The following items must be addressed before we can complete our review: 1) On February 1, 1993, the Division adopted new regulations that require that the infiltration/exfiltration rate must not exceed 100 GPD per inch of pipe diameter per mile of pipe. All sewer extension permit applications received on or after February 1, 1993, must conform to these new regulations. Please revise page 4 of your gravity sewer application to reflect these changes. Please be advised that the Division is in the process of revising the application forms to reflect the new regulations. 2) In accordance with 15A NCAC 2H .0219 (h) (4) and (5), please provide a screened vent for the wet well. Please make the appropriate revisions on the applicable sheet(s) of your plans and forward 3 complete signed and sealed copies of each revised page to my attention. 3) Please revise the following sections of the specifications to comply with the Division required infiltration rate of 100 GPD per inch of pipe diameter per mile: 1) page 02650-1 and 2) page 02655-2. If any other sections of the specifications are also in error, please revise and forward 3 complete signed and sealed sets of specifications to my attention. 4) The Division requires that at all stream crossings, either the top of the pipe be below the stream bed (at least three feet unless ferrous material pipe is used) or that the bottom of the pipe be above the 25 year flood elevation. Please provide the 25 year flood elevation on sheet 5 of your plans to confirm compliance with this requirement. If 25 year flood information is not available, or if the aerial crossing is not above this elevation, please provide written confirmation from the applicant that no damage will be incurred by upstream or adjacent properties due to this aerial crossing, and that the crossing will be properly inspected and maintained. If necessary, please provide 3 signed and sealed copies of the revised plans to comply with the above requirements. P.O. After recalculating, please submit 3 copies of page 5 of the application (with number 11 revised) to my attention. 6) In accordance with 15A NCAC 2H .0219 (1) (1), the Division requires that flow estimates be calculated using the methods outlined in 15A NCAC 2H .0219 (1). Please recalculate your flow estimate using the approved values and submit 3 copies of the revised page 2 to my attention. Refer to the subject permit application number when providing the requested information. Please submit three (3) copies of all information to my attention at the address below. Also, please note that failure to provide this additional information on or before August 11, 1994s will subject your application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
