Since mid December 2013 the system has been converted to subsurface discharge and there has been no effluent discharge. The owner wants to keep the permit active for a while longer. I am not exactly sure what (and where) to write on the DMR & the DWQ Form MR-3 to indicate no discharge. Skvarla, III Governor Director Secretary November 25, 2013 Mr. Gerald Greeson Director of Maintenance Guilford County Schools 3920 Naco Road Greensboro, NC 27401-4379 SUBJECT: Compliance Evaluation Inspection Nathanael Greene Elementary School WWTP NPDES Permit No. NCO038164 Guilford County Dear Mr. Greeson: On November 21, 2013, Mike Mickey of this office met with Rick Myers and Dave Hicks, ORC, to perform a Compliance Evaluation Inspection on the wastewater treatment system serving Nathanael Greene Elementary School. The inspection found the recirculating surface sandfilter system to be compliant with the NPDES permit. The inspection findings are detailed below and an inspection checklist is attached for your records. I. Permit It was noted that installation of the new subsurface drip irrigation system to serve Nathanael Greene Elementary was almost complete. This will allow the existing surface water discharge to be eliminated so that the NPDES permit can be voided. Attached is a NPDES Permit rescission form. Once the discharge is ceased, please complete and submit to the Raleigh address listed on the form. II. Self -Monitoring Program The monthly self -monitoring reports (DMR's) were reviewed for the period January 2013 through September 2013. The recirculating surface sandfilter was found to be in complete compliance with all NPDES permit effluent limits and monitoring requirements for the above review period. III. Flow Measurement The NPDES permit requires weekly instantaneous flow reporting. Flow values are obtained from the in - line totalizing meter on the UV discharge line. Sludge Handling and Disposal Solids are typically removed from the septic tanks in December and at the end of the school year by Jenkins Waste Management. V. Operations and Maintenance The facility appeared to be competently operated and maintained. The issued noted with ponding on the sandfilter in the 2012 inspection had been remedied. VI. Facility Site Review The existing 0.0045 MGD system consists of the following: two septic tanks, dosing tank for initial dose through the large pipes over the surface sandfilter, recirculation tank with dual pumps for dosing with spray nozzles, dual UV disinfection units, in -line totalizing flow meter and effluent discharge pipe. VII. Records/Reports A review of the facility files indicates that all records are being maintained according to the permit. A comparison of the August 2013 self -monitoring report with the operator's daily log and corresponding laboratory data sheets from R&A Labs showed that all values were transcribed onto the DMR correctly. VIII. Effluent/Receiving Waters The system discharges into a tributary of North Prong Stinking Quarter Creek, class "WS-V NSW*" waters in the Cape Fear River Basin. The effluent is discharged to a ditch across Bowman Dairy Road. Our office appreciates your efforts to comply with the NPDES permit. (If units are separated) Does the chart recorder match the flow meter? Comment: Flow values are obtained from the in -line totalizing meter located on the UV discharge line. Effluent Pipe Is right of way to the outfall properly maintained? Are the receiving water free of foam other than trace amounts and other debris? If effluent (diffuser pipes are required) are they operating properly? Comment: Effluent samples are collected from a spigot in the UV building. The effluent then passes through the old chlorine contact chamber before being discharged across Bowman Dairy, Road into a cow pasture. The effluent eventually reaches a tributary to the N. Prong of Stinking Quarter Creek, class "WS-V; NSW" waters in the Cape Fear River Basin. This type of inspection consists of two basic parts: an in office file review and an on -site inspection of the treatment facility. The attached EPA inspection form notes the eight (8) areas that were evaluated. The findings and observations are outlined below: I. Permit The NPDES permit was recently reissued with an effective date of July 1, 2011. II. Self -Monitoring Program The monthly self -monitoring reports (DMR's) were reviewed for the period January 2011 through December 2011. The sandfilter system was found to be complaint with all NPDES, permit effluent limits and monitoring requirements for the above review period, except for a single NH3-N monthly average limit violation that occurred in September (see data summary attached). The wastewater plant at Nathanael Greene Elementary School runs intermittently, shutting down during the summer vacation months of school. September is the first full month of operation after summer vacation. Past records of plant performance in September are unremarkable. However, lab results for September 2011 seem to indicate a high strength waste may have been dumped into the system (BOD & TSS levels increased