DANIEL SMITH Director Matt Lowder, PE 4004 Barrett Drive, Suite 101 Raleigh, NC 27609 NORTH CAROLINA Environmental Quality October 28, 2019 Subject: Request for Additional Information Stormwater Permit No. SW6190903 Short Stop Gas Station Hoke County Dear Mr. Lowder: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on September 25, 2019. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: Please correct the drainage area table so that the sum of the on -site and off -site drainage areas equal the total drainage area (Section IV, 10). 2. Please provide the original, signed version of the Supplement-EZ form and please include the "Drainage Area" sheet of the form. The Supplement EZ form can be found here: hgps://deq.nc. gov/about/divisions/energy-mineral-land-resources/enemy-mineral-land- permit-guidance/stormwater-bmp-manual (Section V and Section VI, 3). 3. Please provide the missing pages from the O&M Agreement form. Make sure you "Enable Editing" and "Enable Content" on the O&M Agreement spreadsheet. Once the SCMs have been entered into the form, click "Click to Update O&M Manual" to generate the additional pages. Please delineate the wetlands on the plans or provide a note stating that no wetlands exist. Provide documentation of the qualifications and identity of the individual who made this determination (Section VI, 8, m). 5. Please clarify the applicant/property owner relationship. The property owner is shown as "Neal Holdings, LLC" and the applicant is "Li'1 Thrift Food Marts, Inc.". Please also ensure that Section IX is appropriately filled out based on Section III. 6. There appears to be a mismatch in the proposed impervious areas shown in Section IV, 10 and the Supplement-EZ form. Please revise for consistency. 7. Please ensure that the volume of the forebay shown in the Supplement-EZ form matches the plans and provided calculations. 8. Please provide riprap calculations showing that the outlet will not cause erosion during the 10-year storm (General MDC 4). 9. Please show the maintenance access and easement for the SCM on the plans (General MDC 8 & 9). 10. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: https://edocs.deq.nc.gov/Forms/SW Project Submittal While it is not required, it is recommended that an emergency spillway be added to the wet pond. While the wet pond is designed to handle the 100-year storm event, a more intense storm or a malfunction in the outlet could cause embankment overtopping and the pond to fail catastrophically. Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to November 27, 2019, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW6190903 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
