do the instream samples only show 3/wk
during June, July, and August? Why is September not included?
NC0034339-Cabin Creek Campground --on the supplemental page- add an
aerated sludge holding tank to the treatment train description
NC0030431-Hewitt's MHP--on the supplemental page -remove the polishing
pond in the description -it is no longer being used.
NC0032239-Mercer Environmental-Regalwood Plant --Why is there no limit
for NH3 as N. discharges to Nutrient Sensitive Waters ALL other plants
in excess of the 50,000gpd have ammonia and phosphorus limits. Also,
the pH should be 6.8-8.5 not 6.0-9.0 due to the class SC -NSW designation
NC0071706-Hinson Arms Apts.--Cover letter should show ONSLOW County n O�
--not Forsyth.
NC0028215-Beachams #2-supplement page add a dosing chamber to the
treatment train
N00056952-Blue Creek Utilities --Supplement page -add a dosing tank,
splitter box, tertiary filters, clearwell, mudwell, and aerated sludge
holding to the treatment train description. The limits page shows no
PO4 limit. Should indicate a 2.0 limit and the pH should be 6.8-8.5 due
to SC -NSW classification.