also). No daily maximum Nitrogen limits were violated, but the monthly average for Ammonia Nitrogen exceeded the permit limit by 1.4 mg/L. We have taken several steps to prevent reoccurrence of this problem. Process control tests for ammonia nitrogen have been implemented to decrease reaction time to this potential problem. Problems with quality and timely delivery of effluent lab results in September slowed response to this problem. We have also implemented system pH control at @ 7.OSU to improve nitrification. Current plant observations indicate the system is actually producing better than normal effluent quality (see attached graph). The root cause of the upset could be misuse of custodial cleaning chemicals; however, we have no way to prove that this was anything other than a plant upset at startup. David Hicks, ORC and Rick Myers were present for the on -site inspections. The inspections consisted of two parts: an on -site inspection of the treatment facilities and an in -office file review. The following are the findings from the subject inspections. I:. `Permit The NPDES permit for Nathanael Greene Elementary School became effective on July 1, 2006 and expires on June 30, 2011. II. Records/Reports A daily visitation log is maintained by the ORC. Information contained in this daily log includes field parameter data, process control data, and sampling data. Enclosed please find an ORC Designation form. In an attempt to update our database, we are asking all facilities who have made an ORC change in 2010 to complete and return this form. Thank you in advance for your, prompt attention to this matter. Also, please submit the Annual Compliance Report for calendar year 2009. This office has no record of a submittal for 2009. III. Facility Site Review The wastewater treatment facility was consistent with the permit description. The system consists of a septic tank; dosing tank, sand filter, disinfection (UV with chlorine back-up), dechlorination and effluent discharge. IV. Effluent & Receiving Waters Effluent on the date of inspection was observed to be free of visible solids and foam. The receiving stream was also free of visible solids, foam and debris on the date of the inspection. The school discharges into an unnamed tributary to North Prong Stinking Quarter Creek (Class C-NSW waters in the Cape Fear River Basin), V. Flow Measurement Flow is measured from the water meter readings. VI Self -Monitoring Program A review of the discharge monitoring reports (DMRs) for the time period of June 2009 through May 2010 showed that the facility experienced permit effluent limitation violations. The Strategy includes a Wastewater Discharge rule (T15A NCAC 02B .0270) that applies to your wastewater treatment facility. Pursuant to Sub -Item (6) (d) of the rule, I am writing to notify you of the nutrient allocations assigned to your facility. This letter addresses only certain provisions of the Jordan Lake Wastewater Discharge rule. I encourage you to familiarize yourself with the remainder of this rule. Nutrient Allocations. The Wastewater Discharge rule establishes the maximum loads of nitrogen and phosphorus that can be released from wastewater treatment facilities into the Jordan Lake watershed. The rule provides that these wasteload allocations are to be divided among the existing facilities in proportion to (with specific exceptions) their 2001 permitted flows. It also provides that facilities permitted to discharge 100,000 gpd or more are to receive nitrogen and phosphorus limits equivalent to their individual allocations. Your facility has been assigned nitrogen and phosphorus allocations. The allocations are expressed as - annual mass loadings delivered to the lake. Due to foreseeable losses between the discharge point and the lake, they are also expressed as the equivalent discharge loads at the point of discharge. However, any nutrient limits already in -your NPDES permit will remain in effect. If the facilities permitted for less than 100,000 gpd, as a group, approach their allocations for nitrogen or phosphorus, the Division will consider whether nutrient limits are necessary to protect water quality at that time. In the event that nutrient limits become necessary, the limits would equal your facility's discharge allocations. Nutrient Trading. The rule provides that, when one utility connects to another, the allocations held by the closing facility are transferred to the remaining facility. NCO038164 Guilford County Dear Mr. Greeson: A Compliance Evaluation Inspection was performed on the subject wastewater treatment plant on September 26, 2007 by Corey Basinger of the Winston-Salem Regional Office. Rick Myers and Sammy Sockwell were present for the on -site inspections. The inspections consisted of two parts: an on -site inspection of the treatment facilities and an in -office file review. The following are the findings from the subject inspections. I. Permit The NPDES permit for Nathanael Greene Elementary School became effective on July 1, 2006 and expires on June 30, 2011. Il. Records/Reports A daily visitation log