Dear Mr. Smith:
On February 26, 2019, Paul DiMatteo of the Division of Water Resources (Division), Winston-Salem Regional
Office met with Robert Martin, Operator in Responsible Charge (ORC), Josh Younts, Wendy Humphrey -Messer, Patrick
Smith, and Adam Conn to conduct a Compliance Evaluation Inspection of the above referenced Wastewater Collection
System.
This inspection consists of (1) a review of the permit and accompanying documentation to determine
compliance with permit conditions, and (2) an on -site inspection of several collection system components. The
attached inspection form notes the areas that were evaluated for the inspection, with notable findings
outlined as follows.
Site Inspections
1. Pump Stations —The Peach Orchard and Eastern Shores pump stations were inspected.
a. At the Eastern Shores pump station, the visual alarm did not illuminate when tested.
b. No other issues were noted.
High Priority Lines — A high priority line near Peach Orchard Pump Station, and 2 lines near E. Cone Blvd and N.
Church St were inspected. No issues were noted.
Manholes — A manhole near Peach Orchard Pump Station was inspected and no issues were noted.
Permit Conditions
1. Please be sure to document an annual general observation of the entire collection system. A certification
statement is acceptable, and an example statement was provided after the inspection.

Comment: Reviewed documentation for incidents 201601299, 201700315, 201800941 and 201803381; all
seemed in order. As of the inspection Greensboro was also sending us copies of their affidavits

Comment: We pulled the high level float and the alarm didn't come on. However, at the next pump
station
there was about a 10 second delay between the float being pulled and the alarm coming
on. We
didn't wait that long at this pump station. The alarm function should be verified.
