DANIEL SMITH Director Rick Baker, PE 5410 Trinity Road, Suite 102 Raleigh, NC 27607 NORTH CAROLINA Environmental Quality October 25, 2019 Subject: Request for Additional Information Stormwater Permit No. SW4190902 Circle K — R&R 2723188 Forsyth County Dear Mr. Baker: The Division of Energy, Mineral and Land Resources (DEMLR), received a Stormwater Management Permit Application for the subject project on September 26, 2019. A review of that information has determined that the application is not complete. The following information is needed to continue the stormwater review: 1. Please verify your project coordinates as the provided ones do not correspond to the project location (Section I, 4). 2. Section III, 2a & b are not needed since the applicant is also the property owner. 3. Please fill in the total project area (Section IV, 7). 4. Since only 1 SCM is being proposed, the project should only have 1 drainage area, the area draining to the SCM (Section IV, 9). 5. Area bypassed around the SCM should not be accounted for in this table (Section IV, 10). 6. Please provide the original, signed version of both the Supplement-EZ form and O&M Agreement (Section V and Section VI, 3). 7. The deed restriction document is not required since the property is not being subdivided (Section VI, 2 and Section VII). No action is required on your part to address this. 8. Please provide calculations showing how the design volume for the SCM was calculated (Section VI, 7). Please refer to the Stormwater Design Manual, Part B for the Simple Method and Discrete NRCS Curve Number Method for determining design volume. Please delineate the wetlands on the plans or provide a note stating that no wetlands exist. Provide documentation of the qualifications and identity of the individual who made this determination (Section VI, 8, m). 10. Please provide a soils report with the seasonal highwater table (SHWT) elevation and a map showing the soil boring location(s) (Section VI, 9). 11. Section IX is not required since the applicant is the property owner. With your permission, I will place a large red "X" through this section. If this solution is not acceptable to you, please provide a revised application with original signatures in the proper sections. 12. We no longer use the Storm-EZ spreadsheet for stormwater calculations as it does not take the 2017 rules into consideration. 13. There appears to be insufficient impervious area draining to the proposed SCM. Per 15A NCAC 02H .1002(43), "...the volume of stormwater runoff generated from all of the built upon area of a project at build -out during a storm of the required storm depth is treated in one or more primary SCMs...". Since this project already has existing BUA, the net increase in BUA should be treated in an SCM. 15A NCAC 02H .1003(6)(b) allows for variations from this rule provided that, "...the project provides equal or better stormwater control and equal or better protection of waters of the State than the requirements..." 14. A sump is indicated on the Supplement-EZ form but does not appear to be shown on the plans. 15. A dewatering orifice is indicated on the Supplement-EZ form but does not appear to be shown on the plans (General MDC #6). 16. Please show the maintenance access and easement on the plans (General MDC #8 & #9). 17. Supplement-EZ a. Drainage Areas Sheet i. Line 4 — Please indicate the type of SCM used in drainage area 1. ii. Line 7 — Please indicate the new BUA for both columns. iii. Line 10 —Please indicate the new BUA for both columns, broken down by type. The items in line 10 should add up to line 7 for each column. iv. Line 16 — Please calculate the percent BUA for both the entire site and drainage area 1. b. Bay Filter Sheet i. Line 3 — The SCM should be sized to treat all surfaces at build -out. ii. Line 27 — Provide the SHWT clearance. iii. Line 28 — Ensure this value is correct the maximum GPM/ft^2 is 0.5. 18. Provide pdfs of all revisions, 2 hardcopies of revised plan sheets, and 1 hardcopy of other documents. Pdfs must be uploaded using the form at: https://edocs.deq.nc.gov/Forms/SW Project Submittal Please note that this request for additional information is in response to a preliminary review. The requested information should be received by this Office prior to November 24, 2019, or the application will be returned as incomplete. The return of a project will necessitate resubmittal of all required items, including the application fee. Please reference the State assigned project number SW4190902 on all correspondence. If you need additional time to submit the information, please submit your request for a time extension to the Division at the contact below. The request must indicate the date by which you expect to submit the required information. The Division is allowed 90 days from the receipt of a completed application to issue the permit. The construction of any impervious surfaces, other than a construction entrance under an approved Sedimentation Erosion Control Plan, is a violation of NCGS 143-215.1 and is subject to enforcement action pursuant to NCGS 143-215.6A.
