Dear Mr. Cranston:
Division of Water Resources' Central and Regional staff has reviewed the application package
received June 8, 2020. However, additional information is required before the review may be completed.
Please address the items on the attached pages no later than the close of business on September 1, 2020.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
listed on the attached pages, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.
Please reference the subject application number when providing the requested information. All
revised and/or additional documentation shall be signed, sealed and dated (where needed), with two paper
copies and one electronic copy submitted to my attention at the address below.

The Division has reviewed Louisiana-Pacific Corporation's permit rescission request. At this
time, the former dedicated land application sites permitted under WQ0000461 have
outstanding groundwater standards exceedances (as specified by 15A NCAC 02L .0202).
Subsequently, permit rescission is denied at this time.
The Permittee shall continue to comply with all permit conditions and requirements until the
waste treatment systems authorized by the permit are properly closed or subsequently
permitted by another permit issued by the appropriate permitting authority for that activity.

In light of this, the renewal application package must address the following items.
A. Cover Letter:
1. Please describe Louisiana Pacific Corporation's intent to distribute bagged and/or bulk residuals.
Alternatively, if residuals will not be distributed, please comment on the disposal method.
B. Annual Fee:
1. Please note, this permit cannot be renewed without receipt of the Annual Fee. This permit will be
billed for the 2020 Annual Fee on September 1, 2020.
C. Application:
The application must be signed by a principal executive officer of at least the level or vice-president
or his authorized representative. The application was signed by the Plant Manager, who has not
been delegated as a permit or application signature authority. Delegation of authority to sign permit
applications to a specific employee, or any employee in a specific position must be provided in
writing to the Division and signed by an authorized person. The delegation may be for a specific
permit application or for certain or all types of water quality permits; the letter shall identify the
extent of delegation. [ 15A NCAC 02T .0106]
D. Site May:
A more detailed map of the site is needed. Please include the following minimum items:
a. A topographic map with contours not exceeding 10 feet or 25% of the total site relief and showing all
facility related structures and fences within the land application area.
b. The location of all wells (including usage and construction details if available), streams (ephemeral,
intermittent, and perennial), springs, lakes, ponds, and other surface drainage features within 500 feet
of land application sites.
c. Setbacks as required by 15A NCAC 02T .1108
d. Site property boundaries within 500 feet of all waste treatment, storage, and the former disposal sites.

E. Operation & Maintenance Plan:
1. The Plan submitted with the application describes land application of residuals generated by the
wastewater treatment plant. If land application is no longer occurring, provide commentary on any
changes to the Operation and Maintenance Plan. Please include all updates to the operation and
maintenance of the facilities and the residuals program.
F. Groundwater Monitoring Plan:
Please submit an inventory of all groundwater monitoring wells currently and formerly under
Permit No. WQ0000461. This should include:
a. Location and status (Active/Inactive) of all monitoring wells. Coordinates should be reported
to 6 decimal degrees. Please also include any synonyms or alternate well names.
b. Provide any information on the construction of wells (depth, casing, screen, date of
installation, etc.)
c. Information on the field/application area each well was serving. Please indicate the position
(gradient) of the well in relation to the application area. For wells located at the review and
compliance boundaries, please specify along with the appropriate field number.
2. Several wells at dedicated fields continue to have 15A NCAC 02L .0202 groundwater standards
for exceedances in nitrate.
Please provide a detailed summary report of the groundwater monitoring program. This report
shall address the nitrate exceedances. This summary shall discuss groundwater compliance for both
previously and currently used land application fields as well as address any measures taken to
mitigate outstanding groundwater exceedances.