is maintained by the ORC. Information contained in this daily log includes field parameter data, process control data, and sampling data. The Annual Compliance Report for the school was submitted on April 23, 2008. Enclosed please find an ORC Designation form. In an attempt to update our database, we are asking all facilities to complete and return this form. Thank you in advance for your prompt attention to this matter. III. Facility Site Review The wastewater treatment facility was consistent with the permit description. The system consists of a septic tank, dosing tank, sand filter, disinfection (UV with chlorine back-up), dechlorination and effluent discharge. IV. Effluent & Receiving Waters Effluent on the date of inspection was observed to be free of visible solids and foam. The receiving stream was also free of visible solids, foam and debris on the date of the inspection. The school discharges into an unnamed tributary to North Prong Stinking Quarter Creek (Class C-NSW waters in the Cape Fear River Basin). V. Flow Measurement Flow is measured from the water meter readings. VI. Self-Monitorin Program A review of the discharge monitoring reports (DMRs) for the time period of July 2007 through April 2008 showed that the facility experienced several permit effluent limitation violations. Notices of Violation or Civil Penalty Assessments were previously issued by this Office for these violations. Compliance Schedules Not applicable. VIII. Laboratory All of the samples are collected and analyzed by Meritech, Inc. IX. Operations and Maintenance No compliance issues were noted with respect to facility operations. Maintenance is performed as needed. It was noted that a couple of the spray nozzles were not functioning properly at the time of inspection. These should be repaired or replaced prior to the start of the upcoming school year It was also noted that during a heavy rain the water in the pump tank becomes cloudy and somewhat muddy. This is most likely due to infiltration into the filter bed or around pipe joints. This will directly affect the effectiveness of the UV system. Please keep close check on this issue and report and significant changes to this Office. X. Sludge Utilization/Disposal The septic tanks and grease tanks are pumped out twice per year at a minimum. Under some circumstances, additional pumping may take place. Records are kept of all pumping activities. Jenkins Septic Tank Service is responsible for the pumping. XI. Pretreatment Stormwater, Pollution Prevention, & Multimedia Not applicable for this facility XII. Sewer Overflow No sewer overflows were reported during the review period. XIII. Authorization is hereby granted for the construction of modifications to the existing Nathaniel Greene Elementary School WWTP, with discharge of treated wastewater effluent into North Prong Stinking Quarter Creek in the Cape Fear River Basin. This authorization results in no increase in design or permitted capacity and is awarded for the construction of the following specific modifications: Installation of a dechlorination system utilizing sodium sulfite pursuant to the fast track application received on December 13, 2007, and in conformity with the Minimum Design Criteria for Dechlorination Facilities. This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No. NCO038164 issued May 22, 2006, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NCO038164. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as the construction of additional or replacement wastewater treatment or disposal facilities. Such notification to the regional supervisor shall be made 'during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Pursuant to 15A NCAC 2H .0140, upon completion of construction and prior to operation of these permitted facilities, the completed Engineering Certification form attached to this permit shall be submitted to the address provided on the form. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Pennittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each Class I facility at least weekly and each Class I1, IlI and IV facility at least daily, excluding weekends and holidays, must properly manage the facility, must document daily operation and maintenance of the facility, and must comply with all other conditions of T15A:8G.0202. A copy of the approved plans and specifications shall be maintained on file by the Permittee for the life of the facility. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. You are reminded that it is mandatory for the project to be constructed in accordance with the North Carolina Sedimentation Pollution Control Act, and, when applicable, the North Carolina Dam Safety Act. In addition, the specifications must clearly state what the contractor's responsibilities shall be in complying with these Acts. Prior to entering into any contract(s) for construction, the recipient must have obtained all applicable permits from the State. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this Authorization to Construct does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction.
