Attached is the modification to General Permit No. 71584 related to the replacement of Bridge No. 11 over Raccoon
Creek on SR 1338 in Perquimans County. The modification incorporates the utilization of two (2) temporary sheet pile
cofferdams (Cofferdam Phase A and Cofferdam Phase B) and the return to the originally approved alignment of the
subaqueous bore for utilities associated with the bridge replacement project.

If any aspect of this modification request should change from what was initially described and what is shown on
submitted documents, please contact DCM before proceeding. All conditions and stipulations of the active permit remain
in force under this Modification.
