Please accept this letter to resend permit number NCG550332, Forsyth County, 5910 Twin Meadows
Drive, Pfafftown, NC for a subsurface sand filter.
As ofJune 12, 2020, the subsurface sand filter has been disconnected from the dwelling at 5910 Twin
Meadows Drive. The discharge point of the sand filter has been capped and the sewer service has been
replaced by a "tap in" to the newly installed sewer line by the City of Winston-Salem NC. Enclosed you
should find a copy of the paid invoice for the connection to the sewer line in addition to a Job Summary
from the Winston-Salem/Forsyth County Planning& Development Services.
