Facility: Flakt Products Remediation Site NPDES Permit #:' NC0085871, Forsyth County Dear Ms. Christopher: Donald R. van der Vaart Secretary Ron Boone of the Winston-Salem Regional Office of the NC Division of Water Resources (DWR or the Division) conducted a compliance evaluation inspection (CEI) of the Flakt Products Remediation Site on August 18, 2015. The assistance and cooperation of Glenn Price, Operator in Responsible Charge (ORC), was greatly appreciated. An inspection checklist is attached for your records and spection findings ings are summarized below. General Information The Flakt Products Remediation Site is located at the former Flakt Products manufacturing facility at 2000 Lowery Street, Winston Salem, Forsyth County, NC. The approximate coordinates of the treatment building are 36.0917430N,-80.2153770W. It is located on the southwest corner of the facility. The permit authorizes Flakt Products, Inc. to operate this 0.0864 MGD groundwater remediation system, which consists of extraction wells, an iron sequestering feed pump, an equalization tank with feed pump, an air stripper, and a flow monitoring device. The permit authorizes Flakt Products, Inc. to discharge the treated effluent into Brushy Fork via outfall 001. Brushy Fork is currently classified as Class C waters in the Yadkin Pee -Dee River basin. Site Review , Mr. Price has done a good job of operating and maintaining the plant. The entire plant was clean and appeared to be well maintained. The right-of-way to the outfall was clear and ready for inspection. General Information The Flakt Products Remediation Site is located at the former Flakt Products manufacturing facility at 2000 Lowery Street, Winston Salem, Forsyth County, NC. The approximate coordinates of the treatment building are 36.0917430N,-80.2153770W. It is located on the southwest corner of the facility. The permit authorizes Flakt Products, Inc. to operate this 0.0864 MGD groundwater remediation system, which consists of extraction wells, an iron sequestering feed pump, an equalization tank with feed pump, an air stripper, and a flow monitoring device. The permit authorizes Flakt Products, Inc. to discharge the treated effluent into Brushy Fork via outfall 001. Brushy Fork is currently classified as Class C waters in the Yadkin Pee -Dee River basin. Site Review Mr. Price has done a good job of operating and maintaining the plant. The entire plant was clean and appeared to be well maintained. Mr. Boone noted no discrepancies or permit violations. The site is vacant and the contamination resulted from metal fabricationactivities and the use of solvents at two degreasing baths and a 7000 gallon above ground storage tank. Site assessments showed that trichlororethene and/or 1,1,1—trichloroethane were released at some time during the operation of the facility. In -situ volatilization was used to remediate soil. Soil clean-up ceased in 1/93 when VOC concentrations of soil were reduced below specified cleanup concentrations. In 11/97 an ATC was issued to address the groundwater contamination. The proposed treatment system consisted of about 60 extraction wells, iron sequestering (citric acid) feed pump, 1000 gallon equalization tank, transfer pump, air stripper, bag filters, granular activated carbon filters, flow monitoring device, and a 1500 foot outfall. The bag and GAC filters were to be installed if the effluent failed to meet TSS or toxicity permit limits. The consultants at MACTEC do not believe the filters were ever needed. The current system consists of 7 extraction wells, iron sequestering, equalization and high pressure air strippers. Records document that the original permit was issued in January 1997 and the groundwater treatment facility started operation in 7/98. A modification request was received in 1998 to increase flow from 0.0288 MGD to 0.0864 MGD. The permit was renewed in August 1999 with the increase in flow as requested and an expiration date of 5/31/04. The facility asked to NPDES PERMIT FACT SHEET Page 2 NPDES No. NCO085871 have the monitoring frequency reduced and in 1/2000 a new Effluent Limitations and Monitoring page was issued reducing all parameter monitoring from monthly to quarterly. Flow remained as a continuous recording, influent or effluent. The permit was renewed again in June 2004 with an expiration date of May 31, 2009. No changes were made to the permit except in the wording of the Chronic Toxicity Permit Page, Section A. (2.). The facility discharges to Brushy Fork, a Class C stream, in the Yadkin -Pee Dee River Basin. Brushy Fork is not on the 303(d) list for. impaired streams. Brushy Fork has a 7Q10 flow of 2.9 cfs and joins with Salem Creek approximately 1,500-ft downstream. DMR Data, Compliance and Permit Developement Initial samples taken during the first year of remediation detected the following parameters in the waste - stream: 1, 1 -dichloroethene, zinc, copper, lead and iron. FeREMEDE, a non -toxic iron -sequestering agent is currently used to reduce iron coagulation and clogging. This facility is not subject to any Categorical Effluent Guideline Limitations but is subject to the DENR, Division of Waste Management, Superfund Section, Inactive Hazardous Sites Branch, Implementation Guidance published in August 2008. This means the facility must meet "Remediation Goals for Groundwater" which are essentially safe drinking water standards before it can stop the remediation process. VOC's must be monitored by the facility as part of this requirement. A compliance inspection was performed on March 12, 2008 by Winston-Salem Regional personnel and operations & maintenance, record keeping, monitoring, site review and flow measurement were all found to be satisfactory. The only parameter limited in the permit is TSS with a daily maximum of 30 mg/L. TSS has been below detectable levels for the last five years. Chronic Toxicity • Since its initial operation in 7/98 the facility has consistently passed its toxicity limit with only one exception. One NOV was issued during the first permit cycle for a toxicity violation in 1999. It is recommended that chronic toxicity sampling remain in the permit. Metals and RPA • A review of the last two permit cycles, Aug. 1999 through March 2009, show the maximum concentration of all detected metals never exceeded standards or action levels. Metals detected during the previous permit cycles included zinc, copper, lead, manganese and iron. DMR's reviewed for the most recent permit cycle found only copper and manganese in detectable levels. All other metals, arsenic, iron, lead, and zinc, were below detection levels for five years. There is no reasonable potential for a metals violation for As, Fe, Pb or Zn. It is recommended that monitoring for these metals be removed from the permit. A Reasonable Potential Analysis performed on copper and manganese resulted in maximum predicted -levels of 0.0002 ug/L for Cu and 0.05 ug/L for Mn "(see RPA attached). Therefore, copper and manganese have no reasonable potential for a standards violation and should be removed from the permit. The only water quality based standard for manganese is for a water supply classified stream which has a limit of 200 ug/L. Brushy Fork is not a water supply classified stream but the RPA show_ s manganese has no potential to violate this standard. Organics • No organic contaminants have been detected in effluent samples (below detection levels) taken since Aug. 1999. Quarterly monitoring for particular organics should remain in the permit due to the nature of the discharge. Mr. Glen Price, ORC, and Joe Deatherage, Senior Engineer of MACTEC Engineering, were present for the inspection. The inspection consisted of two parts: an on -site inspection of the treatment facility and a file review. The following are the findings from. the subject inspection: The facility was found to be in compliance. Permit The current permit became effective June 1, 2004 and expires on May 31, 2009. A renewal application has been received by the Division. The remediation site has a permitted flow of .0864 MGD. Operations and Maintenance Chemicals are stored indoors in clearly labeled barrels. Maintenance is performed as needed. There is a routine maintenance program in place. • At the time of the inspection the facility was not in operation. Repairs were being made to fix a plumbing leak. Record Keeping A review of laboratory reports and Discharge Monitoring Reports (DMRs) revealed no discrepancy between the generated data and the data submitted to DWQ. A copy of the laboratory data and historical flow charts are kept on file. Maintenance records are available for review. Self -Monitoring Program There were no NPDES limits violations from January 2008 thru December 2008. Facility Site Review Facilities are well maintained.
