I have reviewed Jeanette's recent email on how to update contact information with DEQ. We currently have the correct
Permit Owner/Legally Responsible Individual as Kip Padgett. However, the email address associated with his name is
incorrect. Do we need to fill out the "Permit Owner Affiliation Designation Form" in order to update his email
address? His correct email address is [e-mail].
We would like to update the permit contact information to myself, Monica Sarna, [e-mail], 919-435-
9442. The contact information for Scott Miles can be removed as he is no longer a Town of Wake Forest employee.
I will email Aziza Selkane to confirm the correct contact information for our billing.
