Subject: Notice of Violation - Tracking No. NOV-2020-DV-0324
Dear Mr. Basinger,
This letter is a follow-up to our Notice of Violation of the above referenced NOV.
1416 Branch Ave (Incident Number 202001801):
• Incident occurred on 06-04-2020 and ended the same day.
• Pursuant to NC G.S. 143-215.1C, the City of Kannapolis gave notice of an
untreated wastewater spill to surface waters on 06-05-2020.
• The SSO was caused by pipe failure.
• We immediately dispatched a crew out to the location to inspect.
• The impacted area near the affected line failure was flushed with fresh water, raked
and lime spread.
• Restoration of the site was completed as soon as the pipe had been replaced. City
staff replaced 50 ft of sewer pipe.
• We performed CCTV of the sewer line and found no major deficiencies in the
immediate area on 07-14-2020. We are working with our Engineering Department
to determine what upgrades can be made.

® Incident occurred on 06-11-2020 and ended the same day.
® The SSO was caused by debris and grease in the line.
® We immediately dispatched a crew out to the location to inspect.
• The impacted area near the affected line failure was flushed with fresh water, raked
and lime spread.
® Restoration of the site was completed as soon as the blockage was cleared.
® We performed CCTV of the sewer line and found no deficiencies in the immediate
area on 06-16-2020.
