However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on September 14, 2020. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through E, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper copies and one electronic copy submitted to my attention at the address below. Cover Letter: 1. Please include a cover letter expressing the requested permitting action. It is unclear if this is a renewal or if a modification is being proposed. B. Application: 1. Please complete all sections of form DCAR 06-16. Not all required items have been included with the application. 2. Please submit a revised a Residuals Source Certification (FORM: RSC 06-16) for each source generating facility. The application package only includes one source though two were previously permitted. 3. If only one residual source generating facility is proposed, please confirm and specify the dry tons per year requested in this renewal. 4. If no changes to what is currently permitted are proposed, please submit a Residuals Source Certification (FORM: RSC 06-16) for each site proposed in this renewal. Please also include the dry tons per year requested for each source. 5. The proposed dry tonnage per year from all sources is zero. If no changes to the currently permitted 60 dry tons per year, please provide confirmation and complete the RSC Summary table appropriately. C. Residuals Source Certification: 1. Please complete all sections of this form and submit a separate form for all residual sources. 2. Specifically, please address II.7. Pathogen Reduction (FORM: RSC 06-16). Per 15A NCAC 02T.1106, residuals must meet the pathogen reduction requirements. Please provide analytical data to support the specified pathogen reduction requirement. 3. Also, please address II.8. Vector Attraction Reduction (FORM: RSC 06-16). Per 15A NCAC 02T.1107, residuals must meet the VAR requirements. Please provide analytical data to support the specified VAR requirement. Mr. Bob Pudney August 13, 2020 Page 3 of 3 D. Program Information: a. Please provide a product label or an information sheet as provided to the person who receives the processed residual. This information is required by Instruction D on Form DCAR 06-16. b. The label or information sheet shall contain, at a minimum, the following information: a. Name and address of the person who prepared the residual that is sold or given away. b. A statement that application of the residuals to the land shall be in accordance with the instructions on the label or information sheet. c. A statement that the residuals shall not be applied to any site that is flooded, frozen or snow- covered. d. A statement that adequate procedures shall be provided to prevent surface runoff from carrying any disposed or stored residuals into any surface waters. e. Information on all applicable setbacks in accordance with 15A NCAC 02T A 108(b). f. A statement that the residuals shall be applied at agronomic or recommended rates for intended uses. E. Operation & Maintenance Plan: 1. Please submit a copy of the Operation and Maintenance Plan as part of the application package. This information is required by Instruction H on Form DCAR 06-16. 2. This plan should include at minimum: a. Operational functions; describe the operation of the program to show what operations are necessary for the program to function and by whom the functions are to be conducted. b. Maintenance schedules; may include equipment calibration, maintenance of signs, etc. c. Safety measures; may include safety training program, manuals, signs, etc. d. Spill response plan; including control, containment, remediation, emergency contact information, etc. e. Inspection plan including the following information; f. Names and titles of personnel responsible for conducting the inspections. g. Frequency and location of inspections, including those to be conducted by the ORC, and procedures to assure that the selected location(s) and inspection frequency are representative of the residuals management program. h. Detailed description of inspection procedures including record keeping and actions to be taken by the inspector in the event that noncompliance is observed. i. Sampling and monitoring plan including the following information; j. Names and titles of personnel responsible for conducting the sampling and monitoring. k. Detailed description of monitoring procedures including parameters to be monitored. 1. Sampling frequency and procedures to assure that representative samples are being collected. The sampling plan shall account for any foreseen fluctuations in residuals quality and indicate the most limiting times for residuals to meet pathogen and vector attraction reduction requirements.
