This form is to only to be used by the current permittee to notify the Division of a minor
modification. Pursuant to Rule 15A NCAC 02H.1002, a minor modification is a change to the project that:
1) does not increase the net built-upon area; and/or
2) does not change the size or design of the SCM(s).

There has been a modification to the Drainage Area contributing to both Stormwater Ponds. No
Additional BUA is being directed at the Stormwater Ponds. Permanent diversion ditches were
Adjusted on Lots 16-20 as well as 22 &23. Additionally, a Landscape Berm has been added in the
100' Right-of-Way Buffer to allow for topsoil waste on site, adding additional DA to BMP#1.
