Subject: Improvements to SR 1340 (Pruitt Rd.) in Lenoir County, State Project No. 2C.054015.

Dear Mr. Johnson:
The NC Division of Water Quality has reviewed your submittal for a 401 Water Quality Certification and Neuse
Buffer Authorization for the aforementioned project. Review of your application revealed it lacking necessary
information required for making an informed permit decision. The permit application was deficient in the following
areas:
Stormwater Management Plan. A Stormwater Management Plan is needed for this project. Please revise
Section E of the PCN to include the necessary information. The description should include the use of any stormwater BMPs(?ass swales, re-formed scour holes, etc.) to the Maximum Extent Practicable (MEP)
in compliance withTJCDDT's NI?DES permit No. 000250 Post Construction Stormwater Program.
Stream Impacts. Stream Impacts resulting from the two pipe replacements were not included in the PCN.
Please revise Section C(3) to include any impacts to streams resulting from the pipe installations.
Plan Sheets not included. Please provide plan sheets for the pipe installations, including longitudinal and
cross sectional profiles.
Therefore, pursuant to 15A NCAC 2H .0507(a)(5), we will have to place the permit application on hold until we are
supplied the necessary information. You have 21 days to respond in writing with the requested information or
notification to this office that the information is forthcoming. If, at the end of the 21 days, this office has not
received this information in writing, we will assume you are withdrawing your application and it will be returned.
Furthermore, until the information is received by the NC Division of Water Quality, we request (by copy of this
letter) that the US Army Corps of Engineers place the permit application on hold.
