Are the design, maintenance and operation of the treatment facilities adequate for the type of waste and disposal system? ® Yes or ❑ No Note: The screening device at the head of the WWTP needs repair to prevent trash from enteringthe he system and ending up in the residuals for land application. ORC indicated that parts have been ordered and repaired soon. 3. ADDITIONAL REGIONAL STAFF REVIEW ITEMS WSRO staff made a site visit to the subject facility on July 7, 2020 for a review associated with application for permit renewal with no modifications. Mr. Tim Church accompanied staff on the visit. Below is a summary of notes from the review. • Footnote 2. in the existing permit Attachment B needs to be removed. This was a typo that provided exemption from TCLP sampling. TCLP could be listed as a minimum frequency of once per 5 years based on prior testing. Frequency must be either once per year or once per permit cycle. • Fields (hay and pasture fields) were found to have good vegetative cover present. • The site map for Site No. 3 (Fred Walters) has been modified to add a new area to the northwest along the property line. This area is unsuitable for land application due to slopes and a large head slope drain running through the center of the area. A revised site map is needed for this site. • No other changes were noted to site maps and no changes were noted to properties surrounding the application fields. • WSRO staff to follow-up on the screening device at the head of the WWTP to ensure repairs are made to prevent land application of trash that makes its way through the plant into residuals.
